Objective: To explore the effects of home care in the elderly patients under the epidemic of According to the hospital deployment and guidance documents, a series of actions were worked out, including the home care service process and contingency plan, training of the knowledge of novel coronavirus pneumonia (NCP), providing psychological intervention for nurses, providing home care for elderly patients at home and education about home prevention of COVID-19 for the elderly family. Results: From January 19 to March 31, 2020, the home visiting nurses provided home care service for 93 elderly patients at home, representing a 20% increase over the same period last year. The home care service included retention / replacement of gastric tube, retention / replacement of urinary tube, pressure ulcer care, sputum suction care, maintenance of peripherally inserted central catheters and venous blood collection for 127 times. The home visiting nurses were in good physical condition without any bad mood. Conclusion: Under the epidemic of COVID-19, more home-based elderly patients had home care service needs. Home care could not only take advantage under the epidemic situation, but also meet the medical needs of elderly patients at home, and effectively alleviate the burden of epidemic prevention and reduce the risk of cross infection. Home care could play a great role in prevention and control of the COVID-19 epidemic.
Objective: To explore the application of "Internet plus nursing service" program under the pandemic of novel coronavirus pneumonia (NCP). Methods: Nurses of Transitional Care Department of the author's hospital assessed and categorized adequately the patients who applied for services online and used a combination of online and offline approaches to provide nursing care to patients at home. Results: During the pandemic of novel coronavirus pneumonia from January 19 th to March 31 st , Transitional Care Department adopted the "Internet plus nursing service" mode to provide nursing services for 357 cases of the in home patients, which was approximately three times the number of the same period in last year. As a result of a comprehensive evaluation, 248 cases of patients' medical care problems were solved through online educational guidance and consultation; 101 cases of patients were served by home-based services; and 8 cases of patients were recommended to receive professional medical treatment in hospital. There were no patients admitted to the hospital for reasons such as changes in their conditions through follow-up visits, and home visit nurses were in good health. Conclusion: In-home patients have greater demands for the "Internet plus nursing service" due to the pandemic of novel coronavirus pneumonia. Applying the "Internet plus nursing service" mode to serve patients can not only meet the medical needs of in-home patients, but also effectively relieve the pressure of hospital treatment. Moreover, it also reduces the risk of cross-infection by decreasing the number of staffs for mass gatherings, which plays a paramount role in control and prevention of the novel coronavirus pneumonia prevention.
Objective To investigate the current status of the knowledge, attitude and practice (KAP) of primary home caregivers of patients with Alzheimer's disease (AD). To propose targeted intervention measures to improve home caregivers' ability of caring, by analyzing influencing factors. Methods 76 family caregivers of patients diagnosed as AD in a third-class hospital in Guangzhou were selected for this survey of convenient sampling. These patients were followed up at home after discharge from the hospital, and their main family caregivers were investigated by knowledge, attitude, practice questionnaire. Results The total scores of knowledge, attitude and practice of the home caregivers of AD patients respectively were (. There were statistically significant differences between the self-care ability of patients, the occurrence of AD symptoms and the knowledge score of caregivers (P < 0.05). The age of the caregiver was negatively correlated with attitude. The younger caregiver obtained the higher attitude score. And more knowledge about the patient's condition resulted in higher practice score and care ability. Conclusions The scores of home caregivers of patients with AD remain low in knowledge, attitude and practice, so it is necessary to strengthen the health education and care guidance for these caregivers. In this way, the caregivers can obtain more social support to improve their ability to care for patients.
Objective: The concept of discharge preparation services originated from the continuum of care and referral system first established in the United States in 1910. Extended care is a series of nursing care activities to ensure continuity and coordination of care for patients in different health care settings. Guidance to reduce or prevent the deterioration of the health status of patients with chronic diseases. The aim of our study was to evaluate the efficacy of discharge preparation service and extended care in elderly patients with aspiration pneumonia. Methods: A total of 40 elderly patients with aspiration pneumonia who were hospitalized in the respiratory and critical care department of a tertiary care hospital from February 2017 to February 2019 were enrolled in this study and divided into two groups. Patients in the control group received conventional nursing interventions while patients in observation group received specific chronic disease management including discharge preparation service and continuity of care. Patients' satisfaction with nursing services, their ability to take care of themselves in daily life one month after discharge, and the number of hospital readmissions within one year after discharge were documented and compared between two groups. Results: Our study suggested that patients' satisfaction with nursing services and their self-care capabilities one month after discharge were significantly higher in the observation group compared to the control group (P<0.05). Meanwhile, the number of readmissions within one year after discharge was significantly decreased in the observation group than in the control group (P<0.05). Conclusion: The chronic disease management model including specific discharge preparation service and extended care provided patients with better nursing services and resulted in better prognosis.
