Indigenous populations around the world experience a disproportionate burden in terms of oral diseases and conditions. These inequalities are likely due to a complex web of social determinants that includes poverty, historical consequences of colonialism, social exclusion, government policies of assimilation, cultural annihilation, and racism in all its forms (societal, institutional). Despite documented oral health disparities, prevention interventions have been scarce in Indigenous communities. This review describes oral health interventions and their outcomes conducted for Indigenous populations of the United States, Canada, Brazil, Australia, and New Zealand. The review includes research published since 2006 that are available in English in electronic databases, including MEDLINE. A total of 13 studies were included from the United States, Canada, Brazil, and Australia. The studies reviewed provide a wide range of initiatives, including interventions for prevention and treatment of dental disease, as well as interventions that improve oral health knowledge, behaviors, and other psychosocial factors. Overall, 6 studies resulted in improved oral health in the study participants, including improvements in periodontal health, caries reduction, and oral health literacy. Preferred intervention methodologies included community-based research approaches, culturally tailored strategies, and use of community workers to deliver the initiative. Although these studies were conducted with discrete Indigenous populations, investigators reported similar challenges in research implementation. Recommendations for future work in reducing oral health disparities include addressing social determinants of health in various Indigenous populations, training future generations of dental providers in cultural competency, and making Indigenous communities true partners in research.

This study presents the results of an oral health epidemiological survey conducted in 1997, based on WHO criteria, in the Xavánte indigenous community of Pimentel Barbosa (or Etéñitépa)

Objective: To compare patterns of dental caries, periodontal disease and dental care among Indigenous and non-Indigenous Central-West Brazilian subpopulations. Methods: Data were from two population-based cross-sectional studies involving 5-, 12-, 15-19-and 35-44-year-olds. The first examined were the Guarani, Kaiowá, Terena and Kadiwéu Indigenous groups from Mato Grosso do Sul Brazilian state and the second comprised a non-Indigenous population. Mean numbers of sound teeth, decayed, missing and filled teeth (dmft/DMFT), prevalence of poor oral hygiene for adults (35-44 years) and of periodontal disease were estimated. Restorative dental service utilization was measured using the Care Index. Results: The study populations comprised of 1830 Indigenous and 29 395 non-Indigenous people. The Kaiwoá ethnic group had the lowest DMFT among Indigenous groups for ages 12, 15-19 years; 0.9 (95% CI 0.7-1.1) and 2.4 (95% CI 1.9-2.9), respectively. The highest values were observed among the 12-year-old Terena ethnic group and 15-to 19-year-old Kadiwéo groups; 2.2 (95% CI 1.8-2.5) and 3.7 (95% CI 3.1-4.4), respectively. The mean DMFT values were significantly lower among Indigenous than non-Indigenous people for all age groups. The prevalence of bleeding and calculus was 70.3% (95% CI 64.5%-75.5%) and 80.1% (95% CI 74.8%-84.5%), respectively, for Indigenous people and 43.9% (95% CI 34.4%-50.7%) and 61.5% (95% CI 55.4%-67.2%), respectively, for the non-Indigenous population. Restorative services were higher among 5-year-old non-Indigenous children and adults than for Indigenous groups. Among Indigenous groups, the Terena had the highest level of restored teeth (38.0% at 12 years) and Kadiwéu the lowest level (8.8% at 12 years). Conclusion: In our study, Indigenous people had a lower caries burden, but less access to restorative services than their non-Indigenous counterparts. These disparities likely reflect differences in historical, socioeconomic, cultural, environmental and political determinants that both groups have experienced differently over time.

The aim of the study was to investigate caries experience levels in the Xavante Indians, in the state of Mato Grosso, Brazil, with a focus on between-group differences. Oral health data were collected in 2004, following WHO guidelines in four of the seven Xavante reserves (Pimentel Barbosa, Sangradouro, Areões and Marechal Rondon). Data were collected in the largest village in each of the reserves, using the DMFS index. Statistical analyses included sex-specific Poisson regressions to compare DMFS values among Xavante reserves. Prevalence ratios (PR) were used to measure the differences in caries experiences in the 6-to-34-year age group, whose losses varied between 26% and 30%. Xavante from the Pimentel Barbosa community were selected as reference group as they showed a lower prevalence of caries in all age groups. The major difference in DMFS was observed between Pimentel Barbosa and Sangradouro, both in males (PR 2.68, 95%CI 2.41-2.97) and in females (PR 2.03, 95%CI 1.85-2.23). The PRs of the filled component in Areões and Marechal Rondon (relative difference to Pimentel Barbosa) were lower, when compared to the total burden of disease observed in those reserves, indicating very low presence of restorative services. It was concluded that the oral health transition among Xavante has not been homogeneous and the differences found in caries experiences can be associated with the particularities of the interaction process between Indians and non-Indians. Local and regional determinants, including factors related to demography, specific economic and socio-cultural characteristics, health care access and type of oral health services received might have influenced the between-group differences in dental caries observed in the Xavante Indians.

Oral symptoms such as toothache and difficulty speaking due to dental problems play an important role in oral health self-rating. Self-rated oral health is an appropriate indicator for use in diverse sociocultural contexts.

It is concluded that factors related to the social functions of each sex (gender issues) and differential access to information, health services, and education may help to understand the differences observed in the incidence of caries.
