Ueber diphtherieähnliche Anginen mit lymphatischer Reaktion. Von Dr. Rudolf Deussing. Im Verlaufe jeder Diphtherieepidemie kommt unter den als Diphtherie angesprochenen Anginen eine Reihe von Fällen zur Beobachtung, die nach dein Aussehen beurteilt durchaus den Eindruck von Diphtherien machen, di aber durch die bakteriologisohe Untersuchung von der Diphtherie abzusondern sind, die schließlich auch durch ihren Verlauf und das Ausbleiben spezifischer Einwirkungen des Diphtherietoxiris ihre gesonderte Stellung zu erkennen geben. Eine frühzeitige ätiologische Unterscheidung solcher Anginen von Diphtherien ist dadurch erschwert, daß meistens erst der Ausfall der auf die Darstellung der Diphtheriekeime gerichteten Untersuchungen die Aufmerksamkeit auf die Sonderstellung cies Falles lenkt, weil die Aehnlichkeit mit Diphtherie so weit gehen kann, daß zunächst ein Unterschied nicht auffällt. Es sind verschiedene Erreger bekannt, die derartige diphtherieähnliche Krankheitsbilder hervorrufen können. An erster Stelle steht der Streptokokkus, der nicht nur durch seine Beteiligung an der Scharlachangina, sondern auch für sich allein ausgesprochene Beläge und Pseudomembranbildungen auf den Schleimhäuten des Rachens und überhaupt der oberen Luftwege hervorrufen kann.

Deussing.Unsere Anschauungen tiber die Verbreitungsweise der pathogenen Mikroben im Organismus des erkrankten Menschen ha, ben im Laufe der letzten beiden Dezennien ma.ncberlei Wandlungen erfahren. Die bakteriologische Forschung und zwar speziell die systematischer durchgef~hrten Blutuntersuchungen bei infektiSs-fieberhaften Erkra, nk~ngen haben uns gezeigt, dal~ auch bei solchen Infektionskra, nkheiten, bei denen wir in der Regel eine ausschliel~liche oder doch vorwiegend lokale Entwieklung der Krankheitserreger zu beob~.chten pflegen, unter besonderen, zurzeit allerdings nicht, n~her bekannten Voraussetzungen h~ufiger mit einem Einbrueh der fmglichen Mikroben in die Blutbahn und mit der Entwicklung einer tempor~ren Ba,kteri~mie oder auch einer echt6n Septik~mie gerechnet werden mul~. Das gilt namentlich fiir drei im allgemeinen durch lokale Prozesse charakterisierte Erkra,nkungen, n~mlich die Diphtherie, die Gonorrhoe und endlieh die genuine, dutch den Diplocoecus intraeellularis Weich s e 1 b a u m hervorgerufene meningitis epidemica.Gemde fiir die letztgenannte Erkrankung ha, ben sich in der Literatur der j~ngst verflossenen 3ahre die Angaben geh~uft, welehe das Auftreten der Meningokokken im strSmenden Blute sowie die Entstehung prim~rer und sekund~rer, dutch den Diploeoecus intraeellularis bedingter Krankheitsherde in inneren Organen, Gelenken usw. zum Gegenstand hatten. Durch die Hiiufung derartiger Beobachtungen ist bel vielen Autoren, die manehmal auch direkt ausgesproehene Anschauung erweekt worden, als
