La adenoamigdalectomía es la intervención quirúrgica más frecuente en otorrinolaringología, siendo su principal indicación las alteraciones obstructivas de la vía aérea superior asociadas con hiperplasia adenoamigdalina. El resultado posoperatorio habitualmente es evaluado de una forma subjetiva y no se ha estudiado sistemáticamente en nuestro país. En este estudio se utiliza una encuesta de calidad de vida validada en el extranjero (OSA-18), se realiza una prueba piloto y posteriormente se aplica en forma prospectiva a 64 tutores de pacientes pediátricos, previo a realización de adenoamigdalectomía y 3 meses tras la intervención. Adicionalmente se comparan los resultados con un grupo control de 63 niños asistentes a un jardín infantil El puntaje promedio preoperatorio en la muestra fue de 4,3 (2,3-6,1), estableciendo diferencias significativas con el 2,47 (1,3-3,5) de los controles. El puntaje promedio de cambio con el posquirúrgico fue de 1,9. La encuesta demuestra un deterioro en alteraciones del sueño, síntomas físicos, emocionales y grado de preocupación del cuidador en niños con indicación quirúrgica por hiperplasia adenoamigdalina sintomática. Además la evaluación posoperatoria demuestra un beneficio significativo en diversos aspectos de la calidad de vida tras la intervención quirúrgica.
Se ha demostrado, tanto en patologías como en alternativas terapéuticas, una asociación entre la vía aérea alta y baja, pero no existe evidencia suficiente sobre el rol del anillo de Waldeyer en el curso de enfermedades broncopulmonares. En este contexto, estudios previos son contradictorios sobre el efecto de la adenoamigdalectomía sobre el asma bronquial. El objetivo del presente trabajo es determinar si la adenoamigdalectomía provoca cambios en la calidad de vida de pacientes pediátricos asmáticos. Material y método: Estudio prospectivo en pacientes asmáticos, entre 5 y 10 años, con indicación de adenoamigdalectomía. Fueron controlados por broncopulmonar y otorrinolaringólogo, aplicándose la encuesta de calidad de vida de pacientes asmáticos PAQLQ en el preoperatorio y 6 meses posoperatorio. Resultados: Se evaluaron 35 niños, con edad promedio 7 años. El puntaje promedio preoperatorio de PAQLQ fue de 5,3; mientras en el posoperatorio fue de 6,4; diferencia equivalente a una mejoría moderada de la calidad de vida. Los ítems despertar nocturno, tos y capacidad de realizar actividades presentaron una mejoría significativa poscirugía. Discusión: La adenoamigdalectomía tiene un efecto beneficioso en la calidad de vida de los pacientes pediátricos asmáticos, con mejoría de síntomas no sólo de la vía aérea superior, sino también relacionados con reactividad bronquial, por lo que contribuiría al control de la enfermedad en un grupo de pacientes.
