RESUMO -Os adenomas produtores de GH podem ser mono, bi ou pluri-hormonais. A associação mais frequente do GH é com a sub unidade alfa ou com a prolactina. Apresentam-se com as alterações clássicas de acromegalia, mas quando produzem mais que um hormônio podem apresentar sintomas visuais. Os mono-hormonais são geralmente microadenomas e os outros dois grupos tendem a ser macroadenomas. Analisamos nossa experiência com 20 casos deste tipo de adenomas e mostramos o resultado cirúrgico, que foi bastante satisfatório para o grupo mono-hormonal. ABSTRACT -The growth hormone secreting pituitary adenomas can be mono, bi or pluri-hormonals. The most frequent association of GH is with the sub unit alpha or with the prolactin. This type of adenoma comes with the classic alterations of acromegaly, but when they produce more than one hormone, visual symptoms can be present. The adenoma producing just GH hormone is generally microadenoma and the other two groups tend to be macroadenoma. We analyze 20 cases of these adenomas and we have found quite satisfactory surgical result for the mono-hormonal type group. A secreção do hormômio de crescimento (GH) é mediada pelo hipotálamo pela liberação do fator liberador de GH (GHRH), e atua promovendo glicó-lise, gluconeogênese e produz somatomedina C (IGF1 -fator de crescimento semelhante a insulina 1). O IGF1 estimula o aparecimento de somatostatina que atua inibindo a produção de GH. O nível basal de GH é maior pela manhã, mas a maioria das dosagens deve ser menor que 5 ng/mL no sexo masculino e 10 ng/mL no feminino. A hipoglicemia aumenta o GH. De acordo com Scheithauer 1 , as células produtoras de GH representam 50% das células hipofisárias, localizando-se principalmente nas asas laterais da glân-dula. Nabarro 2 descreve as complicações relacionadas com o acromegálico. Estes pacientes tendem a apresentar hipertrofia cardíaca, aumento do débito cardíaco, diminuição da resistência periférica e insuficiência cardíaca congestiva, associada ou não com hipertensão arterial e arritmias, alterações estas relacionadas pela ação direta do GH na musculatura cardíaca, sendo a arritmia cardíaca a principal causa de óbito nestes pacientes. Esta patologia associa-se também a doenças do trato respiratório, pela presença de pólipos nasais, alterações laríngeas que levam à alteração da voz, macroglossia e hipertrofia da epiglote 3 . As alterações ósseas mais observadas são o aumento da mandíbula, que leva a piora da dentição, deformidades da coluna vertebral e alargamento dos seios nasais, levando principalmente proeminência frontal 2 . RESUMO -Apresentamos nossa experiência com 36 casos de adenomas hipofisários não secretantes. Estes adenomas foram divididos em não reativos e inativos de acordo com a imuno-histoquímica. Manifestaram-se por sintomas visuais, principalmente hemianopsia bitemporal; são macroadenomas, não havendo predomínio de adenomas circunscritos ou invasivos nos dois grupos. O tratamento é cirúrgico geralmente por via transeptoesfenoidal, com ressecção total ao redor de 40% dos casos e com melhora visual próxima aos 90% dos casos. These tumors were divided into non reactive and inactive in agreement with the immunohistochemical study. They present with visual symptoms, mainly bitemporal hemianopsia; they were macroadenomas, either invasive or non invasive, in both groups. The treatment is surgical using the transsphenoidal approach. Total resection was possible in about 40% of the cases. Representam 25% dos tumores hipofisários e tendem a crescer por expansão sendo pouco invasivos 1 . O diafragma selar é a área de menor resistência e, portanto, estes tumores tendem a crescer para cima; são geralmente macroadenomas. Os pacientes referem cefaléia, alterações visuais e, às vezes, alterações endócrinas. O achado mais frequente é a hemianopsia bitemporal que se inicia por quadrantoanopsia temporal superior, seguida de diminuição da acuidade visual 2 . Do ponto de vista endócrino, por compressão do parênquima hipofisário, ocorre hipopituitarismo. O eixo gonadotrófico é o primeiro atingido, seguido da parte da tireóide e diminuição da reserva de GH. Existe aumento de prolactina por compressão da haste hipofisária 3 . Pequeno grupo destes tumores é o dos oncocitomas, cujas células apresentam grande número de mitocôndrias e parecem originar-se da região da parte intermédia 4 . Do ponto de vista anatomopatológico 5 estes tumores são cromófobos no estudo pela hematoxilina e eosina mas, em relação à imuno-histoquímica, a maioria dos adenomas não funcionantes sintetiza pequenas quantidades de hormônios glicoproteicos como o TSH, FSH, LH e subunidade alfa. A reatividade também pode ser positiva para o ACTH, GH e PRL. Devido a esta positividade in vitro, Wilson 6 introduziu o termo de adenomas pituitários inativos.O diagnóstico por imagem é feito principalmente pela tomografia computadorizada (TC), ou pelo exame de ressonância magnética (RM) da região da sela turca. RESUMO -Relatamos nossa experiência com 19 casos de adenomas hipofisários produtores de ACTH que foram tratados cirurgicamente. Os adenomas mono hormonais produtores de ACTH eram microadenomas em 50% dos casos, apresentando-se com o quadro típico da síndrome de Cushing. Os pluri-hormonais manifestaram-se por alterações visuais. O resultado do tratamento cirúrgico, geralmente por via transeptoesfenoidal, foi satisfatório na maioria dos casos. Para os pacientes em que não conseguimos cura cirúrgica, foi indicada a radioterapia. As células produtoras de ACTH concentram-se no terço central, na parte intermédia e nas asas laterais da adeno-hipófise e representam 10 a 20% das cé-lulas. O ACTH é um peptídeo derivado de uma grande molécula chamada propiomelanocortina, também precursor do hormônio estimulador dos melanócitos e das beta-endorfinas. A regulação hipotalâ-mica positiva é feita principalmente pelo CRF (fator liberador do ACTH) e por outros neurotransmissores como a acetilcolina, serotonina, opiáceos, vasopressina, adrenalina, noradrenalina, polipeptídio intestinal vasoativo e angiotensina II. O cortisol plasmático é responsável pela inibição da liberação de ACTH, segundo Scheithauer 1 . O ACTH age estimulando a camada média da glândula supra-renal, zona fasciculada, para a produção de cortisol, que atua no metabolismo das gorduras, proteínas e carboidratos e também têm ação antiinflamatória. A síndrome de Cushing é causada principalmente por adenoma hipofisário, adenoma da glândula supra-renal e por produção ectópica de ACTH, que às vezes são difí-ceis de diferenciar nos testes bioquímicos e de imagem 2 . O excesso de secreção de ACTH por um adenoma hipofisário causa a doença de Cushing, que se caracteriza clinicamente por obesidade centrípeta, hirsutismo, estrias violáceas, fácies em lua cheia, acne, hipertensão arterial e diabetes mellitus, sendo dez vezes mais freqüente no sexo feminino. A síndrome de Cushing é mais comum no sexo masculino e está relacionada com tumores do pulmão tipo "oat-cell" ou carcinóide, feocromocitoma, ovário e por hiperplasia ou tumor da supra-renal 3 . A manifestação clínica da síndrome de Cushing é usualmente progressiva, por semanas ou meses, com períodos de quiescência. Existe obesidade centrípeta com deposição de gordura no tronco e na porção dorsal da área cervical muito mais do que nos membros. O fácies de lua cheia deve-se a deposição de PALAVRAS-CHAVES

A prolactina (PRL) é hormônio secretado pelas células lactotróficas da adeno-hipófise. As células produtoras de PRL estão situadas na porção lateral da glândula e representam 20% da sua celularidade. Esse hormônio foi distinto do hormônio do crescimento (GH) apenas em 1970, por Frantz e Likeinberg 1 e a sua positividade na imuno-histoquímica foi descrita por Hwang, Guyda e Friezen 2 em 1971. A síntese da PRL ocorre nos ribossomos do retículo endoplasmático e é armazenada no aparelho de Golgi. Aparecem na microscopia eletrônica como grânulos densamente ou esparsamente distribuídos. A prolactina é o único hormônio hipofisário com secreção espontânea na ausência de estímulo hipotalâmico, sendo sua quantidade modulada por estrógeno, cortisol e tiroxina ou por mecanismo de "bio-feedback". A dopamina é o fator inibidor da PRL, sendo secretada pelo hipotálamo. O ácido gama aminobutírico (GABA), a somatostatina e o hormônio liberador gonadotró-fico (GnRH) também exercem efeitos inibitórios. Os prolactinomas manifestam-se clinicamente por sintomas relacionados a hiperprolactinemia. No sexo feminino pode ocorrer amenorréia e galactorréia (síndrome de Forbes-Albrigh), irregularidade menstrual, infertilidade e hirsutismo e, no masculino, impotência, perda da libido, oligospermia e, mais raramente, ginecomastia e galactorréia. Descrevemos um caso raro de adenoma hipofisário produtor de TSH em uma paciente com quadro de hipertireoidismo, que após quatro anos desenvolveu hemianopsia bitemporal. TC de crânio mostrou tratar-se de tumor hipofisário de grau 2 e estadio C, de acordo com a classificação de Hardy-Vezina e Wilson. O tratamento foi cirúrgico, inicialmente por via transeptoesfenoidal, em que foi possível apenas biópsia, e a seguir a paciente foi submetida a craniotomia órbito-pterional com ressecção sub-total do tumor. Foi indicado tratamento complementar com radioterapia.
