Objetivo: Descrever o perfil e a variação temporal das internações e mortalidade hospitalar por síndrome respiratória aguda grave (SRAG) por COVID-19 no Piauí, Brasil, segundo local de internação. Métodos: Estudo descritivo sobre dados do Sistema de Informação da Vigilância Epidemiológica da Gripe, da 12ª semana epidemiológica de 2020 à 12ª de 2021. Calculou-se a mortalidade hospitalar e respectivos intervalos de confiança de 95% (IC95%). Resultados: Os indivíduos observados eram majoritariamente do sexo masculino (57,1%), negros (61,2%), com uma ou duas comorbidades (30,5%). A mortalidade hospitalar no interior, entre internados (44,1% – IC95% 42,0%;46,3%), admitidos em unidades de terapia intensiva (82,3% – IC95% 79,7;84,8) e indivíduos submetidos a ventilação mecânica invasiva (96,6% – IC95% 94,9;97,8), foi maior do que na capital do estado. Conclusão: O estudo permitiu a caracterização do perfil das internações devidas a SRAG por COVID-19 no Piauí e demonstrou mortalidade hospitalar elevada, mantendo-se alta no período estudado, sobretudo no interior.
A leishmaniose visceral (LV) ou calazar é uma das principais doenças tropicais negligenciadas, de grande importância devido ao caráter letal desta zoonose causada por protozoários do gênero Leishmania. Objetivou-se analisar a tendência temporal dos casos de LV no Estado do Piauí, Brasil, de 2008 a 2018, inserindo-o dentro do período total de 1971 a 2018. Utilizaram-se dados do Sistema de Informação de Agravos de Notificação (SINAN) e do Sistema de Informações sobre Mortalidade (SIM). Para análise da carga da doença, foram coletados dados do Instituto de Métricas e Avaliação em Saúde através do estudo Carga Global de Doenças, Lesões e Fatores de Risco. Foi empregada a análise de regressão Prais-Winsten. Foram registrados 2.374 casos com provável local de infecção no Piauí e 2.492 casos em residentes do estado. A série histórica analisada, de 1971 a 2018, reafirma o caráter cíclico da doença, pela presença dos picos epidêmicos identificados em 1983 e 1984, 1993 e 1994, 2003 e 2004 e 2013 e 2014. Não foram identificadas alterações significativas na tendência dos coeficientes de incidência, mortalidade e letalidade no estado. No entanto, houve aumento significativo do coeficiente de incidência no Território de Desenvolvimento (TD) Chapada das Mangabeiras (variação percentual anual de 17,5%) e redução no TD Vale do Sambito (-18,3%) e TD Vale do Rio Guaribas (-8,1%). A LV tem a maior carga de doença medida em anos de vida ajustados por deficiência entre as doenças tropicais negligenciadas do estado. Recomenda-se o aprimoramento das medidas de vigilância e controle do agravo.
O trabalho teve como objetivo comparar os dados de óbitos pela Covid-19 no Maranhão utilizando três fontes de informação. Para tal, realizamos um estudo transversal e descritivo sobre os dados de óbitos pela Covid-19 no Maranhão, de 7 de abril a 15 de maio de 2020. Utilizou-se dados da Secretaria de Estadual de Saúde (SES/MA), Painel Covid-19, do Ministério da Saúde, e Central de Informação do Registro Civil (CRC Nacional), analisados segundo sexo e faixa etária. Foram divulgados 550 óbitos pela SES/MA, 494 no Painel Covid-19 e 281 na CRC Nacional. Mais óbitos são observados em pessoas do sexo masculino e em faixa etária acima de 60 anos. A demora dos registros de óbitos e erros no monitoramento, somado a grande subnotificação dos reais números de óbitos, necessitam de melhorias para que seja possível retratar mais próxima do real cenário.
Resumo Objetivo: Analisar a tendência temporal e a distribuição espacial da mortalidade por acidentes de trânsito (AT) no Piauí, de 2000 a 2017. Métodos: Estudo ecológico de séries temporais, com dados do Sistema de Informações sobre Mortalidade (SIM), referentes à mortalidade por AT em residentes do estado do Piauí, segundo sexo, faixa etária e município. Empregou-se regressão de Prais-Winsten. Resultados: Registraram-se 14.396 óbitos, com taxa de mortalidade por 100 mil habitantes de 13,9 em 2000 e 30,6 em 2017. Houve aumento significativo na taxa de mortalidade por AT (variação percentual anual [VPA] de 6,4% – IC95% 4,3;8,7), maior entre motociclistas (VPA=14,7% – IC95% 9,7;20,0) e entre ocupantes de veículos (VPA=15,2 – IC95% 10,5;20,2). Conclusão: Houve aumento significativo da mortalidade por AT no Piauí, principalmente de motociclistas e ocupantes de veículos. São necessárias ações para promover a segurança viária e prevenir mortes no trânsito.
Introduction Hospitalizations for ambulatory care sensitive conditions (ACSC) are hospitalizations that can be potentially preventable through care at the first level of contact for health care. In fact, the actions offered in a timely and effective manner can reduce the risk of unnecessary hospitalizations by preventing diseases, controlling acute episodes and managing chronic diseases. Aim Analyze the trends in hospitalizations for ACSC in Piauí, Brazil, from 2009 to 2018. Methods Ecological time-series study based on data from the Hospital Information System (SIH) of the public health system (SUS), which comprised all hospitalizations of residents of Piauí, Brazil, in the period from 2009 to 2018. The explanatory variables were the hospitalizations for ACSC groups. For temporal trend analysis, the Prais-Winsten linear regression model was used in the Stata version 14 program. Results 567,577 were recorded as ACSC, representing 35.3% of the total hospitalizations (1,608,263). Most hospitalizations for ACSC were caused by infectious gastroenteritis (33.5%). The risk of hospitalization for ACSC decreased 36.8%, showing a significant reduction in the trend of the hospitalization rate (Annual Percent Change - APC: -4.6%; 95%CI: -6.4; -2.8). There was a greater decrease in the risk of hospitalization for asthma (70.3%), hypertension (66.4%) and infectious gastroenteritis (61.8%). However, a significant increase was identified for skin infection (6.1 times) and diseases related to prenatal care and childbirth (3.2 times). Conclusions The reduction in hospitalizations due to ACSC is a result of the strengthening of Primary Health Care as the Brazilian health system organizer and the implementation of the Mais Médicos Program, which enabled the presence of doctors in family health teams, especially in places that had not previously been attended. It is also necessary to develop new studies to expand the discussions and debates on these findings. Key messages Hospitalizations reflect the living conditions of individuals, with social changes being perceived by changes in hospitalization patterns over time. The strengthening of primary care policies in Brazil was able to reduce hospitalizations for ACSC and it is necessary to further strengthen these practices to improve people's health care.
Introduction Hospitalizations for ambulatory care sensitive conditions (ACSC) are an indicator that assesses the effectiveness of Primary Health Care. Such hospitalizations burden the public health budget and use the resources that could subsidize other health actions. Aim Analyze expenditure trends in hospitalizations for Ambulatory Care Sensitive Conditions (ACSC) in Piauí, Brazil, from 2009 to 2018. Methods Ecological time-series study based on data from the Hospital Information System (SIH). For temporal trend analysis, a linear regression model was built using the Stata version 14 program. Results US$ 119,559,009.56 (Average = US$ 6,292,579.45/year) was spent on ACSC, representing 17.3% of the total hospitalizations (US$ 690,714,037.24). 2010 was the period with the highest cost (14.8%). In contrast, 2018 was the year with the lowest expenditure (5.8%). 23.7% of the costs went to hospitalizations for gastroenteritis. There was a significant decrease in total expenses with ACSC (β = -1.27; 95%CI: -1.65; -0.89; p < 0.001). There was also a reduction in expenses per individual hospitalized, while in 2009, the average expenditure was US$ 230.97/hospitalization, in 2018 it was US$ 157.30/ hospitalization, representing a significant reduction of 31.9%, (β = -12.4; 95%CI: -18.2; -6.6; p = 0.001). The “asthma” and “hypertension” groups showed the greatest cost reductions, 84.8% and 80.7%, respectively. However, an increase in spending on diseases related to prenatal and childbirth (4.1 times) and skin infection (3.9 times) was identified. Conclusions Part of the cost reduction is explained by the reduction of hospitalizations for ACSC. The reduced spending averages are compatible with lower hospitalizations by most expensive groups. Linked to these findings, it is observed that Brazil has faced a period of recession since 2015. To this end, additional research must be carried out to relate the expansion of health coverage with the results presented. Key messages The costs of hospitalizations for PHC-sensitive conditions reflect the policy of inducing health system coverage, being lower for greater coverage. The evaluation of the cost of specific groups allows the induction of PHC policies focusing on the specific care for each group with a higher cost so as not to burden the health system.
O objetivo deste estudo é determinar a prevalência e os fatores associados à procura pelos serviços de saúde pelos adolescentes de Parnaíba-PI, em 2017/2018. Trata-se de um estudo transversal que analisou dados de inquérito populacional. Foram entrevistados adolescentes do 9º ano do ensino fundamental de escolas públicas da zona urbana de Parnaíba-PI, de outubro de 2017 a maio de 2018. Estimaram-se as prevalências e seus respectivos intervalos de confiança a 95% sobre a procura pelos serviços de saúde entre os escolares, cuja associação com as variáveis explicativas foi realizada mediante regressão múltipla. Nos últimos 12 meses, 42,8% (IC95%: 38,3;47,4) procuraram o serviço de saúde. A Unidade Básica à Saúde (UBS) foi o local mais procurado (71,1%; IC95%: 64,2;77,2). O uso de drogas ilícitas teve associação negativa com a procura de serviços de saúde (OR=0,51; IC95%: 0,28;0,93). A procura pelos serviços de saúde nos últimos 12 meses foi elevada e a UBS o local mais procurado. Adolescentes que consumiram drogas procuraram menos os serviços de saúde, apontando a vulnerabilidade destes jovens.Palavras-Chave: Comportamento do adolescente. Drogas Ilícitas. Fatores de proteção. Fatores de risco.The objective this study is to determine the prevalence and factors that may be associated with the demand for health services among adolescents from Parnaíba-PI, in 2017/2018. This is a study cross-sectional population survey. It was interviewed adolescents from the 9th grade of public elementary schools in the urban area of Parnaíba-PI, from october 2017 to may 2018. The prevalences and their respective 95% confidence intervals of the use of health services among the students were estimated, whose association with the explicative variables multiple regression analysis was performed. In the last 12 months, 42.8% (95%CI: 38.3;47.4) sought for health services. The Basic Health Unit (UBS) as the most demanded place (71.1%; 95%CI: 64.2;77.2). The use of illicit drugs was negative statistic associated with the outcome was verified (OR=0.51; 95%CI: 0.28;0.93). The demand for health services in the last 12 moths was high, being the UBS the most demanded place. Adolescents who consuming drugs used health services less, pointing the vulnerability for these young people.
