Kleinschalig wonen is in, het traditionele verpleeghuis lijkt uit de tijd. De voordelen van kleinschaligheid worden tegenwoordig alom beklemtoond, het pleit lijkt beslist. Maar is dit wel het hele verhaal? Wat zijn de kwetsbare kanten van deze woonvorm? Is kleinschalig wonen geschikt voor de totale verpleeghuispopulatie? Of zijn er ook groepen die in een grootschaliger setting beter af zijn? Ronald Geelen belicht de andere kant van de medaille.Het Nederlandse verpleeghuis bestaat grofweg een halve eeuw. Aanvankelijk stond de opvangfunctie voor ziekenhuispatie¨nten centraal die nog niet naar huis toe konden (vaak de somatische patie¨nten) of daartoe juist niet meer in staat waren (voornamelijk de psychogeriatrische patie¨nten). De toenmalige werkwijze van het ziekenhuis werd min of meer overgenomen in het verpleeghuis (medisch model). Omdat met name de psychogeriatrische patie¨nten veel zorg nodig bleken te hebben, werd de hamvraag in de psychogeriatrische huizen al snel hoe die zorg zo goed mogelijk kon worden georganiseerd. In sommige huizen voerde men een differentiatie naar 'zorgzwaarte' in om efficie¨nter werken mogelijk te maken. Er kwamen begeleidings-, verzorgings-en verpleegafdelingen, naar gelang de benodigde hulp (verpleegkundig/taakgericht model).Hierop kwam een reactie. Mensen mochten niet gereduceerd worden tot zorgbehoeften en lichamelijke afhankelijkheid, het moest gaan om een totaalperspectief op de oudere, lichamelijk, psychisch en sociaal. Daarbij dienden de verschillende disciplines in het verpleeghuis gezamenlijk naar de clie¨nt te kijken en in overleg een zorgplan op te stellen (multidisciplinair model). In het laatste decennium raakte een vraaggericht model in zwang. De wensen en de behoeften van de oudere dienden het vertrekpunt te zijn van de zorg, de behandeling en de woonomgeving. Op elk vlak moest de oudere (of diens vertegenwoordiger) zoveel mogelijk zijn zegje kunnen doen. Evolutie of revolutie?De ontwikkeling die het verpleeghuis heeft doorgemaakt is er niet per se een van slecht naar goed, er is veeleer sprake van een proces van verschuivende accenten die samenhangen met de maatschappelijke wensen en de eisen van de tijd. Het ene model borduurt voort op de verworvenheden van het vorige: multidisciplinaire zorg werd bijvoorbeeld pas mogelijk nadat de fysieke zorg behoorlijk geregeld was. Raakt het huidige verpleeghuis in de problemen, dan zie je een regressie naar een vroeger stadium van functioneren. Bij krappe personele bezetting komt men minder tegemoet aan individuele wensen en voorkeuren, verschraalt het multidisciplinaire overleg en komt efficiency op de afdeling steeds meer op de voorgrond te staan. Uiteindelijk raken individuele behoeften van bewoners zo uit het zicht en verandert de achterliggende kijk op het werk. Hoe dit ook zij, duidelijk is in ieder geval wel dat het 'traditionele' verpleeghuis niet een onveranderlijke moloch is zoals sommige aanhangers van het kleinschaligheidsdenken het doen voorkomen; het heeft in relatief korte tijd juist uiteenlopende gedaa...
