Introducción. Se conoce que las gestantes son más susceptibles a infecciones respiratorias, pero el comportamiento en la infección por SARS-CoV-2 recién se está conociendo. Objetivo. Describir las características epidemiológicas de las gestantes y puérperas infectadas con SARS-CoV-2 en el Hospital Nacional Daniel Alcides Carrión, Callao, Perú. Métodos. Estudio descriptivo transversal. Se seleccionó todas las gestantes hospitalizadas entre el 1 de mayo y el 31 de julio del año 2020 con diagnóstico de infección por SARS-CoV-2, mediante la prueba rápida serológica o la prueba RT-PCR. Resultados. Durante el periodo de estudio se tamizó a 671 pacientes; 308 gestantes y 9 puérperas fueron diagnosticadas de infección por SARS-CoV-2. Los síntomas más comunes fueron tos (57%) y disnea (35%). El 95% fue asintomática, 2% tuvo enfermedad leve, 1% moderada y menos del 1% severa. Solo se registró una muerte materna. 69% de los partos fue por vía vaginal y 31% vía cesárea. Conclusiones. En nuestro hospital, el comportamiento de la infección por SARS-CoV-2 en la gestante no difirió mayormente de lo encontrado para la población general de la misma edad.
Se reporta una serie de 3 casos con diagnóstico prenatal de malformación adenomatoide quística pulmonar variedad macroquística sometida a terapia intraútero con derivación tóraco-amniótica. Se muestra que esta intervención intrauterina altamente especializada puede ser realizada con éxito en nuestro país. Los tres casos sobrevivieron. Se presenta una revisión corta de esta patología y se propone un flujograma de manejo.
La evaluación de la resistencia en las arterias uterinas es empleada para identificar a las gestantes con placentación inapropiada, que las pone en riesgo de desarrollar preeclampsia, restricción de crecimiento intrauterino (RCIU) y otras manifestaciones de insuficiencia placentaria. Objetivo. Identificar cuál de las curvas de referencia del índice de pulsatilidad de las arterias uterinas (IPAUt) publicadas coinciden con nuestros datos institucionales. Métodos. Estudio retrospectivo, transversal, en el que se obtuvo 2 031 evaluaciones del índice de pulsatilidad de las arterias uterinas en 1 753 gestantes. La resistencia promedio de las arterias uterinas fue comparada con las curvas de referencia publicadas por Gomez, Weichert y Limay. Resultados. El IPAUt tuvo una débil pero significativa correlación negativa (r=0,16) con la edad gestacional (EG), entre las 24 y 40 semanas. El 6,5%, 7,5% y 15% de nuestras mediciones superaron el percentil 95 de las curvas de Weichert, Limay y Gomez, respectivamente. Conclusiones. El índice de pulsatilidad promedio de las arterias uterinas en nuestra población se ajustó mejor a la curva de referencia de Weichert. Se requieren estudios prospectivos para validar este hallazgo clínico.
La evaluación del cerebro fetal es un punto imprescindible en el ultrasonido obstétrico, por la gran cantidad de malformaciones que pueden ser diagnosticadas. La guía de ISUOG nos brinda los cortes elementales para la sospecha de la patología cerebral; pero, podemos ampliar y mejorar nuestro ultrasonido con la visualización de estructuras fácilmente reproducibles, tales como el complejo anterior, cuerpo calloso, cisura de Silvio y el cuarto ventrículo. Presentamos algunas herramientas para complementar la evaluación del cerebro fetal.
