EN: El general en su laberinto de Gabriel García Márquez.Desde una perspectiva lingüística y semiótica fue Roland Barthes quien a finales de la década de los sesenta planteó el interrogante de si la tradicional diferenciación entre géneros narrativos continuaba siendo sostenible luego de un análisis estructural de los mismos. Es decir, si en lo que tienen de enunciación de la realidad, el discurso poético y el novelesco son irreconciliables o si una narrativa de ficción es en verdad opuesta a una narrativa "científica" como la del texto histórico. Es sabido que esa misma pregunta ya se la había formulado con anterioridad, y desde la literatura, Jorge Luis Borges.1 No obstante lo que para el autor de Otras inquisiciones (1952) fue un tema de sus relatos, para Barthes y los que le siguieron fue y es un asunto de indagación conceptual y argumentación teórica. Estas indagaciones cuyos centros reflexivos se han concentrado en demostrar las coincidencias retóricas entre el texto histórico y el ficcional, exaltan además los supuestos culturales que, sin distinción de genero, intervienen y orientan toda narración, en especial la narración escrita.2 Ahora bien , independientemente del sendero que elijamos para abordar la problemática de la disolución o entrecruzamiento de los géneros, lo cierto es que en Latinoamérica, y nuevamente desde la literatura, la llamada nueva novela histórica, 3 encarna y retoma esta preocupación. A partir de la década de los * Profesor de The Johns Hopkins University 1 Algunos cuentos del los libro La historia universal de la infamia (1935), Historia de la Eternidad (1936) y de Ficciones (1944), ya contenían el recurrente y borgeano asombro frente a la imposibilidad de abarcar, a través del lenguaje, la verdad histórica o la realidad. Para Borges el discurso histórico lo mismo que el filosófico o el novelesco son versiones inacabadas de la realidad o mejor, versiones de algunos de los interrogantes que a lo largo del tiempo se plantea el ser humano. La respuesta a estos interrogantes siempre será incompleta, no tanto por lo inconmensurable de la pregunta si no porque ellos empiezan y terminan en las fronteras del lenguaje .2 Mas comprometidos teóricamente que el mismo Barthes en señalar los puntos de encuentro entre la retórica del discurso histórico con la del discurso ficcional podemos nombrar, en la tradición francesa a , Paul Veyne,1971 y Michael de Certeau, 1975, 1978 (1987) 3 Teniendo en cuenta que en la novela latinoamericana el referente histórico siempre ha sido un elemento de singular importancia y que la nueva novela histórica se distingue de la que no lo es, habría que hacer un paréntesis y destacar las características y diferencias entre este nuevo subgénero y la tradicional novela histórica. The treatment of low-grade osteosarcomas is surgical resection with wide margins. In instances of dedifferentiation, a therapeutic paradigm similar to that of conventional high-grade osteosarcoma has not been adequately evaluated in these neoplasms. The main objective of this review was to define whether the addition of chemotherapy to surgical treatment has an impact on the survival of patients with dedifferentiated low-grade osteosarcomas. Secondary objectives were to observe the degree of histological response to neoadjuvant chemotherapy and to describe the percentage of de novo dedifferentiation. A systematic search of articles including dedifferentiated low-grade osteosarcomas, published between 1980 and 2022 was carried out in the PubMed, Cochrane and Scielo databases. A qualitative synthesis of the results was performed. Twenty-three articles comprising 117 patients were included. The survival of patients treated with surgery alone and surgery with chemotherapy was not statistically significant between the two groups. A good histological response was seen in 20% of specimens treated with neoadjuvant chemotherapy. De novo dedifferentiation was seen in approximately a fifth of low-grade osteosarcomas. The evidence available suggests that the addition of chemotherapy does not have an impact on the survival of patients with low-grade dedifferentiated osteosarcomas.
