Sao Paulo Med J. 2005;123(Suppl):11.INTRODUÇÃO O transplante simultâneo pâncreas e rim (TSPR) é realizado em pacientes diabéticos insulino-dependentes com insuficiência renal crônica em terapia dialítica. O uso de anestesia venosa total (AVT) para o TSPR pode gerar a estabilidade hemodinâmica necessária, bem como comodidade e segurança para o anestesiologista, facilitando a extubação precoce. RELATO DO CASOTodos os pacientes foram monitorizados com oximetria de pulso, pressão arterial invasiva e cardioscópio com derivações DII e V5, capnografia, diurese e pressão venosa central, além de acesso venoso periférico com agulha plástica n o 14 G e acesso central com cateter duplo lúmen em veia jugular interna direita. Gasometria arterial, eletrólitos, hematócrito, hemoglobina e glicemia foram coletados a cada hora e, durante a manipulação pancreática (anastomoses e reperfusão), medidas glicêmicas capilares foram realizadas a cada 30 minutos. Foi um total de sete casos com idade média 30,8, variando de 25-37 anos; quatro do sexo masculino e três do feminino; com tempo médio de TSPR (min) de 401,4, variando de 390 a 530 min. A anestesia foi realizada com infusão contínua de remifentanil (0,2-0, 5 mcg/hg/min), e propofol com dose alvo controlada (3-4 mcg/ml). Rocurônio foi utilizado na indução anestésica (0,6-0,9 mg/kg), sendo ela realizada em seqüência rápida com manobra de Sellick. Empregou-se atra-cúrio como bloqueador neuromuscular (BNM) de manutenção, sendo repetido em bolus conforme necessidade. Furosemida e manitol serviram para manter ou gerar débito urinário. A glicemia foi mantida entre 100-150 mg/dl. Ao término do procedimento foram feitas a reversão do BNM e a desintubação orotraqueal, e os pacientes foram encaminhados à UTI. DISCUSSÃOEvitar hipotensão, hipo ou hiperglicêmia e gerar débito urinário. Essa tríade é, em suma, o que caracteriza a qualidade de uma anestesia para TSPR, que, apesar de uma cirurgia de grande porte e risco, pode ser gerida pelo anestesiologista por meio da AVT, possibilitando bom controle hemodinâmico e anestésico no intra-operatório, bem como desintubação precoce desses pacientes, reduzindo complicações respiratórias sem aumento de riscos. INTRODUÇÃO O bloqueio subaracnóideo é uma técnica comum na anestesia obstétrica, sendo a administação dos agentes anestésicos realizada através de agulha de fino calibre. Relatamos um caso de quebra de agulha Quincke 27 G no espaço intervertebral, complicação rara em raquianestesia.
