Introdução: O agente causador da Doença de Chagas ou Tripanossomíase Americana é o Trypanosoma cruzi. Em 2006 o Brasil foi capaz de controlar a transmissão vetorial do Triatoma infestans, considerada a principal forma de contaminação da doença. Porém, devido a mudança do padrão epidemiológico, aumento da transmissão oral e colonização de outras espécies de triatomíneos, a doença persistiu e é considerada endêmica em regiões do Norte e Nordeste do Brasil. A prevenção da doença de Chagas relaciona-se diretamente à forma de transmissão, o que justifica a relevância desse estudo. Objetivo: Identificar formas de transmissão e sintomas desta moléstia. Material e métodos: Utilizou-se como método o levantamento bibliográfico nas Plataformas SciELO e Google Acadêmico e como material quinze artigos científicos referentes ao período de 2018 a 2021 que embasam o tema “Doença de Chagas”. Resultados: As formas de transmissão apresentam diferentes períodos de incubação podendo ser: pela mulher chagásica para o feto durante a gravidez ou parto ou por leite materno; por transfusão de sangue ou transplante de órgãos (de 30 a 40 dias); pelo contato de mucosas com material contaminado em laboratório (20 dias); pela ingestão de alimento contaminado (de 3 a 22 dias) e pelo contato direto com as fezes de triatomíneo infectado após a picada (de 4 a 15 dias). A fase aguda pode ser sintomática (causando febre, fastio, fraqueza, edemas no rosto e pernas, manchas avermelhadas na pele, adenomegalia, hepatomegalia, esplenomegalia, manifestações hemorrágicas, aumento do coração) ou não. Pacientes não tratados na fase aguda desenvolvem a fase crônica, que inicialmente pode não apresentar sintomas por longo período ou avançar de forma ativa manifestando complicações cardíacas (como insuficiência cardíaca, cardiomiopatia dilatada associada à miocardite, fibrose e disfunção cardíaca), digestivas (como alterações motoras no trato digestivo superior, megacolon e megaesôfago) ou neurológica (como alterações de psicomotricidade, distúrbios de atenção e do sono, déficit mnemônico, depressão e diplegia cerebral espástica). Conclusão: Os mecanismos de transmissão são transfusional/transplante, vertical/congênita, acidental, oral e vetorial. Após o contágio, o paciente desenvolve a fase aguda, e caso não receba tratamento, manifesta a fase crônica, podendo apresentar complicações cardíacas, digestivas, cardiodigestivas e neurológicas.
Introdução: O tabagismo é uma doença crônica provocada pela dependência da nicotica (substância presente em produtos derivados do tabaco). É referido pela Revisão da Classificação Estatística Internacional de Doenças e Problemas Relacionados à Saúde [CID-10] como parte do grupo de transtornos mentais e comportamentais por se tratar da adicção de substância psicoativa (nicotina). Conforme a Organização Mundial da Saúde (OMS), o cigarro mata mais de oito milhões de pessoas por ano, sendo sete milhões pelo uso direto e um milhão e duzentas mil pelo uso passivo. Das inúmeras doenças causadas pelo tabagismo, podemos elencar doenças cardiovasculares, acidente vascular encefálico, úlceras gástricas e intestinais, catarata, infertilidade, impotência sexual, osteoporose e principalmente síndromes respiratórias e diversos tipos de câncer. Este estudo justifica-se, a medida em que o tabagismo é o principal fator de risco prevenível de morbimortalidade precoce, sendo considerado um dos principais fatores de risco evitáveis para mais de vinte tipos de neoplasias. Objetivo: Identificar o papel do tabagismo na fisiopatologia de neoplasias. Material e métodos: Foi realizado levantamento bibliográfico nas plataformas SciELO, PubMed e Google Acadêmico, onde foram reconhecidos treze artigos científicos do período de 2001 a 2022 que embasam o tema “Tabagismo e carcinogênese”. Resultados: A formação do câncer é um longo processo em que há alteração na genética e composição das moléculas celulares, causando desequilíbrio em sua funcionalidade e gerando células cancerosas. Existem em torno de 4.700 substâncias tóxicas na composição do cigarro, dentre elas 60 sendo substâncias carcinogênicas. Assim, essas toxinas, associadas a fatores extrínsecos como o hábito e ato de fumar, induzem uma resposta inflamatória dos tecido e podem levar a malignização dessas células e lesões na pele, mucosa e cavidade oral do indivíduo. Conclusão: O principal papel do tabagismo na fisiopatologia das neoplasias é facilitar as alterações e mutações celulares, seja por causas: mecânicas (contato crônico do cigarro com o epitélio dos lábios), físicas (alta temperatura durante a queima da substância) e químicas (substâncias carcinogênicas irritantes e inflamatórias).
OBJETIVO: O seguinte estudo objetivou descrever a semiologia do paciente portador de hemorragia digestiva alta, considerando como determinante na avaliação de potencias focos hemorrágicos. METODOLOGIA: Foram realizadas buscas nas plataformas do SciELO, LILACS, PubMed, Scopus e Google Scholar,utilizando os descritores gastrointestinal bleeding, peptic ulcerous disease e varicose hemorrhage, sendo identificados 35 estudos, dos quais foram incluídos 13 artigos completos. Desses estudos, 5 avaliaram as principais etiologias, 2 o surgimento de novos testes diagnósticos, 2 analisaram os aspectos epidemiológicos e 1 a sintomatologia apresentada pelo acometimento da hemorragia digestiva alta. Observou-se inicialmente a abundâncias de informações conceituais sobre o sangramento, como um transtorno clínico comum, acompanhada de inúmeras manifestações, considerando que o foco hemorrágico pode ocorrer em qualquer porção do trato gastrointestinal. Neste estudo, todas as publicações eleitas apresentaram o quadro semiológico composto por algia abdominal, indícios de choque hipovolêmico e taquicardia, alguns exibiram quedas abruptas da pressão arterial, odinofagia, êmese, náuseas e estado ictérico. Os pacientes implicados, cronicamente, já manifestaram ocorrências prévias, devido ao caráter recidivante torna-se essencial investigar a existência de varizes, fístula aorto-entérica, angiodisplasia e doença ulcerosa. CONCLUSÃO: Elucida-se que a hemorragia digestiva alta representa a principal causa de sangramento do trato gastrointestinal, majoritamente manifesta-se como hematêmese ou melena e cursam com o quadro sintomatológico que auxilia na avaliação da gravidade deste e o embasamento de potenciais focos de sangramento e que contribuam para disseminação de informações e intervenções futuras.
Introduction: Annually, 50% of medical expenses worldwide stem from damage to the body's tissues and organs. Large skin defects can be caused by tumor excision, venous ulcers, diabetic foot ulcers, and burns. The 3D bioprinting of skin has an advantage compared to other technologies for skin substitutes, the capacity for directional and spatial handling at the cellular level with variable density. Objective: It was to identify the most efficient 3D bioprinting strategies and their application in studies in vivo and in vitro animal models, to demonstrate state of art in skin regeneration, and to direct new clinical research with translational studies. Methods: The rules of the Systematic Review-PRISMA Platform were followed. The research was carried out from November 2022 to February 2023 and developed based on Scopus, PubMed, Science Direct, Scielo, and Google Scholar. The quality of the studies was based on the GRADE instrument and the risk of bias was analyzed according to the Cochrane instrument. Results: A total of 237 articles were found and 97 articles were evaluated in full, and 16 were included and described in the present study. According to the GRADE instrument, most studies (X2 =90.5%>50%) followed a controlled clinical study model and had a good methodological design. the biases did not compromise the scientific basis of the studies. Conclusion: Most organotypic skin models have an epidermal layer of keratinocytes and a dermal layer of fibroblasts embedded in an extracellular matrix-based biomaterial. Furthermore, skin comprising epidermis, dermis, and hypodermis stratified with blood vessels, nerves, muscles, and cutaneous appendages can be fabricated. These findings provided evidence for further advances in translational studies in humans.
Introdução: A epidemia de obesidade pediátrica é uma realidade mundial. O tratamento desta doença destina-se a infância (lactentes, pré-escolares e escolares) e adolescência. A obesidade é o excesso de gordura corporal que resulta em agravos à saúde. A justificativa desse estudo refere-se ao impacto desta condição na saúde individual e coletiva. A relevância desse trabalho diz respeito a repercussão desta moléstia na saúde física e psicossocial do indivíduo predispondo-o à hipertensão, dislipidemia, diabetes, crescimento avançado, maturidade precoce, esteatose hepática, colelitíase, deficiência pulmonar, asma, apneia do sono, anovulação crônica hiperandrogênica, alterações dermatológicas, fraturas ortopédicas, ansiedade e depressão; além de ser um fator de risco importante para obesidade na vida adulta e complicações cardiovasculares de alta mortalidade. Objetivo: Identificar quais ações são eficientes no tratamento da obesidade pediátrica. Material e métodos: Foi realizado levantamento bibliográfico nas Plataformas LILACS, PubMed e Google Acadêmico onde foram reconhecidos dezesseis artigos científicos referentes ao período de 2018 a 2021 que embasam o tema “Obesidade na Pediatria: um problema de saúde pública”. Resultados: Dietas nutricionais com baixa ingestão de carboidratos ou de gordura foram equivalentes na diminuição do Índice de Massa Corporal (IMC). Exercícios físicos diminuíram a glicemia de jejum e a pressão arterial. Ações educacionais junto a família e escola (como orientação nutricional, comportamento não sedentário, higiene do sono e limitação do tempo de tela) revelaram declínio do IMC, circunferência de cintura e pressão arterial. Intervenções medicamentosas (Metformina, Orlistat e Sibutramina) reduziram o IMC e a circunferência da cintura, porém aumentaram o colesterol e a pressão arterial. Intervenções cirúrgicas (banda gástrica, bypass gástrico em Y de Roux e Gastrectomia Vertical) mostraram a maior redução do IMC, porém apresentaram deficiências nutricionais de Ferro, Vitamina D e B12. Conclusão: A redução da obesidade infantil apresenta melhores resultados a longo prazo com a adoção de estratégias combinadas (mudança do estilo de vida e terapia comportamental familiar). Em adolescentes obesos estas medidas podem ser associados a Sibutramina ou Orlistat (fármacos que oferecem melhores resultados). Já em adolescentes obesos extremos ou portadores de comorbidades graves deve ser considerada a banda gástrica ou o Y de Roux (cirurgias mais eficientes).
