RESUMENIntroducción: La mortalidad nacional en otorrinolaringología no ha sido analizada en nuestro medio desde 1985, en esa fecha Kirschbaum y Aracena demostraron que el cuerpo extraño en la vía aérea era la causa de muerte más frecuente.Objetivos: Describir la tendencia de las tasas de mortalidad por causa otorrinolaringológicas y compararlas con los resultados de estudios anteriores.Material y método: Estudio descriptivo de mortalidad en el que se realizó una revisión retrospectiva de los anuarios de demografía del INE entre 1991-2003 en base a la clasificación internacional de enfermedades CIE IX y CIE X.Resultados: Se observó una media aritmética de mortalidad por causas otorrinolaringológicas de 7,34 por 100.000 habitantes, con una tendencia a la disminución en los años estudiados que va desde 10,12 a 4,81 por 100.000 habitantes entre los años 1991 y 1999 respectivamente, lo que equivale a una disminución de 53%. Esto se explica principalmente por una baja significativa de la tasa de mortalidad por cuerpo extraño en la vía aérea, siendo ésta también la principal causa de muerte en otorrinolaringología, ya que las otras patologías, en su mayoría tumores malignos de distinta localización, presentan una estabilidad en su tasa de mortalidad.Discusión: Al igual que en estudios anteriores, el cuerpo extraño en la vía aérea, sigue siendo la principal causa de mortalidad en otorrinolaringología, seguido por el cáncer de laringe, que continúa siendo de predominio masculino. No se observó una variación significativa de la mortalidad por patología maligna en los años estudiados, como tampoco en comparación con el estudio realizado por Kirschbaum y Aracena en 1985; pero destaca la importante disminución de la tasa de mortalidad por otitis media supurativa en relación a ese estudio, lo que se explicaría por una mayor accesibilidad de la población a tratamiento médico.Palabras clave: Tasa de mortalidad, Otorrinolaringología ABSTRACT Background: National mortality in otolaryngology has not been analyzed in our population since 1985, when Kirschbaum and Aracena demonstrated that a strange body in the airway was the most frequent reason of death.
RESUMENObjetivo: Analizar los resultados iniciales de un estudio prospectivo en pacientes con schwanomas vestibulares tratados con radiocirugía en la Pontificia Universidad Católica de Chile.Material y método: Se presentan 17 pacientes portadores de schwanomas vestibulares esporádicos tratados con radiocirugía y seguidos entre 6 y 26 meses. El tratamiento fue realizado con acelerador lineal Varian clinac 21 EX, con equipamiento Varian-Zmed. La dosis marginal administrada al tumor fue de 12 a 12,5 Gray. El seguimiento a 6, 12 y 24 meses consta de resonancia magnética, audiometría y evaluación clínica.Resultados: La dosis marginal de irradiación usada fue entre 12 y 12,5 Gray normalizada a la isodosis 70% u 80%. En todos los pacientes se documentó disminución de la captación de contraste del tumor y en 16 (94%) se observaron áreas de necrosis centro tumoral. No hubo mortalidad, la preservación de audición útil fue 62,5% actuarial a 2 años. No ha existido deterioro de la función de los nervios facial ni trigémino. Todos los pacientes que previamente estaban trabajando retornaron a sus labores en promedio 11,5 días luego del tratamiento.Conclusiones: Los resultados iniciales de esta serie son comparables a los resultados publicados en la literatura y refuerzan el demostrado rol de la radiocirugía en el tratamiento de los schwanomas vestibulares.Palabras clave: Schwanomas, radiocirugía, acelerador lineal.
SUMMARYObjective: To analyze the preliminary experience of radiosurgery for Vestibular Schwannomas at the Pontificia Universidad Católica de Chile.Material and methods: The first 17 patients with sporadic Vestibular Schwannomas treated by radiosurgery at our institution are reported. The marginal dose used was 12 to 12.5 Gy. prescribed at the 70 or 80 isodose line. Patients were controlled at 6, 12 and 24 months with magnetic resonance, audiometric study and clinical examination.Results: In all of the 17 patients treated a decrease tumor enhancement on MR was demonstrated. In 16 patients (94%) a pattern of central tumor necrosis was observed during the firs year. Actuarial useful hearing was maintained in 62.5% at 2 year after treatment. Facial nerve function was maintained in all of the 15 patients with normal function at treatment (100%).
