A Síndrome de Cushing compreende os sinais e sintomas associados com uma exposição prolongada a níveis impropriamente elevados de glicocorticóide livre no plasma. Excluídas as causas iatrogênicas, a causa mais comum da Síndrome de Cushing é a Doença de Cushing, acometendo cerca de 70% dos casos. Apresentamos o caso de um paciente de 20 anos, masculino, com obesidade centrípeta, face de "lua-cheia" e estrias púrpuricas, cuja investigação diagnóstica demostrou tratar-se de macroadenoma hipofisário. O paciente foi submetido à hipofisectomia transesfenoidal, entretanto evoluiu com recidiva precoce do quadro clínico, sendo submetido a novo procedimento cirúrgico seguido de radioterapia. O paciente encontra-se atualmente em remissão clínico-laboratorial. When iatrogenic causes are excluded, the commonest cause of Cushing's syndrome is Cushing's disease, accounting for approximately 70% of cases. We present the case of a 20-year-old male patient with central obesity, moon face and purple-red striae, whose diagnostic investigation shows a pituitary macroadenoma. The patient was submitted to transsphenoidal hypophysectomy, but developed early recurrence. He was submitted to a second transsphenoidal intervention followed by pituitary radiation. Presently, the patient is in clinical and laboratory remission.
