A obesidade é considerada um problema mundial de agravo à saúde, de causa complexa, e inclui uma combinação de influências psicossociais, ambientais e biológicas. A cirurgia bariátrica é tratamento mais eficaz para pacientes com obesidade grave, resultando na melhora do quadro das comorbidades relacionadas à obesidade. No entanto, o reganho de peso e a recidiva de doenças metabólicas, como diabetes e hipertensão, têm sido observados em um subgrupo de pacientes pós-cirurgia bariátrica. O manejo por equipe multidisciplinar com avaliações periódicas é fundamental para otimizar os resultados obtidos desses pacientes. Palavras-chave. Bariatric surgery is the most eficient treatment for patients with morbid obesity, resulting in improvement of the aspects on obesity-related comorbidities. However, weight regain and recurrence of metabolic disorders such as diabetes and hypertesion have been observed in a subgroup of patients after bariatric surgery. Management by a multidisciplinary team with periodic evaluations is fundamental to optimize the results obtained from these patients.

Evidências atuais apontam a cirurgia bariátrica como o tratamento da obesidade grave mais eficiente se comparado a atividade física, modificações no estilo de vida e medicação.Nota-se que 10 a 30% daqueles submetidos ao tratamento da cirurgia bariátrica possuem uma perda de peso abaixo da ideal, ou seja, abaixo dos 50%; ademais, pode haver o reganho do peso, o qual pode ser equivalente ao montante perdido no primeiro ano em até 35% dos casos. Pobres resultados de perda de peso a longo prazo após cirurgia bariátrica são frequentemente atribuídos à baixa adesão a recomendações comportamentais no pós operatório, particularmente aquelas relacionadas à alimentação e à atividade física, também são relevantes fatores de ordem psicológica. Conhecer os fatores relacionados ao reganho de peso após uma cirurgia bariátrica é essencial para o manejo na prática clínica.O presente estudo é do tipo observacional transversal/seccional, sendo a unidade de estudo os pacientes obesos submetidos à cirurgia bariátrica no Hospital Regional da Asa Norte (HRAN-DF), durante o segundo semestre de 2019 e primeiro semestre de 2020. O fator principal em estudo serão os fatores implicados no reganho ou manutenção de peso após a realização da cirurgia bariátrica.De acordo com os dados colhidos, foi observada uma predominância do procedimento de cirurgia bariátrica no sexo feminino, em detrimento do sexo masculino, havendo percentual, respectivamente, de 91,8% e 8,2%. A idade média para realização do procedimento foi de 49,02 anos.Na amostra de 49 pacientes, com um total de 9 pacientes com porcentagem de perda de peso inferior a 50% e de 9 pacientes com reganho maior do que 20% e percentual de perda de peso superior a 50%.No que diz respeito às comorbidades, observou-se remissão total em 57,57% dos pacientes acometidos por hipertensão; de 61,9% nos pacientes diabéticos; e de 80,7% no caso de apneia. A remissão foi parcial em 21,21% dos pacientes com hipertensão; e de 4,7% dos pacientes com diabetes.Com relação aos hábitos de vida, 93,9% dos pacientes negaram tabagismo e 63,26% negaram etilismo. 34,69% dos pacientes realizam atividade física adequada, segundo a OMS, 150min/semana de atividade moderada ou 75min/semana de atividade intensa.Percebeu-se uma baixa adesão relacionada ao auto manejo pós cirurgia bariátrica, apenas 10,2% dos pacientes apresentaram bom desempenho no auto manejo. A compulsão alimentar está presente de forma leve em 22,44% dos pacientes e de forma grave 12,24%.Os índices mais relevantes foram os relacionados a comportamento depressivo. 24,48% dos entrevistados apresentaram índices relativos a quadro depressivo leve,14,28% a quadro depressivo moderado, 12,24% a moderadamente severo e 22,44% a quadro severo de depressão.
