In Porto Alegre, Rio Grande do Sul State, Brazil, public primary healthcare (PHC) is provided by traditional Basic Health Units (BHU), the Family Health Strategy (FHS), the Murialdo Health Center (MHC), and the Community Health Service (CHS) of Grupo Hospitalar Conceição. This study compares the quality of adult healthcare among these different types of services. Quality of care was measured by interviews focusing on the experience of physicians and nurses in the services (n = 340), using the Primary Care Assessment Tool. Overall scores for the FHS, MHC, and CHS were significantly higher than for the BHU. The CHS also showed a significantly higher overall score compared to the FHS and MHC. In the multivariate analysis, the variables "specialized training in PHC" and "continuing education" were associated with high overall score. Thus, expansion of FHS coverage and investment in professional training are important potential strategies for improving PHC in Porto Alegre.
A satisfação dos usuários é um importante desfecho para avaliar serviços de saúde. Este estudo trata-se de urna avaliação do grau de satisfação dos usuários em relação à última consulta realizada na rede de atenção primária A saúde de Porto Alegre-RS, tendo como objetivo comparar a satisfação de usuários que avaliam a qualidade do processo de atenção recebida corno sendo de alto escore de APS com a de usuários que a avaliam corno baixo escore de APS. Estudo transversal de base populacional, utilizando o Instrumento de Avaliação da Atenção Primaria, Primary care Assesment Tool-PCATool, que possibilita a classificação dos serviços de saúde no seu grau de orientação à APS por meio do escore geral de atenção primária, de finindo-o corno alto ou baixo, de acordo com a experiência dos usuários. A satisfação do usuário foi medida por um questionário composto de doze questões referentes A Ultima consulta, sendo medida por uma escala Likert de 5 itens: "muito bom", "bom", "regular", "ruim" e "muito ruim". Para a análise, os resultados foram agrupados em duas classificações, sendo "satisfeito", quando referido "muito bom" ou "bom", e "insatisfeito" quando referido algum dos demais itens. Observou-se diferença significativa nas 12 variáveis que refletem a satisfação em vários aspectos da consulta, demonstrando maior satisfação em usuários que classificaram o seu serviço como de alto escore. A avaliação geral da consulta apresentou 95,6% e 73,5% de "satisfeitos" nos serviços com alto e baixo escore de APS, respectivamente (p<0,001). Usuários de serviços corn alto grau de orientação à atenção primária obtêm maior satisfação em suas consultas. Introdução: A aplicação do método clínico centrado na pessoa (MCCP) busca alcançar a compreensão da pessoa e da sua doença por meio da abordagem de duas perspectivas: a do médico e a da pessoa. A satisfação dos usuários é considerada um importante indicador de avaliação dos serviços. Objetivo: O presente artigo tem como objetivo investigar se há associação entre o grau de orientação ao MCCP, segundo a percepção da pessoa, e a satisfação com a última consulta médica em um serviço de atenção primária à saúde (APS). Metodologia: Trata-se de estudo transversal, realizado com pessoas com hipertensão e/ou diabetes nas 12 unidades de um serviço de APS, em Porto Alegre, RS (n = 408). O grau de orientação ao MCCP foi medido pelo instrumento “Percepção do Paciente sobre o Centramento da Consulta” (PPCC). Resultados: O baixo escore geral do PPCC (o que corresponde à alta orientação ao MCCP) esteve associado significativamente com a maior satisfação das pessoas com aspectos relacionados ao serviço (forma de agendamento e cordialidade da recepção) e ao atendimento médico (atenção dada às queixas pelo médico, exame físico do médico, explicações sobre problema e explicações sobre prognóstico), com a consulta em geral e com o tratamento da hipertensão arterial sistêmica e/ou do diabetes mellitus na unidade de saúde. Discussão: O médico de APS deve ser mais orientado à pessoa para que possa atingir, portanto, níveis mais altos de satisfação. Nessa perspectiva, a incorporação da satisfação dos usuários na avaliação dos serviços de saúde deve se constituir em indicador de qualidade, sobretudo, da abordagem praticada pelo médico.
