Plasmacytoma is considered as the initial presentation of multiple myeloma (MM), a devastating and fatal disease. MM is more frequent in men (male-female ratio = 4:1) and generally occurs between the 5th and the 8th decade, affecting with greater frequency vertebrae, skull, pelvic bones, ribs, humerus and femur. According to the WHO, in 2002, MM was the 17th cause of death by neoplasms in the world. We report a case of a 30-year-old woman, referred to the Maxillofacial surgery pregraduate school of the Faculty of Odontology of national autonomous University (Mexico) for a swelling of her left jaw, with an initial diagnosis of infection, and later histopathologically diagnosed as a plasmacytoma. A complete work-up including blood tests, urine tests and total-body x-rays led to a definitive diagnosis of MM, with a rare histologic profile, considered to be having a poor prognosis. The patient was subjected to chemotherapy but she passed away five months later. This case represents one of those undiagnosed cases of malignant neoplasms, considered first as an unsuspected infective lesion and treated as such with antibiotic therapy. The case also highlights the role that could be played by the dental profession in the early diagnosis of bone cancers, which often have a poor prognosis.Le MM est une néoplasie maligne développée à partir des lymphocytes B, caractérisée par une prolifération de plasmocytes
El objetivo del trabajo fue utilizar plasma autólogo rico en factores de crecimiento (PRGF) para mejorar la respuesta quirúrgica, estimulando los mecanismos de reparación, mediante la técnica de regeneración ósea y tisular, desarrollada por Biotechnology Institute, SL. Este es un sistema para la obtención de proteínas, a partir de la propia sangre del paciente momentos antes de su utilización. Se presenta un caso de odontoma compuesto compositum de 3 x 4 cm de diámetro mayor en el proceso alveolar anterior del maxilar y la retención del órgano dentario central superior izquierdo, los cuales fueron enucleados. En el sitio del defecto quirúrgico, se colocó plasma rico en factores de crecimiento (PRGF), con material de injerto de hidroxiapatita reabsorbible, observando un mejor manejo del mismo, así como menor tiempo en la cicatrización de la herida
El objetivo del estudio fue evaluar la respuesta de la pilocarpina en la xerostomía inducida por radioterapia en cabeza y cuello. El diseño de estudio fue descriptivo, prospectivo de corte longitudinal, doble ciego. Los pacientes se distribuyeron aleatoriamente en dos grupos. El grupo 1 (n = 10) de control recibió placebo. Al grupo 2 (n = 10) de estudio se le administró 5 mg diarios de pilocarpina cada 8 h por vía oral. Se realizaron tres cuantificaciones del flujo salival; al inicio del tratamiento, a la quinta y a la séptima semana, por el método de diferenciación de pesos. En la primera cuantificación, al inicio del tratamiento de radioterapia, los pacientes no habían recibido ningún tipo de medicamento, en ningún grupo. Comparando ambos grupos se obtuvo t = 0.525, P = 0.606, por lo que no existía diferencia estadísticamente significativa, en el flujo salival. A la quinta semana se observó en ambos grupos disminución del flujo salival, comparándolos se obtuvo t = 0.481, P = 0.636, por lo que no se encontró diferencia estadísticamente significativa. Sin embargo, el grupo de estudiotuvo un incremento estadísticamente significativo a la séptima semana , del flujo salival, donde se obtuvo una media 0.651 y d.s 0.380, mientras el grupo control mostró una media 0.256, d.s 0.251 al aplicar una t de Student se obtuvo t = 2.743, P = de 0.013. Concluyendo que la pilocarpina es un tratamiento alternativo, viable, seguro y económico, para la xerostomía inducida por radioterapia.
