0 comprometimento do sistema nervoso por blastomicetos é eventualidade relativamente rara. Entretanto, merecem ser assinalados os aspectos clínicos e laboratoriais em que se baseia o seu diagnóstico, para que erros sejam evitados, e para que se recorra a tempo às terapêuticas adequadas. Dentre as afecções com que tais micoses mais se confundem ressalta a meningoencefalite tuberculosa, dada a possibilidade de se associarem às micoses nervosas processos pulmonares da mesma etiologia, e também pelo caráter do liqüido cefalorraqueano, especialmente na torulose. Outrossim, são comuns os diagnósticos de tumor encefálico ou medular nos casos de micoses que assumem a forma granulomatosa ou quando bloqueiam as vias de deflúvio do liquor ventricular. Nos casos pouco esclarecidos de meningoencefalite subaguda ou crônica, ou de tumor encéfalo-medular, não se deve descurar, portanto, da possibilidade de se tratar de uma neuromicose, contra a qual, muitas vezes, podemos lançar mão de recursos eficazes.Acreditamos oportunas, pois, as considerações que faremos a propósito de duas formas de blastomicose (paracoccidioidose e criptococose) do sistema nervoso, com base em três casos por nós estudados. PARACOCCIDIOIDOSE
O presente estudo tem como objetivo demarcar a produção científica brasileira em Gestão do Conhecimento e do Capital Intelectual, entre 1997 e 2011, tendo por foco trabalhos apresentados nos Encontros Anuais da ANPAD - ENANPAD. Tendo como ponto de partida os ENANPAD, foram levantados 9.027 artigos. A operacionalização possibilitou identificar: os artigos relacionados à Gestão do Conhecimento e Capital Intelectual; os temas que os pesquisadores estão abordando; os métodos de pesquisa predominantes em cada trabalho; os autores dos artigos e filiação acadêmica; e os tipos de referências utilizadas nos trabalhos. Em seguida, as informações coletadas foram tratadas quantitativa e qualitativamente, sendo para isso utilizado o modelo de reversibilidade. A pesquisa permitiu identificar 272 artigos relacionados à Gestão do Conhecimento (GC) e Capital Intelectual (CI), com publicação concentrada nos últimos quatro anos, levando a inferir que essas publicações são recentes. Quanto aos autores, percebe-se que os trabalhos em parceria (mais de 80% do total dos trabalhos) prevalecem sobre as publicações individuais. Acredita-se que esse mapeamento contribuirá de forma efetiva para visualização dos atores e do corpo de conhecimento, estimulando o debate sobre a produção científica em GC e CI no Brasil.
O quadro clínico dêsses tumores nada tem de característico. Êles se comportam como processos compressivos medulares de evolução lenta, localizando-se, na maioria das vêzes, em níveis cérvico-torácicos 3, 5 . São freqüentemente confundidos com processos degenerativos ou inflamatórios, afecções que são afastadas pela verificação de bloqueio do canal raqueano mediante manometria e perimielografia. Em geral a natureza histológica do tumor não é suspeitada antes do ato cirúrgico; sòmente a presença de um lipoma subcutâneo situado no mesmo nível da compressão medular (10% dos casos 1 ) pode sugerir o diagnóstico. Em muitos casos, em virtude de seu crescimento lento, os lipomas não chegam a dar sintomatologia clínica, constituindo achados de necropsia em indivíduos falecidos por outras causas 4 .As radiografias da coluna vertebral podem mostrar alargamento do canal raqueano com erosão dos pedículos, dos arcos e dos corpos vertebrais, indicando tratar-se de processo de longa duração. O exame radiológico também pode revelar defeitos congênitos -cifoses, escolioses, espinas bífidas e malformações da transição occípito-cervical -muitas vêzes associados a estas neoplasias 3, 5 . O exame do líquido cefalorraqueano mostra as características de processo compressivo, podendo existir, ou não, bloqueio total à manobra de Stookey. A perimielografia revela processo expansivo, intra ou extramedular, podendo haver ou não bloqueio total ao trânsito do contraste
