A sub-luxação atlanto-axial traz, às vezes, problemas terapêuticos de solução complicada.Descreveremos um caso, conseqüente a febre reumática, que foi tratado com sucesso mediante descompressão cirúrgica por via transorofaríngea. OBSERVAÇÃO D.A.S. 17 anos, sexo masculino, internou-se no Hospital das Clinicas da Faculdade de Medicina da U.F.M.G. em 5-11-1976. Dezoito meses antes da internação, o paciente apresentou dor intensa nas articulações dos membros inferiores, seguida, dois dias após, por dor na nuca, tendo sido medicado, obtendo melhora.Voltando ao meio rural, notou reaparecimento da dor nucal com dificuldade de mobilização do pescoço e desvio da cabeça que era mantida em posição oblíqua com o queixo voltado para a direita.O quadro manteve-se estável até a internação, apenas melhorando a dor na nuca com o uso de analgésicos. Exame físico -Sopro sistólico tipo regurgitaçâo grau V/VI, mais audível no foco mitral irradiando-se para todo o precórdio. O pescoço era mantido em flexão com cerca de 30> de rotação para a direita.Exame neurológico -inteiramente normal. Exames laboratoriais -Muco-proteinas 4 mg%; proteina C reativa++; anti-estreptolisina O 600 unidades Todd; hemossedimentaçao 70 mm/h. Radiografia de coluna cervical mostrou sub-luxaç&o atlanto-axial (Fig. 1); a distância do processo odontóide ao arco do atlas era de 16 mm.O paciente foi colocado em tração trans-esquelética com o aparelho de Crutchfield, não se obtendo porém resolução da sub-luxação.Foi então submetido a mielografia que revelou estreitamento do canal na altura da sub-luxação (Fig. 1). Diante deste quadro foi realizada, por outro cirurgião, uma fixação posterior da coluna cervical com fios de aço e acrílico. O paciente evoluiu bem no pós-operatório e recebeu alta sem deficit neurológico em 14-2-1977, tendo sido recomendado o uso preventivo de penicilinabenzatina.Reinternado em 7-7-1977 por ter havido aumento da flexâo do pescoço. O paciente apresentava tetraparesia com redução de cerca de 40% da força dos membros superiores e 50% da força dos membros inferiores.Havia hiperreflexia tendinea nos membros
