RESUMO -Os autores descrevem episódios de mutismo após cirurgia para tumores da fossa posterior em pacientes jovens. São apresentados dois casos e é feita análise da ausência de expressão verbal que estabeleceu após a cirurgia. Também é feita análise do mutismo do ponto de vista de suas conotações neurológicas e psicológicas, ao mesmo tempo em que é revista a literatura sobre o assunto.PALAVRAS-CHAVE: tumor da fossa posterior, complicação pós-operatória, distúrbios verbais, mutismo. Mutism after posterior fossa tumor surgery: report of two casesSUMMARY -The authors present two cases of mutism after posterior fossa surgery. Two patients aged 16 and 5 years old respectively with astrocytoma and medulloblastoma, developed mutism after the operation. Topographic aspects of the condition are discussed and its neurological expression.KEY WORDS: posterior fossa tumor, post operatory complication, speech disorder, mutism.Alterações da expressão verbal são sempre significativas para o neurocirurgião. Em geral, tais quadros decorrem de lesões do compartimento supratentorial, comumente no lobo dominante. A afasia motora pura seria a ausência máxima de linguagem e impossibilidade de comunicação verbal. Nas lesões da fossa posterior, o sinal deficitário esperado é a disartría, com palavra escandida mas com a manutenção dos pressupostos básicos de comunicação e engajamento ao meio, isto é, a consciência permanece íntegra. De acordo com contribuições recentes registradas na literatura, lesões cirúrgicas da fossa posterior são passíveis de provocarem mutismo 2 . Dois pacientes operados em nosso Serviço, por tumores da fossa posterior, apresentaram no pós-operatório mutismo decorrente da manipulação e exerese do tumor. RELATO DOS CASOSCaso 1. LR (Reg. 55883), branco, masculino, 16 anos. Quadro de hipertensão intracraniana há 8 meses caracterizado por: cefaléia progressiva, holocraniana; dificuldade para enxergar. Ao exame neurológico apresentava edema de papila bilateral. A tomografia computadorizada de crânio (TCC) mostrou massa expansiva na fossa craniana posterior (Fig 1) e hidrocefalia.
