Introducción: La gran mayoría de lesiones que comprometen el conducto auditivo interno corresponden a schwannomas vestibulares (90%Resultados: Los resultados anormales de asimetría de ganancia y de sacadas correctivas fue de 13 pacientes (72%) y con hipoexcitabilidad en la prueba calórica fue de 9 pacientes (50%), con una sensibilidad de la prueba de 72% para vHIT y de 50% para la prueba calórica al compararlo con la resonancia magnética. Conclusión: El v-HIT es una herramienta complementaria a las pruebas calóricas y debe ser incluida como test diagnóstico dentro de la batería de exámenes para evaluar el sistema vestibular en forma completa mediante un vestibulograma.Palabras clave : Schwannomas, v-HIT, vestibulograma. ABSTRACT Introduction: The majority of injuries involving the internal auditory canal correspond to vestibular schwannomas (90%). Within the new diagnostic tests for the vestibular system appears the Head Impulse test and the video head impulse test (v-HIT)which allows the studying of all the semicircular canals.
El abordaje quirúrgico de la patología selar ha sufrido importantes cambios desde sus primeras descripciones. Inicialmente de manejo neuroquirúrgico, hoy en día el abordaje hipofisiario transeptoesfenoidal se ha transformado en una de las vías de acceso más utilizadas por el otorrinolaringólogo para lesiones a nivel de silla turca. Esta publicación describe un tipo de acceso transeptoesfenoidal utilizado por el otorrinolaringólogo en el Instituto de Neurocirugía Dr. Alfonso Asenjo (INCA) en el abordaje de lesiones selares, tanto en cirugía primaria de hipófisis, cirugía secundaria o en casos con alteraciones anatómicas del septum nasal posterior. RESUMENLas fístulas de líquido cefalorraquídeo (FLCR) nasales se producen por la comunicación entre el espacio subaracnoídeo y el tracto aerodigestivo. Pueden ser adquiridas (secundarias a trauma o iatrogenia) o espontáneas. Éstas últimas pueden ser de origen congénito, tumoral o idiopáticas.El canal lateral craneofaríngeo o de Sternberg se produce por la falta de fusión de los puntos de osificación del seno esfenoidal durante el período embrionario. Esta región, ubicada posterolateral a la pared inferior del seno esfenoidal, queda cubierta solamente por tejido conectivo, siendo la zona de menor resistencia de la base de cráneo.La persistencia de este canal puede causar FLCR, especialmente cuando se asocia a elevación de la presión intracraneana y extensa neumatización. La cirugía endoscópica nasal es la técnica de elección para la resolución de estos casos.En el presente artículo se realiza una revisión de la técnica quirúrgica endoscópica nasal para la resolución quirúrgica de encefaloceles secundarios a persistencia del canal de Sternberg, en base a dos casos clínicos que fueron sometidos a cirugía endoscópica nasal.Palabras clave: Canal de Sternberg, encefalocele, fístula de líquido cefalorraquídeo. ABSTRACTNasal cerebrospinal fluid leaks are caused by a nasal communication between the subarachnoid space and the aerodigestive tract. They may be acquired (secondary to trauma or iatrogenic) or spontaneous. The latter can be congenital, tumoral or idiopathic. The lateral craniopharyngeal canal or Sternberg canal is caused by the lack of fusion of the points of ossification of the sphenoid sinus during the embryonic period. La amigdalitis hemorrágica espontánea define al sangrado no iatrogénico de las amígdalas. Es una patología generalmente secundaria a infecciones locales comunes como la amigdalitis, siendo una consecuencia de una erosión de vasos de pequeño calibre localizados en el órgano sangrante y que ha logrado su control en la actualidad gracias al advenimiento de la antibioticoterapia precoz. Se presenta constantemente dentro de las indicaciones de amigdalectomía, pero, en su mayoría, se asume como entidad teórica dada esta baja incidencia y prevalencia. Es importante para su manejo adecuado, mantener la indicación y procurar que ésta no sea olvidada por parte de los especialistas. En esta revisión, presentamos tres casos clínicos de adenoamigdalitis hemorrágica espontánea, revisión de la literatura, destacando un paciente con sangrado exclusivo del adenoides, entidad no descrita previamente en la literatura.
Las lesiones que comprometen la rinofaringe son de difícil diagnóstico debido al complejo acceso a esta región, excepto cuando éstas alcanzan mayor tamaño o cuando presentan síntomas y/o signos secundarios al compromiso de estructuras adyacentes o de diseminación. El propósito de esta investigación fue analizar y describir casos clínicos de pacientes portadores de patología tumoral de la rinofaringe, con la finalidad de aportar nuevos antecedentes en un área en la cual existen pocos estudios publicados, dada su baja incidencia. Se realizó un análisis descriptivo de corte transversal de 10 casos clínicos de pacientes portadores de patologías que comprometían la rinofaringe, evaluados en la unidad de Otoneurología del Instituto de Neurocirugía (1999-2008). De los casos analizados, se observó una mediana de edad de 52,5 años, el tiempo desde la presentación de los síntomas hasta el diagnóstico fue de 8,5 meses. Los tumores de origen epitelial fueron la causa más frecuente, presentándose además tumores de origen óseo y linfoide. El tratamiento fue quirúrgico y asociado a terapia complementaria según necesidad. El número de casos analizados es pequeño, por lo que no es posible sacar conclusiones, sin embargo, dado la clínica inespecífica es fundamental un alto índice de sospecha, e idealmente en el estudio inicial realizar un examen endoscópico en forma rutinaria.
