Residents of urban slums suffer from a high burden of zoonotic diseases due to individual, socioeconomic, and environmental factors. We conducted a cross-sectional sero-survey in four urban slums in Salvador, Brazil, to characterize how poverty and sanitation contribute to the transmission of rat-borne leptospirosis. Sero-prevalence in the 1,318 participants ranged between 10.0 and 13.3%. We found that contact with environmental sources of contamination, rather than presence of rat reservoirs, is what leads to higher risk for residents living in areas with inadequate sanitation. Further, poorer residents may be exposed away from the household, and ongoing governmental interventions were not associated with lower transmission risk. Residents at higher risk were aware of their vulnerability, and their efforts improved the physical environment near their household, but did not reduce their infection chances. This study highlights the importance of understanding the socioeconomic and environmental determinants of risk, which ought to guide intervention efforts.
Este trabalho objetiva-se a identificar e evidenciar a importância da atuação das equipes multiprofissionais da Atenção Primária à Saúde (APS) na prevenção contra o avanço da pandemia ocasionada pela COVID-19. Trata-se de uma pesquisa bibliográfica em que foram utilizadas as bases de dados online: SciELO e Google Acadêmico. Assim, foram selecionados artigos que contemplaram os objetivos e abordagens da APS, utilizando-se de palavras chave como os termos: “Atenção Primária à Saúde”, “Unidade Básica de Saúde” e “COVID-19”. Nos critérios de inclusão foram priorizadas pesquisas nacionais, bem como pesquisas publicadas entre 2017 à 2020. Desse modo, evidenciou-se as potencialidades das ações já realizadas antes da pandemia com os principais grupos de risco, contudo esse novo contexto exigiu novas abordagens na assistência à saúde para a população de cada localidade, como o uso de tecnologias em prol de facilitar os agendamentos de consultas e até mesmo teleconsultas, com renovação de receitas de medicações. Dentre os principais fatores destacaram-se a atenção e acompanhamento constante dos principais grupos de risco e o desenvolvimento de alternativas para respeitar as medidas preventivas de isolamento social, assim como a aplicação de medidas de proteção e prevenção a saúde dos profissionais de saúde.
Leptospirosis, an endemic disease in Brazil, with high lethality, is related to the environmental health of the territories, mainly impacting populations in a state of socioeconomic vulnerability. The present work aimed to investigate the gradients of environmental health in popular neighborhoods of Salvador-Bahia and its influence on Leptospira infection. The investigation involved the collection of primary and secondary data about the environmental health and the occurrence of individuals with antibodies against Leptospira. An Environmental Health Index (EHI) for the areas of study was elaborated, the information was georeferenced, and the data were statistically analyzed. The results indicated that fifteen of the twenty-three study micro-areas had unsatisfactory environmental health conditions and the statistical and spatial analyzes indicated a relation between low EHI scores and higher concentration of individuals with antibodies against Leptospira. In conclusion, the prevention and control of the disease must involve integrated and integral actions for basic sanitation in the territory, surpassing a medicalassistance approach.
In the world of drilling, the drill bit dull condition contains our best forensic evidence of the drilling assembly's interaction with the formation. Dull grading forensics is the first place to look to identify drilling dysfunction yet commonly overlooked or misunderstood by operators. The drill bit dull condition can be leveraged to learn about the formation, drilling dynamics and drilling practices (Watson et. al. 2022). The IADC bit dull grading classification system received its most recent revision in 1992 and currently consists of an average inner and outer dull grade severity, rated from 0 – 8 with a major and other dull characteristic along with a reason pulled. These grades can be used to make critical operational and bit design decisions to overcome drilling challenges thereby improving performance and allowing drilling teams to drill consistently further and faster. The oil and gas industry is becoming more reliant on digitally enabled applications to improve performance through big data, machine learning and automation, but at the time of this paper, the critical IADC dull grading system has remained the same. It is still a crude and subjective characterization of the complex drill bit dull condition. A key challenge with the current classification system and industry standard grading technique is that it is highly dependent on the person grading the bit. Personal subjectivity and lack of training can result in key forensic evidence being overlooked that otherwise could have aided in understanding the root cause of drilling dysfunction. A cross disciplinary committee of subject matter experts (SME's) from operators, drill bit providers, cutter manufacturers, and digital solution providers have convened to define and introduce a new standard dull grading system as replacement for the current outdated IADC dull grading. The new dull grading system will allow for an objective cutter-by-cutter dull grading to be stored with relevant drilling data with reduced subjectivity and enhanced accuracy. With recent advancements in mobile phone hardware and applications, a solution was developed that delivers high quality, cutter-by-cutter dull grading automatically and connecting with drilling meta data from a drilling records database containing over 1.8 million well records with over 5 million bottom-hole assembly (BHA) runs. It leverages videos with machine learning combined with an algorithm to deliver cutter specific, major dull characteristics of a scanned bit. This high quality photographic digital dull information is incorporated into workflows allowing for rapid improvement in cutting structure and cutter development lifecycle timelines leading to rapid improvements in drilling performance for operators.
Background Leptospirosis is a zoonosis caused by pathogenic species of bacteria belonging to the genus Leptospira. Most studies infer the epidemiological patterns of a single serogroup or aggregate all serogroups to estimate overall seropositivity, thus not exploring the risks of exposure to distinct serogroups. The present study aims to delineate the demographic, socioeconomic and environmental factors associated with seropositivity of Leptospira serogroup Icterohaemorraghiae and serogroup Cynopteri in an urban high transmission setting for leptospirosis in Brazil. Methods/Principal Findings We performed a cross-sectional serological study in five urban informal communities in the city of Salvador, Brazil. During the years 2018, 2020 2021, we recruited 2.808 residents and collected blood samples for serological analysis using microagglutination assays. We used a mixed-effect multinomial logistic regression model to identify risk factors associated with seropositivity for each serogroup. Seropositivity to Cynopteri increased with age in years (OR 1.03; 95% CI 1.01-1.06) and was higher in those living in houses with unplaster walls (exposed brick) (OR 1.68; 95% CI 1.09-2.59) and where cats were present near the household (OR 2.00; 95% CI 1.03-3.88). Seropositivity to Icterohaemorrhagiae also increased with age in years (OR 1.02; 95% CI 1.01-1.03) but was higher in males (OR 1.51; 95% CI 1.09-2.10), in those with work-related exposures (OR 1.71; 95% CI 1.10-2.66) or who had contact with sewage (OR 1.42; 95% CI 1.00-2.03). Spatial analysis showed differences in distribution of seropositivity to serogroups Icterohaemorrhagiae and Cynopteri within the five districts where study communities were situated. Conclusions/Significance Our data suggests distinct epidemiological patterns associated with serogroups Icterohaemorrhagiae and Cynopteri within the high-risk urban environment for leptospirosis and with differences of spatial niches. Future studies must identify the different pathogenic serogroups circulating in low-income areas, and further evaluate the potential role of cats in the transmission of the serogroup Cynopteri in urban settings.
Objetivo: compreender os impactos da palhaçoterapia enquanto estratégia de formação para conceitos e práticas de humanização do profissional de saúde. Método: estudo de abordagem qualitativa, de caráter descritivo e exploratório, desenvolvido com 14 profissionais da área da saúde de quatro especialidades que atuaram na extensão por pelo menos um ano. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa. As entrevistas foram realizadas entre dezembro de 2018 e fevereiro de 2019, analisadas através da análise temática de conteúdo. Resultados: emergiram três temáticas: Humanização na formação acadêmica: reflexos nas práticas de saúde; Desafios da humanização: acolhimento x cuidado; Reverberações da palhaçoterapia na formação para a humanização, presentando a palhaçoterapia como facilitadora de habilidades essenciais a humanização, através da valorização do sensível, do acolhimento e da corresponsabilização do cuidado. Conclusões: logo, o lúdico pode ser utilizado como ferramenta para aproximar a essência humana e promover saúde mental, desempenhando um papel terapêutico.
Introdução: Identificar os principais achados da assistência de enfermagem direcionados a pacientes que tiveram hanseníase, desde o espectro mais leve, até os mais graves. Objetivo: identificar os principais achados da assistência de enfermagem direcionados a pacientes que tiveram hanseníase. Metodologia: Trata-se de uma pesquisa bibliográfica, qualitativa que selecionou e reuniu os artigos científicos das seguintes plataformas: CAPES, SciELO e BVS. Como critérios de inclusão foram elencados estudos publicados nos últimos 10 anos, nos idiomas português e inglês. Resultados: Desse modo, foram encontrados 27 artigos que após a aplicação dos critérios estabelecidos restaram 12, que após leitura flutuante reduziu para 10, seguida de uma leitura na íntegra que resultou na seleção dos 5 artigos que compõem esta pesquisa, representados no quadro 1. Na análise temática emergiram duas classificações: “Profissionais de enfermagem: potencialidades e desafios da assistência ao paciente com hanseníase” e “Dados epidemiológicos: contribuições para a assistência de enfermagem na saúde de pacientes com hanseníase”. Conclusão: Apesar da escassez de pesquisas que abordassem a temática, foi possível identificar as contribuições do enfermeiro ao longo da última década e consolidar a sua importância, tanto das de ações de vigilância epidemiológica, quanto na liderança e assistência junto a equipe de enfermagem. Reiterando o impacto das ações de educação em saúde sobre a situação atual e real de cada localidade a qual são responsáveis.
O problema da delinqüência juvenil tem interessado as ciências sociais e humanas. O construtoda busca de sensações, que enfatiza as diferenças individuais, mostrando que a partir da perspectivapersonalística a avaliação do grau com que os jovens pretendem viver experiências em correrriscos e sua predisposição às sensações de novidades e intensidades vem contribuir para explicaçãodo problema. 504 jovens, entre 11 e 20 anos, responderam o questionário de comportamentoagressivo e da busca de sensação. A busca da sensação de intensidade e novidades relacionou-sepositivamente com o comportamento agressivo físico, verbal, raiva e hostilidade. Os traços depersonalidade, no construto da busca de sensação, ainda têm contribuído na explicação da condutajuvenil, especialmente aquelas permeadoras da delinqüência. Tais resultados apontam na direçãode uma orientação das atitudes conscientes frente à excitação e investimento de atividadespermeadoras de risco.
