SUMMARY Current information was obtained from I 17 respondents, including 71 who completed interviews, of a population of 810 individuals who had been diagnosed with cerebral palsy between 1951 and 1974. Original chart reviews constituted the basis for predicting employment capabilities of the interviwed cohort. A comparison of predictions with actual functional outcomes demonstrated a tendency to pessimistic underestimation of'long‐term functional outcome, sounding a note of caution to paediatric developmentalists. RÉSUMÉ Ce qu'il advicni des enfants IMC ‐Prédiction à long terme des devenirs fonctionncls Sur une population de 810 individus, chez qui un diagnostic ayant répondu, dont 71 recontrés dans une entrevue. SUMMARY A group of 97 cerebral‐palsied students between the ages of seven and 16 years were assessed in the 1960s and 1970s, and predictions were made as to their vocational status as adults. In 1983, 60 of the 76 over 18 years of age were contacted. 39 had been employed at some time since leaving high‐school, but only 17 were employed at the time of the survey. Of those who had worked, over half had achieved or exceeded their vocational predictions. Those in competitive employment were more likely to be mildly disabled, to have higher IQs and to have been educated in integrated schools. Those who had never been employed were older and less independent in self‐care and travel in the community. However, intelligence was not a major factor in distinguishing the employed from the never‐employed. Recommendations are made for integrated education and job training in competitive settings, for community‐based assessments and for family involvement in all aspects of planning to improve the vocational prospects of cerebral‐palsied children. RÉSUMÉ Insertion professionnelle prévue et réalisée chez 60 jeunes odultes IMC Un groupe de 97 élèves IMC âgés de sept à 16 ans ont été examinés durant les années 1960–1979 et une prédiction a été faite de leur insertion professionnelle comme adultes. En 1983, 60 parmi les 76 sujets de 18 ans ont été retrouvés. 39 avaient travailléà un moment quelconque après avoir quitté l'école mais 17 seulement étaient effectivement au travail lors de l'étude. Parmi ceux qui avaient travaillé plus de la moitié avait réalisé ou dépassé la prédiction sur l'insertion. Ceux qui étaient insérés dans le milieu normal du travail étaient plus vraisemblablement légèrement atteints sur le plan moteur, avec un QI plus éléve et une bonne éducation en intégration dans une école. Ceux qui n'avaient jamais travailléétaient plus âgés et plus dépendants dans l'autonomie sociale et les déplacements publiques. Cependant, l'intelligence ne s'est pas révélée un facteur majeur distinguant les sujets n'ayant jamais travaillé de ceux qui avaient travaillé. There is a significant difference in the collaborative process between whether a public health nursing research project is requested (of the researchers) by the agency or if it is conceived by an outside investigator. This article discusses the underlying concepts of negotiation, mutuality, and respect that support the process of an externally initiated study in an agency. In a progressive listing format, the important components within the planning, development, implementation, and completion phases are then described so that they can be useful to beginning researchers as a guide and to experienced researchers as a reminder.
The purpose of this exploratory study was to identify the pattern of public health nursing service provided to substance-using mothers and their infants. To determine the effectiveness of public health nursing in a later randomized clinical trial (subsequent study), the first essential step was to identify clearly the nursing services that would constitute the intervention. For consistency, a clinical-practice checklist was developed by the public health nurses (PHNs) with the assistance of the investigators. Over a 6-month period, six PHNs made home, telephone, and office or clinic visits to mothers and their infants who were referred for follow-up due to a positive toxicology screen test at the time of delivery. For this client group (N = 28), the PHNs made one or two home visits to 17 mothers (61%), one or two telephone visits to 12 mothers (43%), and one or two office or clinic visits to 5 mothers (2%). Nearly one third of the time (29%), home-visit attempts were unsuccessful, and the client attrition rate was 50%. Polydrug use was common in this group, with 71% using two or more substances at the time of delivery. Referrals to substance-abuse treatment programs were usually not accepted by the clients, and many referrals were made for medical care and infants supplies. The most frequently mentioned area for advocacy was housing. Overall, public health nursing services were provided in a very limited time span of client availability.
