Objetivo: Compreender por meio de uma revisão narrativa como o autocuidado e as fontes de apoio social influenciam no manejo do Diabetes Mellitus Tipo 1 (DM1). Revisão bibliográfica: DM1 é uma doença autoimune decorrente da destruição das células β pancreáticas, resultando na deficiência completa na produção de insulina. É uma das condições crônicas mais comuns na infância e as suas manifestações clínicas iniciais são, em geral, abruptas. O diagnóstico de DM1 é similar ao do indivíduo adulto e o paciente diabético descompensado pode apresentar algumas complicações clínicas. O autocuidado é um componente fundamental na gestão do DM1, ao propiciar o envolvimento da pessoa em seu tratamento e maior adesão ao esquema terapêutico, minimizando complicações e incapacidades associadas aos problemas crônicos. O apoio social, também, é uma estratégia benéfica para o enfrentamento efetivo do DM1, uma vez que é uma forma de oferta de suporte, apoio afetivo, emocional e material. Considerações finais: DM1 é um desafio constante para os seus portadores, tendo em vista a dificuldade presente em aderir ao novo estilo de vida e às mudanças comportamentais necessárias para promover um controle efetivo da doença. Por isso, o autocuidado e as fontes de apoio social se tornam influências positivas no manejo do DM1.
Exames de triagem neonatal são realizados nos primeiros dias de vida do recém-nascido, ajudam a identificar precocemente algumas doenças e a prevenir futuras consequências. Dentre os exames realizados nessa triagem, disponíveis na Atenção Primária à Saúde, tem-se o teste do pezinho, o qual é fundamental para identificação de distúrbios, como a hiperplasia adrenal congênita (HAC). METODOLOGIA: Trata-se de uma revisão narrativa da literatura com busca nas bases de dados U.S National Library of Medicine (PubMed) e Scientific Eletronic Library Online (SciELO) utilizando os descritores "Adrenal Hyperplasia, Congenital" e "Neonatal Screening". RESULTADOS: Foram identificados seis artigos essenciais para o desenvolvimento deste estudo. Esses trabalhos ocorreram nos últimos três anos, sendo o ano de 2019 o de maior evidência. DISCUSSÃO: A hiperplasia adrenal congênita constitui-se como um grupo de doenças autossômicas recessivas que geram um comprometimento metabólico do cortisol e da aldosterona. Esta pode ser classificada em três tipos e cada uma se apresenta de uma forma no corpo. Nos casos mais graves, pode causar déficit cognitivo ou até a morte. CONCLUSÃO: A triagem neonatal é uma estratégia eficaz para o diagnóstico precoce da HAC e é benéfica no contexto da saúde pública e da qualidade de vida do paciente.
Objetivo: Apresentar aspectos importantes da Atrofia Muscular Espinhal (AME) e analisar os possíveis impactos do rastreio precoce através do Teste do Pezinho Ampliado no prognóstico das crianças acometidas. Revisão bibliográfica: AME é uma doença neuromuscular caracterizada pela degeneração de neurônios alfa na medula espinhal decorrente de uma deleção em 5q11.2-q13.3 que afeta o neurônio motor de sobrevivência 1 (SMN1). Cursa com atrofia muscular progressiva, fraqueza, paralisia e morte. A classificação tradicional divide a AME em 5 tipos, mas sua importância prática vem sendo progressivamente reduzida. A suspeição diagnóstica se dá pelas manifestações características da doença e a confirmação é feita por meio de testes genéticos moleculares. O manejo do paciente com AME envolve medidas não farmacológicas, além de terapia medicamentosa. A triagem neonatal objetiva rastrear doenças em recém-nascidos no período pré-sintomático. Seis doenças estão incluídas no rastreio, mas outras serão incorporadas, incluindo a AME. Considerações finais: Como o sucesso da terapia está associado à precocidade de sua instituição, o rastreio precoce possibilitará melhor perspectiva de cuidado na AME, maximizando os resultados funcionais e promovendo qualidade de vida.
Insuficiência cardíaca (IC) provoca sintomatologia de baixo débito e congestão pulmonar. Sua prevalência tem aumentado, caracterizando-se como a principal causa de hospitalização e elevada taxa de mortalidade. Em pacientes diabéticos, existe associação de risco aumentado de insuficiência cardíaca com fração de ejeção reduzida (ICFER). METODOLOGIA: Revisão narrativa da literatura com busca nas bases de dados PubMed e LILACS, utilizando os descritores "SGLT-2 inhibitors", "heart failure" e "ventricular ejection fraction". RESULTADOS: A amostra final contemplou 17 estudos, sendo o ano de 2020 o de maior evidência. Dentre os artigos selecionados, oito mostraram maior relevância e destacaram-se como principais referenciais teóricos. DISCUSSÃO: IC é uma síndrome na qual há dificuldade do coração em manter a adequada perfusão tecidual. O uso de inibidores de SGLT2 mostra redução significativa do risco de hospitalização por IC. Esses fármacos atuam para reduzir a reabsorção de glicose nos túbulos proximais renais e possuem benefícios no sistema renal e cardiovascular que corroboram com a redução da mortalidade e reinternações decorrentes da ICFER. Além disso, são bem tolerados, sendo que os efeitos adversos graves são pouco frequentes. CONCLUSÃO: Os inibidores de SGLT2 se mostram benéficos em pacientes com ICFER, por proporcionar controle glicêmico e efeitos cardioprotetores e renoprotetores.
