Résumé L'auteur présente une synthèse des travaux théoriques et cliniques de Gisela Pankow concernant la psychose. La lecture des travaux de cette psychanalyste a un caractère fascinant pour diverses raisons : elle fournit de nombreux cas cliniques détaillés et vivants, elle prend en analyse des patients pour lesquels d'autres méthodes de traitement ont échoué. Mais cet aspect spectaculaire risque de faire passer le lecteur à côté de l'essentiel de l'oeuvre qui est une théorisation et une approche, pratique originales concernant la psychose. Le résumé des thèses de Pankow s'avère d'autant plus nécessaire que cette dernière ne prend pas toujours le temps de présenter ses travaux de façon claire et organisée. Les principaux apports de Pankow sont les suivants : l'utilisation de techniques non traditionnelles (face à face, dessins, modelages, contacts avec la famille) dans le traitement du psychotique, la redéfinition de l'image du corps avec une double fonction de forme et de contenu, le développement de la technique des greffes de transfert, par opposition au transfert classique, la redéfinition du concept de forclusion (Lacan) en tant que mécanisme de défense impliquant directement l'image du corps. Le présent texte peut servir d'introduction à la lecture des travaux de Pankow en mettant quelque peu les idées en ordre.
La schizophrénie serait due avant tout à un désordre biochimique et par conséquent le traitement devrait avant tout être d'ordre psychopharmacologique. L'auteur examine les recherches les plus importantes faites à ce sujet pour montrer les lacunes tant au plan des théories étiologiques que des méthodes de traitement, avant tout centrées sur les aspects biochimiques. Les schizophrènes ne sont pas guéris et ils sont à peine soignés, puisque la pensée délirante et le mode de vie psychotique restent présents pendant de longues années. La schizophrénie met en échec le savoir et le pouvoir des thérapeutes en raison de son caractère inexplicable et irréductible.Schizophrenia is caused above all, by a biochemical disorder, and, consequently, the treatment should be primarily of a psycho pharmacological nature. The author examines the most important researches related to this subject to demonstrate the gaps both at the level of actio-logical theories and of treatment methods centered, above all, on biochemical aspects. Schizophrenics are not cured and they are hardly treated, since delusional thoughts and the psychotic life style remain present many years.
