Brucellose ist eine vor allem im Mittelmeerraum verbreitete Zoonose, die durch engen Tierkontakt bzw. durch den Verzehr von Milch und deren Produkten übertragen wird. Wir berichten über einen 58−jährigen in Österreich lebenden türki− schen Patienten, der sich zur weiteren Abklärung von seit 7 Wochen bestehenden, nächtlich beton− ten Rückenschmerzen in unsere stationäre Be− handlung begab. Ein ambulant angefertigtes MRT der Wirbelsäule zeigte eine ausgeprägte Spondylodiszitis BWK8 / 9 sowie BWK10 / 11. Die Anamnese ergab, dass der Patient sowohl an un− dulierenden Fieberschüben sowie starkem Schwitzen und chronischer Müdigkeit litt. Wei− ters bestand ein Gewichtsverlust von 5 kg in den letzten 7 Wochen und der Pat. wurde aufgrund einer depressiven Symptomatik mit 2 Antide− pressiva behandelt. Nach gezielter Anamnese stellte sich dar, dass der Patient sich zwar zuletzt vor 6 Monaten in einem ländlichen Gebiet in der Türkei aufhielt, aber auch in Österreich nahezu ständig von Verwandten mitgebrachten, in der Türkei in Eigenproduktion hergestellten Schafs− und Ziegenkäse konsumierte. Laborchemisch be− standen ausgeprägte Entzündungszeichen. Wir diagnostizierten aufgrund der typischen Klinik sowie einer positiven Blutkultur mit Brucella meli− tensis Bruzellose und behandelten mit einer anti− biotischen Kombinationstherapie: Rifampicin über 8 Wochen und Doxycyclin für 6 Monate. Da− runter besserten sich die Beschwerden ebenso wie die Entzündungsparameter stetig, sodass der Patient nach 4 Wochen beschwerdefrei war. Unser Fall dokumentiert eindrucksvoll, dass auch in Mitteleuropa, insbesondere bei Migran− ten aus dem Mittelmeerraum eine Brucellose bei Spondylodiszitis differenzialdiagnostisch in Er− wägung gezogen werden muss, auch wenn die Reiseanamnese vermeintlich eine akute Infek− tion ausschließt. Abstract !Brucellosis is a widespread zoonosis, especially in the Mediterranean region, and is transmitted by close contact with animals or by consumption of milk and dairy products. We report a 58−year− old Turkish patient living in Austria who was re− ferred for clarification of back pain of 7 weeks du− ration that was particularly pronounced at night. An MRI scan taken as outpatient demonstrated pronounced spondylodiscitis at TH8 / 9 and TH10 / 11. The case history revealed that the pa− tient suffered not only from undulating episodes of fever but also profuse sweating and chronic fa− tigue. Furthermore, there was a weight loss of 5 kg in the past 7 weeks and the patient was being treated with 2 antidepressants for depres− sive symptoms. After specific questioning we found that, although the patient had last been in a rural area in Turkey six months previously, in Austria he almost continuously consumed home− made sheep and goat cheese brought from Tur− key by relatives. Laboratory tests revealed pro− nounced signs of inflammation.
