Rekam medis adalah berkas yang berisi adalah suatu berkas yang berisikan catatan dan dokumen tentang identitas pasien, pemeriksaan, pengobatan, tindakan dan pelayanan lain yang telah diberikan kepada pasien. Rekam medis harus diisi selengkap-lengkapnya sesuai peraturan permenkes, maka dilakukan sosialisasi tentang pentingnya keterisian rekam medis pada fasilitas kesehatan guna meningkatkan pengetahuan siswa SMAN 8 Batam terkait rekam medis. Materi yang disampaikan yaitu tentang rumah sakit, puskesmas, rekam medis, isi dari rekam medis, manfaat dari rekam medis, serta hal-hal yang berkaitan dengan rekam medis pasien di rumah sakit atau fasilitas kesehatan lainnya. Materi yang disampaikan dapat dipahami dengan baik oleh peserta, peserta juga cukup aktif dalam mengikuti kegiatan PKM. Karena keterbatasan jumlah peserta yang mengikuti kegiatan ini, maka sangat disayangkan seluruh siswa kelas 12 tidak dapat mengikuti kegiatan PKM yang membahas tentang rekam medis guna menambah pengetahuan siswa SMAN 8 Batam tentang rekam medis.
Background: Medical records are an important part in assisting the implementation of service delivery to patients in hospitals. This research aimsMethods: Quantitative with survey research, a quantitative approach is used to find out how effective the relationship between waiting time and patient satisfaction is at the Baloi Permai Health Center.Results: The results of the chi square statistical test showed that the p-value of 0.001 was less than 0.050, so it can be said that there is a significant relationship between waiting time and patient satisfaction. The odds ratio for the relationship between waiting time and patient satisfaction is 7.263 with 95% CI between 2.143- 24.614. Patients with long waiting times are 7,263 or 7 times more likely to have a low level of satisfaction compared to patients whose waiting times are not too long.Conclusions: Based on the results of the study, it can be concluded that there is an effect of patient waiting time on outpatient satisfaction. The staff of the Baloi Perma Batam outpatient unit should further improve services, especially for waiting time for outpatients. To speed up the performance of registration officers in health services, education is carried out for patients regarding this medical identity card (KIB) so that patients realize the importance of bringing KIB when going for treatment. This community service activity was carried out at the Batam Botania Health Center, with the aim of providing counseling to patients about the importance of carrying KIB in order to speed up services and make services of a higher quality. The method used in this service is in the form of outreach, where the audience are patients who come to visit at the time of this counseling. Socialization is delivered through lectures with the help of audio-visual media, so that the material can be conveyed properly. The results of the activity show that socialization is going well and it is hoped that patients can receive material and raise awareness of the importance of carrying KIB during treatment.
Background: Medical records are important files or documents for each hospital institution. The process of organizing medical records begins when the patient is received at the hospital, followed by the recording of patient medical data by doctors or dentists or other health professionals who provide direct health services to patients. The objective of the study was to find out the implementation of inpatient medical record services at Home Ibnu Sina Hospital, Padang in 2019.Methods: Qualitative with a system approach of input, process, and output. Research informants was 5 people.Results: In making a decision and policy, not all parties were included, such as medical personnel, paramedics, and medical records officers, tools and materials that support the implementation of medical records, especially where the storage of active medical records is not yet available.Conclusions: The implementation of medical records at RSI Ibnu Sina Padang in 2019 has not been carried out thoroughly as seen from the input, process and output. For this reason, the addition of an active medical record repository, the existence of coordination, integration and synchronization in making decisions and policies so that medical personnel, paramedics and medical records officers are included.
