INTRODUÇÃO: A doença COVID-19, causada pelo vírus SARS-CoV-2, manifesta-se, majoritariamente, pelo envolvimento do trato respiratório inferior. Entretanto, há evidências de que o vírus invade todos os sistemas do organismo, dentre eles, o sistema neurológico. REVISÃO DE LITERATURA: Nessa perspectiva, estudos relataram a presença de manifestações neurológicas em pacientes com COVID-19, as quais podem ser classificadas em primárias ou secundárias. As primárias resultam do acometimento direto do Sistema Nervoso Central (SNC) ou do Sistema Nervoso Periférico (SNP); enquanto as secundárias resultam do acometimento de outros sistemas. Tais manifestações incluem meningite, encefalite, acometimento vascular, ageusia, anosmia e a síndrome pós-covid, na qual os pacientes permanecem apresentando sintomas mesmo após a carga viral se tornar indetectável. Nesse contexto, concomitante ao crescimento de pacientes relatando injúrias ao Sistema Nervoso Central, faz-se necessário compreender os mecanismos dos danos neurológicos e suas consequências para o organismo. OBJETIVO: O presente trabalho objetivou fazer uma revisão de literatura acerca dos acometimentos neurológicos causados pela COVID-19, a fim de compreender como a doença afeta o Sistema Nervoso Central, explorando brevemente sua fisiopatologia. MATERIAIS E MÉTODOS: Esta é uma revisão descritiva, realizada a partir de uma revisão de literatura utilizando as bases de dados Google Acadêmico, PubMed e Scielo.Foram selecionados e analisados seis artigos em inglês, sendo dois americanos, três britânicos e um austríaco. As palavras-chave utilizadas foram: COVID-19, Cérebro, Lesão Encefálica, Manifestações Neurológicas, Sistema Nervoso. RESULTADOS E DISCUSSÃO: Os estudos ainda não apresentam um consenso acerca da fisiopatologia dos sintomas e das sequelas neurológicas presentes em diversos casos da COVID-19. Todavia, o conjunto das recentes pesquisas convergiu em alguns aspectos da doença. Nessa perspectiva, o acometimento neurológico pode ocorrer de forma direta ou indireta. Em relação à forma direta, foi relatado que as possíveis rotas de acesso do SARS-Cov-2 ao cérebro são o trato olfatório, a via vagal, a via trigeminal, a Barreira Hematoencefálica (BHE) e os órgãos circunventriculares. Já a forma indireta se dá por meio da redução do fluxo sanguíneo para o cérebro e pelo estímulo de moléculas imunes que podem causar danos às células cerebrais. No que tange ao mecanismo de lesão encefálica, estudos sugerem que o vírus pode infectar os astrócitos do cérebro e os pericitos de capilares do SNC. Ademais, há evidências de que alguns danos podem resultar de autoanticorpos que conseguem atravessar a BHE. Todos esses fatores podem desencadear sinais e sintomas como anosmia, ageusia, cefaleia, esquecimento, dificuldade de concentração e confusão, os quais, se permanecerem após a infecção, resultarão na síndrome pós-COVID-19.
