Compartment Syndrome is defined as the elevation of tissue pressure above 30 mmHg in a compartment and because comparing the compartment tissue pressure with the diastolic blood pressure below 30 mmHg has proved to be more reliable, this is now a predecessor of Crush Syndrome, which is defined as a post traumatic rhabdomyolysis with systemic distress mainly associated with acute renal failure. The use of amputation as a method to improve the patient’s clinical condition is still controversial, thereby we present the clinical case of a patient rescued from a collapsed building 24 hours after the earthquake that affected Mexico City on September 19, 2017, followed by an updated literature review. Objetivo: comparar la efectividad de la escala RIPASA en adultos con sospecha de apendicitis aguda, con y sin tratamiento previo con analgésicos no esteroideos y establecer si hay modificación en su puntaje por la medicación previa. Material y métodos: se incluyeron 80 pacientes entre 18-65 años de edad, con sospecha de apendicitis aguda atendidos en el Nuevo Sanatorio Durango, quienes fueron evaluados mediante la escala RIPASA; se asignaron a dos grupos en relación al antecedente de tratamiento previo o no con analgésicos no esteroideos. Resultados: en 82.5% de los casos se confirmó el diagnóstico de apendicitis aguda. En el grupo con premedicación se descartó apendicitis aguda en 27.5 versus 7.5% en pacientes sin medicación. La apendicitis aguda tuvo mayor prevalencia (92.5%) en el grupo sin medicamentos antiinflamatorios no esteroideos (AINE) versus 72.5% en el grupo medicado p ≤ 0.019. La náusea/vómito, migración del dolor y anorexia presentaron diferencias estadísticamente significativas entre ambos grupos, siendo más prevalentes en aquéllos con terapia farmacológica p ≤ 0.05. Conclusiones: la premedicación con AINE altera el puntaje de la escala RIPASA en pacientes con sospecha de apendicitis aguda.
La colecistectomía laparoscópica (CL) es el tratamiento ideal para la colelitiasis, pero la persistencia de síntomas postoperatorios es frecuente, a éstos se les conoce como síndrome postcolecistectomía (SPC) y está poco reportado en México, por lo que el objetivo del estudio es determinar su incidencia. Material y métodos: Estudio observacional transversal. Se analizaron 220 expedientes clínicos de pacientes colecistectomizados en 2017, en el Hospital Nuevo Sanatorio Durango. Se evaluaron los cinco síntomas más frecuentes: dolor abdominal en cuadrantes superiores, náusea, vómito, diarrea y distensión abdominal, prequirúrgicamente y a un año de la cirugía. Resultados: El SPC se presentó en 23.1% (n = 36), los síntomas persistentes fueron: distensión abdominal (21.8%), dolor (14.7%), náusea (5.8%), diarrea (4.5%) y vómito (0.6%). Los pacientes con dolor abdominal de tipo biliar, náuseas y vómito redujeron significativamente al año de la cirugía. La cirugía no modificó el riesgo de presentar diarrea y fue un factor de riesgo para presentar distensión abdominal (OR 2.08). Diez pacientes no tuvieron un diagnóstico certero como causa del SPC (32.3%). Conclusión: El SPC se presenta frecuentemente, debe evaluarse de manera correcta y descartar complicaciones postquirúrgicas. Se ha relacionado una mayor incidencia de síndrome de colon irritable tras una CL.
