Introdução: A colangiopancreatografia retrógrada endoscópica (CPRE) é considerado um importante método terapêutico para as patologias que envolvem a árvore bíleopancreática. Atualmente, seu uso com fim diagnóstico está cada vez mais restrito devido a presença de complicações inerentes ao método e que também se relacionam com o perfil dos pacientes, devendo-se conhecer as características desta população que será submetida ao procedimento. Objetivo: Descrever o perfil dos pacientes submetidos a CPRE em um hospital terciário do estado do Ceará. Metodologia: Trabalho retrospectivo realizado a partir da análise de 216 prontuários de pacientes submetidos a CPRE no primeiro semestre de 2017. Resultado: Foram avaliados 216 pacientes, com idades variando entre 18 e 96 anos (média de 56 anos), sendo 84,4% do sexo feminino e 15,6% do sexo masculino. As principais indicações para a CPRE foram: coledocolitíase (71,2%), estenoses benignas das vias biliares (12,5%), neoplasia pancreática (5,5%), dilatação de vias biliares à esclarecer (2,2%), colangiocarcinoma (2,7%), neoplasia de vesícula biliar (1,3%), hepatolitíase (1,3%), pancreatite crônica (0,4%), neoplasia de papila (0,4%) e colangite supurativa aguda (0,4%). Com relação a coledocolitíase, principal indicação da CPRE, o procedimento foi efetivo na retirada dos cálculos em 94,7% dos casos. Conclusão: A casuística do serviço corrobora com a literatura, demonstrando que a coledocolitíase é a principal indicação de CPRE, com predominância no sexo feminino e em indivíduo em torno da quinta década de vida. O procedimento possui alta taxa de sucesso no tratamento desta e de outras patologias pancreatobiliares.
