La infección por el virus de la inmunodeficiencia humana es una enfermedad requiere un control y seguimiento estricto para una adecuada evolución. El virus se puede transmitir de una madre seropositiva al feto durante el embarazo, el parto o la lactancia materna. En pacientes embarazadas, el diagnóstico previo o nuevo de infección es esencial para el inicio temprano de tratamiento antirretroviral, además de controles prenatales periódicos para verificar su cumplimiento con las pruebas inmunológicas y virales. Las pruebas diagnósticas en el paciente pediátrico varían según la edad del paciente. Actualmente se recomienda la terapia antirretroviral para todos los niños y adolescentes con infección.
La fibrosis quística es la enfermedad hereditaria, multisistémica, potencialmente fatal, más frecuente en la población de raza blanca. Se caracteriza por una alteración genética en la proteína del gen regulador de la conductancia transmembrana de fibrosis quística. La afectación pulmonar es la más frecuente y la principal causa de mortalidad. La detección neonatal por medio de la tripsina imunoreactiva de posibles casos de fibrosis quistica, ha logrado la detección temprana de los pacientes y así realizar la confirmación diagnóstica con la prueba del sudor y pruebas de ADN. Los moduladores del gen regulador de la conductancia transmembrana de fibrosis quística son los tratamientos más novedosos aprobados para tratar esta enfermedad y tienen como diana el defecto que genera dicha alteración genética.
La artritis idiopática juvenil (que para efectos prácticos citaremos como AIJ) es una enfermedad sistémica, inflamatoria, crónica y de causa desconocida que afecta a pacientes menores de 18 años de edad. Es considerada la enfermedad autoinmune y reumática más frecuente en niños y adolescentes. La AIJ abarca varios subgrupos diferentes y se presenta predominantemente como artritis periférica. El espectro de la enfermedad se manifiesta a través de varias condiciones clínicas. Por definición, el inicio de la enfermedad antes de los 16 años y la artritis durante más de 6 semanas, son criterios necesarios para el diagnóstico de AIJ. El diagnóstico es clínico y la exclusión se realiza con base en pruebas de laboratorio, con hallazgos que apoyen el diagnostico. Actualmente, con los tratamientos disponibles, los medicamentos modificadores de la enfermedad como el metrotexato y la terapia biológica, son de primera línea; así es posible mantener un adecuado control de la enfermedad y de esta manera evitar las posibles complicaciones.
El Zika es un flavivirus transmitido principalmente por la picadura de mosquitos infectados. Los signos y síntomas pueden incluir erupción cutánea, prurito, fiebre, dolor de cabeza, artralgia, mialgia, conjuntivitis, dolor lumbar y dolor retro orbitario. Se trata de un virus teratógeno, el cual puede provocar microcefalia, entre otras malformaciones congénitas; además se asocia a síndrome de Guillain Barré en adultos. Debido al riesgo de transmisión intrauterina, es importante realizar ecografías seriadas a las gestantes para controlar el crecimiento fetal y la neuroanatomía. Su diagnóstico se realiza pediante sospecha clínica y se confirma con pruebas de amplificación de ácidos nucleicos y pruebas de anticuerpos IgM. Eliminar los lugares donde el mosquito pone huevos, principalmente contenedores artificiales que contienen agua además del uso de repelentes tópicos contra insectos, es de vital importancia como medidas de prevención. El tratamiento es sintomático, mediante medidas de soporte como hidratación, analgesia y antihistamínicos.
La esclerosis lateral amiotrófica es una enfermedad progresiva que se caracteriza por la degeneración de las neuronas motoras en el cerebro y la médula espinal. Existen dos formas fundamentales de esta enfermedad: la forma esporádica y la forma familiar. Aún no existe claridad sobre su etiología. Las manifestaciones clínicas incluyen signos de daño de las neuronas motoras superiores e inferiores, tanto en las extremidades como en la musculatura bulbar. El diagnóstico continúa siendo fundamentalmente clínico, apoyado por estudios neurofisiológicos. No existe tratamiento curativo. El riluzole es el tratamiento farmacológico de primera línea que ha demostrado retrasar el curso normal de la enfermedad. El edaravone está aprobado por la FDA para el uso en esta enfermedad en conjunto con el riluzole.
