Fisher descreveu, em 1956, a síndrome de oftalmoplegia, ataxia e arreflexia em três pacientes 4 -Enfatizou sua relação com a síndrome de Guillain-Barré e comentou a semelhança do quadro apresentado por um de seus pacientes com as manifestações de uma das formas clínicas de oftalmoplegia incidindo no curso de polineurite infecciosa, descrita segundo ele por Collier em 1932. O pequeno número de casos registrados na literatura, as controvérsias existentes sobre a topografia das lesões e a escassez de dados referentes ao exame otoneurológico, motivaram esta publicação. OBSERVAÇÃO L.J.P., 27 anos, branco, brasileiro, registro 101537-HC. Atendido em 03/08/82, com história de virose de vias aéreas superiores há 10 dias. Há 7 dias, concomitantemente ao desaparecimento dos sintomas de infecçâo, surgiram subitamente cefaléia supra--orbitáría à direita e vertigens; há três dias, diminuição da acuidade visual à direita e diplopia; há um dia, sensação de parestesias nas extremidades. Exame fisicoBom estado geral, eupneico, afebril. PA β 13/8 cm de Hg; freqüência cardíaca ¢= 70 batimentos/minuto; temperatura axilar <= 37oC. Sem outras alterações, exceto as do exame neurológico: marcha atáxica, discreta diminuição global de forca muscular, arreflexia profunda apendicular global, hipopalestesia em extremidades e hipoestesia táctil palmo-plantar. Oftalmoplegia externa completa, com ptose palpebral discreta à esquerda e elevador da pálpebra integro à direita. Anisocoria, a pupila direita sendo maior que a esquerda; reflexos fotomotores bradirreagentes e reflexo de acomodação normal. Disfagia, disfonia e paralisia facial periférica discretas. (Fig. 1). Exames complementares -Glicemia de jejum: 120 mg/100ml. Teste de tolerância à glicose; na fase aguda, discreta alteração da curva glicêmica compatível a intolerância a carbo dratos; dois meses após o inicio do quadro, normal.
