During pregnancy, a large part of the body is affected by the physiological and anatomical changes that start in early pregnancy. Many of these changes significantly affect the pharmacokinetic (absorption, distribution, metabolism, and excretion) and pharmacodynamic properties of different drugs (1-3). During treatment, the use of medicines with the content of only some active ingredients is important to ensure maternal and fetal safety during pregnancy (4-6).Drug-nutritional element interactions are defined as the physicochemical, physiological, and pathophysiological relationships between a drug and nutritional element or between a drug and multiple nutritional elements, nutrients, and constituents or the nutritional status in a broader sense (6-8). Drug-nutritional element interactions can occur in four steps: extracellular bioinactivation, decrease/increase in absorption, decrease/increase in efficacy, and decrease/ increase in excretion (elimination) (7). Therefore, the fact that health professionals consider the physiological changes and the factors affecting the distribution of the drug in the body during pregnancy is important to ensure the effectiveness of pharmacotherapy. In this review article, we aimed to investigate the effects of physiological changes during pregnancy on the pharmacokinetic properties of the drug and the interactions of drugs, nutrients, and nutritional elements. Physiological Changes and Drug Metabolism in PregnancyPregnancy is a process in which many physiological differentiations occur in the cardiovascular, respiratory, excretory, gastrointestinal, and circulatory systems, which alter the interactions of nutrition, nutritional element, and drug metabolism. In summary, the rate and volume of the heart increase, lung capacity decreases, glomerular filtration rate (GFR) increases, blood volume increases, blood circulation decreases, and gastrointestinal system (GIS) motility decreases; in addition, changes, such as nausea and vomiting, occur affecting nutrition. The physiological changes associated with pregnancy are summarized in Table 1. During pregnancy, the interactions between drug metabolism and nutrition and nutritional element may be pharmaceutical, pharmacokinetic, and pharmacodynamic. The pharmaceutical interactions occur within the delivery medium, such as enteral tube or gastrointestinal lumen. This can affect the bioavailability of the drug or nutritional element (8). ÖzIn recent years, the role of chronic low grade inflammation in the emergence of chronic diseases has been emphasized. Chronic low grade inflammation is a metabolic process characterized by systemic circulation and increased inflammatory mediators and some proinflammatory cytokines in various tissues. C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-1 (IL-1), interleukin-4 (IL-4), interleukin-6 (IL-6), toll like receptor-4 (TLR-4) are some proinflammatory molecules that play a role in the chronic low grade inflammation. Recent studies have shown that fructose and saturated fatty acids, which are consumed with increased consumption of processed foods in the current diet, might trigger chronic low grade inflammation. In this review, the effects of dietary high fructose and saturated fatty acids on development of chronic low grade inflammation through some proinflammatory mediators were examined.
Günümüzde hazır işlenmiş besinlerin tüketimindeki artış kronik hastalıkların ortaya çıkmasında önemli bir sağlık tehditi olarak değerlendirilmektedir. Batı toplumlarında en sık karşılaşılan kronik karaciğer hastalıklarının başında alkole bağlı olmayan karaciğer yağlanması hastalıkları bulunmaktadır. Bu hastalıkların başlatıcısı olarak kabul edilen hepatik steatoz, genellikle insülin direnci ile seyreden, karaciğerde yüksek miktarda lipid depolanması ile karakterize bir bozukluktur. Diyetle alınan yağ asitlerinin tür ve miktarı karaciğerde lipid metabolizması, insülin duyarlılığı ve inflamasyonu etkileyerek hepatositlerde aşırı miktarda lipid birikimi ve dolaylı hücresel disfonksiyona neden olabilmektedir. Doymuş yağ asitlerinin uluslararası rehberlerde önerilen düzeylerin üzerinde tüketilmesi, karaciğerde yağ asit biyosentezi, lipid metabolizması ve insülin sinyalizasyonunu etkileyebilmektedir. Böylece oluşan hepatik steatoz, pro-inflamatuvar yolakların aktivasyonu ile ilişkili sitokinlerin sentezi ve Tollbenzeri reseptörler gibi kalıp tanıma reseptörlerinin aktivasyonunu artırmaktadır. Diyette doymuş yağ asitleri ve alkole bağlı olmayan hepatik steatoz ve altında yatan mekanizmalar ile ilgili güncel çalışmalar artarak devam etmektedir. Bu derleme, diyetle yüksek doymuş yağ asitleri alımının hepatik steatoz oluşumu mekanizmalarına olan etkilerini değerlendirmek amacıyla yazılmıştır.
Dünyadaki ölüm nedenleri arasında ilk sırada yer alan kardiyovasküler hastalıkların önlenmesi ve tedavisi için diyet büyük önem arz etmektedir. Kardiyovasküler sistem üzerine etkili beslenme programları genellikle lipid ve karbonhidratlar üzerinde odaklanmışken proteinlerin etkisi göz ardı edilmektedir. Bu nedenle bu derleme; diyetteki proteinlerin, aminoasitlerin ve bazı diğer aminli bileşiklerin kardiyovasküler sağlık üzerine etkilerinin incelenmesi amacıyla yazılmıştır. Hayvansal veya bitkisel kaynaklı proteinler; süt ve süt ürünleri, yumurta, et, tavuk ve balık etleri, baklagiller ve tahıllar gibi besinler farklı protein kompozisyonuna sahip oldukları için kan basıncı ve lipid profilinin regülasyonu üzerine etkileri değişmektedir. Proteinleri oluşturan aminoasitler de protein kaynaklarına benzer olarak kardiyovasküler sistem üzerine farklı etkilere sahiptir. Bu açıdan kükürtlü aminoasitler başta olmak üzere aromatik aminoasitler, dallı zincirli aminoasitler, arjinin, ornitin, sitrulin, glisin ve glutamin aminoasitleri kardiyovasküler sistemi farklı metabolik yollarla etkilemektedir. Bu kapsamda tek karbon metabolizması, hormon sentezi, sinyal yollarının uyarılması ve aminoasitlerin metabolizmaları sonucunda oluşan ara ve son ürünlerin etkileri sayılabilir. Protein ve aminoasitlerin dışında diyetle alınan diğer aminli bileşikler içinde trimetilamin N-oksit, heterosiklik aromatik aminler, polisiklik aromatik hidrokarbonlar ve maillard reaksiyonu ürünleri yer almaktadır. Bu aminli bileşikler genellikle oksidatif stresi, inflamasyonu ve aterosklerotik plak gelişimi tetikleyerek kardiyovasküler hastalıklar için risk oluşturabilmektedir. ÖZ Diyetteki fruktozun kaynakları bal, meyve ve sükrozun yanı sıra yüksek fruktozlu nişasta bazlı şeker ile tatlandırılmış çeşitli yiyecek ve içeceklerdir. Fruktoz; diyetle alınan toplam enerji miktarı ile lipogenezi arttırarak obeziteye yol açabilmektedir. Alım dozu, süresi ve fruktozun kaynağına bağlı olarak, alınan fruktozun çoğu metabolize edilerek de novo lipit sentezine katılabilmektedir. Kan glikoz ve insülin seviyelerini etkileyerek insülin direnci oluşumu, non-alkolik karaciğer yağlanması ve böbrek hastalıkları riskini de arttırabilmektedir. Ayrıca yüksek fruktoz tüketimi organizmadaki inflamatuar süreçleri başlatabilmektedir. Öte yandan tipik batı tarzı diyette yüksek fruktozla birlikte yüksek yağlı veya tuzlu beslenme fruktozun kronik hastalıklar üzerindeki bu olası etkilerini arttırabilmektedir. Sonuç olarak klinik çalışmalarla tam olarak desteklenmemiş olsa da deneysel çalışmaların sonuçlarına göre yüksek miktarda fruktoz alımının kronik hastalık riskini arttırabileceği düşünülmektedir. Ayrıca tipik batı tarzı yüksek yağlı, şekerli ve tuzlu diyetle birlikte fazla miktarda fruktoz tüketiminin obezite, kardiyovasküler hastalıklar gibi kronik hastalıkların riskini arttırabileceği ve metabolik sendrom parametrelerini daha fazla kötüleştirebileceği unutulmamalıdır. Ancak sentetik fruktoz, yüksek miktarda alındığında bazı olumsuz metabolik etkilere neden olabilse de kaynağı meyve ya da bal olan fruktozun yüksek miktarda tüketiminde bu olumsuz etkiler görülmemekte ya da tüketim miktarına bağlı olarak daha az oluşabilmektedir. ABSTRACTDietary sources of fructose are not only honey, fruit, sucrose, but also high fructose corn syrup in various foods and beverages. Total amount of daily fructose intake is rising by especially increasing use of high fructose corn syrup in the food industry. Fructose can lead to obesity by contributing to high-energy intake and lipogenesis in the body. Depending on the source of fructose, dose and duration, it was involved in de-novo lipid synthesis. Fructose may increase the risk of insulin resistance, non-alcoholic fatty liver and kidney diseases by affecting blood glucose and insulin levels. On the other hand, fructose may initiate inflammatory processes in the organism. In addition to these, fat or salt consisting typical western type diet with high fructose consumption, can increase the potential effect of fructose on chronic diseases. As a result, although it is not fully supported by clinical studies, it is thought that high amounts of fructose intake may increase the risk of chronic disease shown by experimental studies.Also it should be noted that beside high fructose, typical western-style high-fat and high-salt diet may increase the risk of chronic diseases such as obesity, cardiovascular diseases and worsen metabolic syndrome parameters.
