Ter acesso à água fluoretada é um reconhecido fator de proteção contra a cárie dentária. No Brasil, a fluoretação da água de abastecimento público tornou-se obrigatória por lei em 1974, seguindo-se a esta regulamentação acentuada expansão da cobertura, sobretudo nas regiões de maior desenvolvimento socioeconômico. A ampliação dessa cobertura em todo o país é uma das prioridades da política nacional de saúde bucal. Neste artigo, sistematizam-se informações sobre a implantação e expansão da fluoretação no Estado de São Paulo, no período de 1956 a 2009, utilizando-se dados secundários obtidos em relatórios técnicos, documentos oficiais e no sistema SISAGUA. Em 2009, a cobertura se estendia por 546 (84,7%) dos 645 municípios paulistas, chegando a 85,1% da população total e a 93,5% da população com acesso à rede de distribuição de água. Tais resultados indicam que a medida está consolidada como parte da política estadual de saúde. No entanto, persiste o desafio de implantar e manter a fluoretação em 99 municípios, beneficiando cerca de 6,2 milhões de habitantes excluídos do benefício.
Introdução-A cárie dentária representa, ainda, em termos de saúde bucal, o principal agravo em saúde pública no Brasil. A fluoretação da água de abastecimento público tem participação comprovada na redução dos índices de prevalência, tendo sido considerada uma das dez grandes conquistas da saúde pública do século XX. Regulamentada no país em 1974, a medida teve acentuada expansão nos anos 80, sobretudo nas regiões de maior desenvolvimento sócio-econômico, como o Estado de São Paulo. A ampliação da cobertura da fluoretação aos municípios com sistemas de tratamento de água é uma das prioridades da política nacional de saúde bucal. Objetivos-Identificar, reunir e sistematizar informações disponíveis sobre a cobertura da fluoretação e descrever sua evolução no Estado de São Paulo, no período de 1956 a 2009. Material e Método-Estudo descritivo, com utilização de dados secundários, tendo como unidade de análise os municípios do Estado de São Paulo, descrevendo um histórico da cobertura desde a instituição da medida. A apresentação dos dados é feita por meio de tabelas, gráficos e mapas temáticos, produzidos a partir de sistemas de informações geográficas. Resultados-A cobertura da fluoretação no Estado de São Paulo evoluiu de apenas um para 546 municípios, no período de 1956 a 2009. O Estado apresenta, em 2009, 85,1% da população total coberta pela medida e 93,5% da população com acesso à rede de distribuição de água, servida por água fluoretada. A maioria (99%) da população sem acesso reside em municípios de até 50 mil habitantes. Conclusão-A expansão foi lenta, com rápido e expressivo aumento nos anos 80, mas consistente ao longo do tempo, mostrando que o Estado de São Paulo consolidou esta importante medida de saúde pública como parte da política estadual de saúde. Apesar da evolução favorável neste período, houve interrupção da fluoretação em 35% dos municípios que a implementaram até o ano de 1988. Tendo em vista que todos os municípios paulistas contam com sistemas de tratamento de água e a regulamentação nacional dispõe que todos os sistemas incluam a fluoretação como etapa do tratamento, persiste, ainda, o desafio de implantar e manter este recurso em 99 municípios. Não obstante sejam todos de pequeno porte demográfico, suas populações têm direito a esse benefício e cabe às autoridades públicas tornar esse direito uma realidade, visando assegurar a diminuição das desigualdades em saúde.
