Our study showed that self-efficacy, chance locus of control, and intrinsic religiosity were associated with non-adherence to immunosuppressives. A broader perception of the kidney transplant patient´s integrality can help health professionals to design strategies to promote adherence in this population.
Recebido em 15/6/11; aceito em 16/9/11; publicado na web em 3/11/11 A simple, rapid and selective method using high-performance liquid chromatography with ultraviolet detection (267 nm) was applied for the determination of tryptophan in plasma. Separation was carried out on a C18 column (150 x 4.6 mm internal diameter) in 6 min. The mobile phase consisted of 5 mM the sodium acetate and acetonitrile (92:8, v/v). The method was shown to be precise and accurate, and good recovery of analyte was achieved, characterizing the method as efficient and reliable for use in laboratory analysis.
Background Nonadherence (NAd) to antihypertensive medication is associated with lack of blood pressure control and worsened long‐term outcomes. Increased access to a programme for high‐risk cardiovascular patients has the potential to reduce NAd and improve clinical outcomes. We evaluated implementation NAd prevalence and risk factors among severely hypertensive patients after 12‐month‐long access to secondary healthcare centres. Methods The Morisky Green Levine Scale (MGLS) was used to analyse antihypertensive medication NAd in a prospective cohort of 485 patients. Logistic regression models evaluated the influence of ecological model factors on NAd. Results The majority of patients were female, had low health literacy, a low family income and a mean age of 61.8 ± 12.5 years. Prevalence of NAd fell from 57.1% at programme entry to 28.3% (P < .001) at the end of the study. After access to a secondary healthcare centre, we observed better blood pressure control, an increase in the number of pills/day and a higher number of antihypertensive medications. Predictive variables of NAd were age (OR 1.027; CI 1.003‐1.051; P = .023), low health literacy (OR 1.987; CI 1.009‐3.913; P = .047), systolic blood pressure (OR 1.010; CI 1.003‐1.021; P = .049), dosages ≥ 2 times/day (OR 1.941; CI 1.091‐3.451; P = .024) and patient satisfaction with the healthcare team (OR 0.711; IC 0.516‐0.980; P = .037). Conclusions Greater access to health services is associated with a reduction in NAd to antihypertensive medication and better blood pressure control. NAd was correlated with modifiable variables such as treatment complexity and, for the first time, team satisfaction, suggesting that implementation of similar programmes may limit NAd in similar patient groups.
Background: Low health literacy (LHL) is common among patients with chronic diseases. Studies on the association between LHL and uncontrolled blood pressure are limited to primary and tertiary levels of healthcare. Objective: We evaluated the prevalence and association between LHL and uncontrolled blood pressure in hypertensive patients in a secondary healthcare in Brazil. Material and methods: Our study had a cross-sectional design and included 485 patients, between August/2014 to March/2016. We evaluated health literacy, by Short Assessment of Health Literacy for Portuguese-Speaking Adults (SAHLPA-18), and the blood pressure control. Individuals with a score ≤14 in SAHLPA-18 were considered as LHL, and individuals with blood pressure ≥140/90 (or ≥130/80 in diabetic patients) were considered as having uncontrolled blood pressure. Results: A total of 56.0% female, with mean age 62.0±12.6 years. Illiterate participants were 61.6%, and 65.4% earned up to one reference wage. Both uncontrolled blood pressure and LHL were highly prevalent (75.1% and 70.9%, respectively), but there was no association between these two parameters. In multivariate analysis, uncontrolled blood pressure was associated with age (OR:0.96, CI:0.94-0.98, p<0.001), diabetes mellitus (OR:4.36, CI:2.54-7.51; p<0.001) and number of pills (OR:1.16, CI:1.08-1.25, p<0.001). Conclusion: Even we found a high prevalence of LHL, the lack of association between LHL and uncontrolled blood pressure may be due to demographic characteristics of the sample, i.e., elderly people with low income and low schooling. HL assessment provides important information that supports actions to improve hypertension control and treatment.
