Purpose To determine whether using different intraperitoneal insufflation pressures for transperitoneal laparoscopic urologic surgeries decreases postoperative pain.Materials and Methods 76 patients who underwent transperitoneal laparoscopic upper urinary tract surgery at different insufflation pressures were allocated into the following groups: 10mmHg (group I, n=24), 12mmHg (group II, n=25) and 14mmHg (group III, n=27). These patients were compared according to age, gender, body mass index (BMI), type and duration of surgery, intraoperative bleeding volume, postoperative pain score and length of hospital stay. A visual analog scale (VAS) was used for postoperative pain.Results Demographic characteristics, mean age, gender, BMI and type of surgeries were statistically similar among the groups. The mean operation time was higher in group I than group II and group III but this was not statistically significant (P=0.810). The mean intraoperative bleeding volume was significantly higher in group I compared with group II and group III (P=0.030 and P=0.006). The mean length of postoperative hospital stays was statistically similar among the groups (P=0.849). The mean VAS score at 6h was significantly reduced in group I compared with group III (P=0.011). At 12h, the mean VAS score was significantly reduced in group I compared with group II and group III (P=0.009 and P<0.001). There was no significant difference in the mean VAS scores at 24h among three groups (P=0.920).Conclusion Lower insufflation pressures are associated with lower postoperative pain scores in the early postoperative period.
Kliniğimizde gerçekleştirilen ürolojik laparoskopik cerrahi olguların sonuçları ve komplikasyonlarını geriye dönük olarak değerlendirmek. Yöntemler: Ocak 2012 -Ocak 2015 tarihleri arasında laparoskopik operasyon geçiren toplam 115 hasta geriye dönük incelendi. Hastaların demografik özellikleri, preoperatif tanıları, laparoskopik yaklaşım şekli, ameliyat ve yatış süreleri, cerrahi işlem esnasında ve sonrasında gelişen komplikasyonlar ve açık cerrahiye geçilen hastalar açısından incelendi. Bulgular: Hastaların 61'i kadın, 54'ü erkek ve ortalama yaşları 52,4±11,7 yıl idi. Altmış sekiz olguda transperitoneal, 47 olguda ise retroperitoneal girişim uygulandı. Toplam 29 hastaya basit böbrek kisti eksizyonu, 25 hastaya basit nefrektomi, 22 hastaya üreterolitotomi, 19 hastaya radikal nefrektomi, 15 hastaya piyelolitotomi ve 5 hastaya da piyeloplasti yapıldı. Toplam 115 vakanın 4'ünde (%3,4) açık operasyona geçildi. Bu hastalar dışında majör komplikasyon ve mortalite görülmedi. En sık yapılan ameliyatların ortalama süreleri ise; böbrek kist eksizyonu 62 (50-110) dk, basit nefrektomi 125 (95-140) dk, üreterolitotomi 108 (90-130) dk, radikal nefrektomi 141 (105-175) dk, piyelolitotomi 116 (95-140) dk ve pyeloplasti 166 (150-190) dk idi. Ortalama yatış süresi 3,7±2,8 gün (2-11) idi.Sonuç: Uyguladığımız laparoskopik cerrahilerin sonuçları, başarı ve komplikasyon oranları literatürle uyumlu bulundu. Laparoskopik cerrahi, teknolojinin gelişmesi, deneyimlerin artmasıyla ayrıca hastalar tarafından daha iyi tolere edilmesi nedeniyle açık cerrahiye alternatif, güvenli ve minimal invaziv bir tedavi yöntemi olduğunu düşünmekteyiz.
Complete disruption of the urethra due to penile fracture is a very rare occurrence. Herein, we report a case of a 39-year-old man who presented with penile fracture and complete disruption of the urethra as a result of sexual intercourse and present a review of the literature on penile fracture. Surgical exploration at presentation was performed and primary repair was done. In follow-up, the patient reported moderate erectile dysfunction which over time responded to phosphodiesterase-5 inhibitor drugs. Early surgical repair and evaluation for concomitant urethral injury in case of penile fracture is highlighted in this report.Penil fraktür nedeniyle üretranın tam kopması nadiren görülür. Burada, cinsel ilişki sonucu üretra tam kopma ile beraber penil fraktür ile başvuran 39 yaşındaki bir erkek olguyu literatür eşliğinde sunuyoruz. İlk başvuruda hastaya cerrahi eksplorasyon yapıldı ve primer onarım uygulandı. Takipte, hasta fosfodiesteraz-5 inhibitör ilaçlarına cevap veren modarete erektil disfonksiyon bildirdi. What's known on the subject? and What does the study add? Amaç: Bu çalışmanın amacı, lokalize prostat kanseri nedeniyle radikal prostatektomi yapılan hastalarda preoperatif trombosit kitle indeksi (PMI) ile tümörün patolojik özellikleri ve postoperatif biyokimyasal nüks arasındaki ilişkiyi araştırmaktır. Gereç ve Yöntem: Nisan 2004 ile Nisan 2017 arasında lokalize prostat kanseri nedeniyle radikal prostatektomi yapılan 141 hastanın verileri retrospektif olarak tarandı. Hastaların yaş, preoperatif prostat spesifik antijen (PSA), trombosit sayısı, ortalama trombosit hacmi ve PMI değerleri, N/L oranı, tümör evresi, Gleason skoru, tümör hacmi, lenf nodu tutulumu, cerrahi sınır pozitifliği ve 3. ayda biyokimyasal nüks olup olmadığı kaydedildi. Ameliyat öncesi PMI değerinin yaş, PSA, patolojik parametreler, cerrahi sınır pozitifliği ve biyokimyasal nüks ile olan ilişkisi incelendi. Bulgular: Hastaların yaş ortalaması 61,79±5,98 yıl, ortalama PSA değeri 9,50±6,69 ng/mL, ortalama PMI değeri 2003,91±486,69 ve ortalama N/L oranı 2,79±2,06 idi. PMI değeri ile PSA, patolojik evre, Gleason skoru, lenf nodu tutulumu, tümör hacmi, cerrahi sınır pozitifliği, biyokimyasal nüks arasında bir korelasyon izlenmedi. PMI ile Gleason skoru arasında istatistiksel olarak anlamlı negatif korelasyon izlendi. Sonuç: Çalışmamızda prostat kanseri nedeniyle radikal prostatektomi uygulanmış hastalarda ameliyat öncesi PMI değerini, erken biyokimyasal nüks için prognostik bir faktör olarak gösteremedik. Genelleme yapabileceğimiz daha iyi bir sonuç elde etmek için, daha geniş hasta serileri ile prospektif olarak tasarlanmış çalışmalara ihtiyaç vardır.
Background To evaluate current approaches to ionizing radiation by personnel working in urology operating room at all levels of health service in Anatolia, Turkey. Methods A questionnaire was e-mailed to urology operating room personnel including MDs at different academic levels, between August and September 2020 in Anatolia, Turkey. The questionnaire included demographic questions and questions about ionizing radiation, exposure and protection. Results Of the 173 (11.9%) respondents, 44.5% were doctors, 24.9% were nurses, 20.8% were radiology technicians, and 9.8% were other operating room personnel. While most participants (73.4%) had some knowledge of radiation, only 42.2% stated they had received radiation-specific training. The two most commonly used personal protective equipments (PPEs) among respondents (79.8%) were lead aprons and thyroid protectors. Interestingly, both types of PPE were preferred among those who had not received radiation safety training. Conversely, besides lead aprons and thyroid protectors; gonadal protection, goggles and gloves were the preferred PPEs among respondents who had received radiation training (p < 0.001). Radiology technicians exhibited the highest compliance rate for dosimeter usage (100%), followed by faculty members (37.5%), urology specialists (36.7%) nurses (27.9%), operating room staff (17.6%) and assistants (15%). While differences in compliance were statistically significant (p < 0.001), no significant relationship was found between radiation protection measures and years of experience, daily fluoroscopy exposure or education level. Conclusions Despite advances in medical science, our findings suggest that many hospital personnel are still insensitive to the dangers of fluoroscopy, which is of great importance in urology operating rooms.
