Oxidative DNA damage is an intervening variable for TP53 mutation in colorectal adenoma-carcinoma. Our data suggests that oxidative DNA damage is a potential marker of TP53 mutation in colorectal carcinogenesis.
ResumoNos últimos anos, o ensino médico vem sofrendo modifi cações pedagógicas e curriculares, visando promover a formação de um novo profi ssional de saúde. Uma das alternativas pedagógicas propostas é a educação problematizadora, que trabalha essencialmente com a construção de conhecimentos a partir da vivência de experiências signifi cativas. Tal modelo de aprendizagem tem viabilizado mudanças curriculares, em que a psiquiatria vem conquistando espaço signifi cativo para o desenvolvimento de uma prática médica mais humanística e integral. O artigo se propõe a discutir a experiência do Curso de Medicina da Universidade São Francisco, em Bragança Paulista (SP) referente à implantação do internato de psiquiatria pela metodologia da aprendizagem baseada em problemas, desde 2003. O internato de psiquiatria ocorre no quinto ano do Curso de Medicina, com carga horária de 320 horas, correspondendo a 8 semanas de 40 horas. As atividades são desenvolvidas no hospital-dia e no ambulatório, e a interconsultoria é realizada no hospital geral. Observa-se signifi cativa evolução no conhecimento adquirido dos alunos, que conseguem, ao término do internato, desenvolver raciocínio clínico e sedimentação de conceitos e práticas psiquiátricas adequadas à formação médica generalista, compatíveis ao que o curso de graduação propõe. A experiência relatada pode servir de modelo para a implantação de uma proposta de aprendizagem em saúde mental inserida nos preceitos da reforma psiquiátrica no Brasil, buscando-se implementar o desenvolvimento de práticas comunitárias e interdisciplinares. AbstractIn the last few years, medical education has been going through curricular and pedagogical changes with the purpose of educating a new health professional. One of the pedagogical alternatives is the education based on problem-solving, which works essentially with the construction of knowledge using signifi cant experiences. RESUMOIntrodução: O modelo de formação médica no Brasil vem apresentando importantes modificações nos últimos anos. Com o lançamento do Programa Mais Médicos em 2013, cujo propósito era a formação de recursos humanos na área médica para atuarem preferencialmente em áreas carentes, sob a referência do Sistema Único de Saúde (SUS), e o posterior lançamento das Diretrizes Curriculares Nacionais, no ano seguinte, uma nova realidade se apresentou em relação ao internato médico, quanto à necessidade do módulo de saúde mental. O surgimento de novas escolas médicas, bem como a inexistência de internato de saúde mental em escolas mais antigas, pressupõe a reflexão e viabilização desse novo campo de atuação. O artigo propõe apresentar a experiência do curso de Medicina da Universidade São Francisco (USF), de Bragança Paulista, que desenvolve internato de saúde mental há 15 anos. Método: Como premissa estrutural estabelecemos cinco norteadores de organização, planejamento e execução do internato, respeitando a especificidade e realidade de cada curso no que tange à carga horária, ao corpo docente e aos cenários de aprendizagem. Resultados: O objetivo é apresentar informações sobre o conteúdo programático de saúde mental na graduação, a estruturação do internato, a existência de internato próprio de saúde mental ou vinculado a um módulo mais abrangente, a carga horária existente, a distribuição do número de alunos por módulo, o número de docentes e os cenários de aprendizagem existentes. Conclusão: Os autores apresentam o modelo de funcionamento do internato de saúde mental do curso de Medicina da USF, que deve servir como proposta de viabilização e norteamento para os cursos de Medicina que necessitam de um referencial de formação. A educação médica que se encontra em importante momento de inflexão deve formar profissionais que, mesmo não sendo psiquiatras, sintam-se preparados para as demandas de saúde mental, que se apresentam no cotidiano da atenção à saúde, sob a estruturação dos princípios básicos do SUS. Objetivo: Relatar o caso de um paciente com episódio depressivo grave e sintomas psicóticos, que por dificuldade de acesso à ECT gerou a internação prolongada. Método: Trata-se de um paciente atendido no Serviço de Psiquiatria do Hospital Universitário São Francisco na Providência de Deus – HUSF, localizado na cidade de Bragança Paulista - SP. Relato do Caso: Paciente do sexo masculino, 57 anos, aposentado, com sintomas de isolamento social, anedonia, perda de peso e diminuição intensa da ingestão hídrica. Apresentava delírio misto, inapetência e alteração do sono, sendo encaminhado a um serviço especializado após piora com o tratamento inicial. Por conta de sua internação prolongada e baixa resposta ao tratamento (inicialmente com citalopram, levomepromazina e quetiapina), a equipe de psiquiatria solicitou transferência para um Serviço com ECT, porém, sem sucesso. Durante a internação, o regime terapêutico foi alterado para uma combinação de venlafaxina e quetiapina, sendo a última posteriormente substituída por risperidona. Houve melhora gradual do quadro, com ganho de peso e alta após 53 dias. No acompanhamento pós-alta, foi mantida a estabilidade do quadro e o uso regular das medicações, com retorno completo da funcionalidade. Conclusão: Caso esteja disponível, a ECT pode ser uma ótima ferramenta para tratamento de episódios depressivos graves, principalmente com sintomas psicóticos associados, especialmente em pacientes com resistência aos tratamentos farmacológicos. Seu uso costuma diminuir o tempo de internação e devolver a qualidade de vida aos pacientes. O caso apresentado neste trabalho é um exemplo pontual de como a ausência da ECT provocou um aumento no tempo de internação de um paciente com depressão psicótica, afetando negativamente sua rotina e reduzindo sua qualidade de vida, além de gerar elevação dos custos decorrentes da internação prolongada.
