Osteocalcin is no longer regarded as a molecule exclusive to bone remodeling and osteogenesis, but as a hormone with manifold functions. The discovery of the interaction of osteocalcin with the G protein-coupled receptor family C group 6-member A (GPRC6A) receptor has accompanied the characterization of several roles that this peptide serves in body regulation and homeostasis. These roles include the modulation of memory in the brain, fertility in the testis, fat accumulation in the liver, incretins release in the intestine and adaptation to exercise in muscle, in addition to the well-known effects on β-cell proliferation, insulin release and adiponectin secretion. The aim of the present review was to provide a practical update of the multi-organ effects that osteocalcin exerts through its interaction with GPRC6A and the clinical implications of this.

El síndrome metabólico (SM), es un problema de  salud pública que afecta cada vez más a la población de jóvenes. La Encuesta Nacional de Salud y Nutrición 2006 en población mexicana documentó sobrepeso y obesidad en 70%, diabetes mellitus en 7% e hipertensión arterial en 30.8%.1 En estudiantes universitarios se ha encontrado una prevalencia del 3.7%.2 El objetivo de este estudio fue documentar la prevalencia del SM en  estudiantes de medicina de la  Universidad Autónoma de Aguascalientes  (UAA). Metodología: Estudio descriptivo, no experimental, transversal.  Se efectuó en un grupo de estudiantes con edades entre los 17 a 26 años, de agosto a diciembre de 2011. Mediante un muestro de conveniencia, se estudiaron a 230 estudiantes, a los que se determinaron los criterio de SM de acuerdo con ATP III 3. Resultados: de los 230 estudiantes, 108 fueron mujeres y 128 hombres. La prevalencia del SM fue de 15.2% (35 estudiantes); 22 estudiantes (9.56%) tuvieron 3 criterios  y otros 13 alumnos, 4 criterios.  Los factores más frecuentes fueron: índice de masa corporal ≥ 25 (IMC) en el 80%,  lipoproteínas de alta densidad  disminuídas (HDL) en el 60%, hipertensión arterial sistémica, 51 %, hipertrigliceridemia 45 % y obesidad abdominal 45 %. Conclusión: El SM y los criterios de diagnóstico del mismo, pueden encontrase también en edades tempranas de la vida. El criterio más frecuente documentado fue el sobrepeso. Estos resultados nos  deben motivar a efectuar programas de prevención en nuestra universidad.

Introduction: There are just a few reports that deal with long-term outcomes of a partial amputation surgical repair. Long-term studies have reported similar rates of disability among patients with amputations and those that have been undergoing reconstructive surgery. Objective: The purpose of this report is describing a clinical case of a patient with partial traumatic amputation of an upper limb with an excellent functional recovery after 13 years of follow-up. Clinical case: The case of an 8 year old male patient with severe trauma to the upper left limb is described. The lesions included an oblique diaphyseal open fracture of the distal region of the humerus, along with detachment of the biceps and triceps muscles. The fracture was fixed transiently with 2.0 mm Kirschner's wire followed by immobilization with Sarmiento's brace, and finally, open reduction and internal fixation with a 3.5 mm dynamic compression plate were performed. The muscular and neurovascular integrity allowed microsurgical repair of the radial nerve and neuromuscular rehabilitation. Conclusion: This clinical report represents a case with an excellent functional recovery witnessed through a 13-year follow-up period.
