Introdução: A prática do jejum pré-operatório está estabelecida desde do século passado e, desde então, o conceito tradicional de jejum e privação de água tem sido questionado por anestesistas e cirurgiões após anos de prática. Diversos protocolos foram propostos por sociedades, porém essa realidade, no entanto, não condiz com a prática exercida em muitos centros de assistência à saúde. Objetivo: Realizar uma revisão da literatura e analisar os estudos que abreviaram o tempo de jejum pré-operatório publicados nos últimos 5 anos. Método: A seleção dos trabalhos foi realizada em fevereiro de 2023 na base de dados PUBMED. Foi utilizado como descritores: fasting AND preoperative AND shorten. O recorte temporal foi os últimos 5 anos. Resultados: A busca finalizou com 12 artigos que atenderam adequadamente aos critérios de inclusão. Conclusão: A abreviação do tempo de jejum pré-operatório de 2h para líquidos claros pode ser possível, segura e promove benefícios. Para o paciente, é capaz de melhorar o conforto, reduzir sede e a fome, melhorar a função gastrointestinal e diminuir a ansiedade pré-operatória. Além disso, diminui o uso de analgésicos, do volume de infusão intravenosa e estabiliza os níveis de glicemia, insulina e eletrólito.
Introduction: Neuroimaging assessment and neuromonitoring are important in the treatment of patients at risk of severe acute brain injury. However, the neurological examination may be unreliable, either due to sedation or the patient's baseline status. Therefore, a non-invasive and easily accessible tool stands out, the measurement of the Optic Nerve Sheath Diameter (ONSD), which is related to Intracranial Pressure (ICP). Objective: To review the literature, summarizing the evidence related to the measurement of ONSD in prostatectomies for the analysis of intraoperative ICP. Method: The paper's search was performed in Pubmed/Medline, Lilacs and Google Scholar databases, using the descriptors: intracranial pressure AND ultrasound AND optic nerve AND prostatectomy. Results: 30 articles were selected, 20 of which were excluded because they did not adequately discuss the proposed theme. Conclusion: ONSD is a very useful method for determining ICP increase, especially during Robot-Assisted Laparoscopic Radical Prostatectomy (RALRP), which may be associated with inadequate emergence of anesthesia. Dexmedetomidine, administered during the course of surgery, attenuates the increase in ICP during pneumoperitoneum in the Trendelenburg position. Finally, propofol reduces ICP changes in patients undergoing robotic surgery in comparison to sevoflurane.
Introdução: A aspiração perioperatória do conteúdo gástrico é uma complicação grave da anestesia e está associada à alta morbimortalidade, podendo resultar em pneumonia grave que requer suporte de ventilação mecânica em até um terço dos pacientes. Com o advento da ultrassonografia portátil, a realização de ultrassom no local de atendimento se tornou relativamente fácil e viável. Objetivo: Realizar uma revisão da literatura buscando avaliar os ensaios clínicos que avaliaram a determinação de volume e conteúdo gástrico por ultrassonografia à beira do leito (POCUS). Método: A seleção dos trabalhos foi realizada em agosto de 2021 na base de dados PUBMED. Duas estratégias de busca foram utilizadas: 1 - Ultrasound AND gastric AND fasting and anesthesia; e 2 - bedside AND ultrasound AND gastric. Não foi realizado corte temporal. Todavia, somente estudos clínicos foram considerados na presente revisão. Resultados: A busca finalizou com 10 artigos que atenderam adequadamente aos critérios de inclusão. Conclusão: A POCUS é uma técnica não invasiva muito útil na determinação do conteúdo e volume gástrico com alta sensibilidade e especificidade, mesmo que o conteúdo gástrico seja incerto. Além disso, a aplicação da POCUS na determinação do esvaziamento gástrico apresentaria boa sensibilidade e especificidade na discriminação de pacientes com e sem íleo.
