Human papillomavirus (HPV), one of the most common sexually transmitted diseases worldwide, has an established role in the pathogenesis of genital malignancies such as cervical cancer. The virus has also been implicated in the oncogenesis of nongenital cancers including head and neck malignancies (specifically oropharyngeal cancers) as well as anal cancer. There is less clarity regarding its role in lung and esophageal cancers. Worldwide, the incidence and prevalence of HPV-associated oropharyngeal cancer has been increasing over time. These patients have improved outcomes compared with those with HPV-negative oropharyngeal cancers, and there is continued interest in designing treatments specifically for this HPV-positive subgroup. Clinicians continue to gain an understanding of HPV in anal cancers and the risk factors associated with infection and progression to malignancy. This has potential implications for the eventual screening of high-risk groups. While HPV vaccination is currently approved for the prevention of cervical cancer, it also has potential in the prevention of all HPV-associated malignancies. In this review, current understanding of the role of HPV in nongenital cancers is discussed, as well as future implications for treatment and prevention.
The authors studied the psychological variables of 25 men who participated in the winter-over team of an Indian expedition to Antarctica for possible associations with seasonality and isolation. It was found that increased cigarette smoking was associated with the stress of the beginning of isolation (March), sleep difficulty was associated with midwinter (June), rapport was at a minimum at the point of maximum isolation in temporal terms (September), and decreased satisfaction with work and life situations was associated with continued isolation (December, January). The study suggested that variables postulated to affect performance undergo changes during the course of wintering over in Antarctica.
The very young and the very old are vulnerable to choking, and there are over 4000 choking-related deaths annually in the United States. Complications from the Heimlich maneuver (HM), as reported in infrequent case reports, predominantly relate to the elderly. It is doubtful that the denominator, numerator, appropriateness, expertise, and problems of maneuvers applied to choking victims will ever be known. This case report and literature review suggests that the treatment for acute elderly choking victims should be applied carefully and that esophageal food impaction, which should be rapidly distinguishable from choking, can have serious complications with application of the HM.
Hereditary spherocytosis is a common form of hemolytic anemia sometimes requiring splenectomy in recalcitrant cases. The complications of splenectomy include an increased risk of thrombosis, usually presenting with deep vein thrombosis or pulmonary embolism. However, common complaints such as headache should warn clinicians of involvement in less common sites, primarily the dural venous system. The case of dural venous thrombosis in a patient with hereditary spherocytosis and splenectomy is discussed. The potential mechanisms associated with thrombosis formation in this population and its treatment are discussed.
