We report the case of a 30-year-old female being studied for a colonic tumor, on the computed tomography-scan an abdominal mass with wall reinforcement and air bubbles suggestive of a retained sponge, Barium enema showed an extraluminal radiopaque marker and a mass migrated into the colon, on surgery, an enterocolic fistula was found, and we resected the ascending colon and 40 cm of terminal ileum and performed a ileotranverse anastomosis. Gossypibomas, despite preventive measures, remain a concern for surgeons due to its medicolegal implications, a high clinical suspicion leads to a correct diagnostic protocol and an early treatment.
