Objective: to present a protocol for the Consultation of Nursing for the Alzheimer carriers in a Center of Reference of the City of Recife (PE). Method: descriptive, exploratory study, bibliographical survey. Guided this study the Zagobel guidelines: problems collecting, systematized comment, situation diagnostic, care plan and register. The population was constituted five Alzheimer patients carriers. A questionnaire for the data collect was used. Results: the data had been analyzed descriptive that resulted in the protocol elaboration for the Alzheimer carriers, in the Nursing plan interventions as well as in the Nursing diagnosis identification: 1) Process of Modified Thought; 2) Familiar Risk for Modified Processes; 3) Risk for Consuming of the Person who Care; 4) Deficit in the Selfcare. Conclusion: the benefits not only extend to the patient and its family, as well as to the Nursing, since, with this practical, each time more the care based on scientific knowledge are devoted, allowing the nurse systemize the nursing assistance and describe plan care action for the Alzheimer carriers, based on Nursing diagnosis. Método: estudo descritivo e exploratório, norteados pelas diretrizes de ação de Zagobel: levantamento de problemas, observação sistematizada, diagnóstico da situação, plano de cuidados e registro. A população amostral se constituiu de cinco pacientes. Foi elaborado um questionário para a coleta de dados. Resultados: os dados foram analisados descritivamente que resultou na elaboração do protocolo aos portadores de Alzheimer, do plano de intervenções de Enfermagem bem como na identificação dos diagnósticos de Enfermagem: 1) Processo de Pensamento Alterado; 2) Risco para Processos Familiar Alterados; 3) Risco para Desgaste da Pessoa que Presta Cuidado; 4) Déficit no Autocuidado. Conclusão: os benefícios se estenderam não apenas ao paciente e sua família, como também à Enfermagem, visto que, com essa prática, cada vez mais se consagram os cuidados pautados no rigor científico, permitindo-se sistematizar a assistência de Enfermagem e descrever ações por meio de um plano assistencial aos portadores de Alzheimer, respaldado nos Diagnósticos de Enfermagem identificados. Método: estudio descriptivo, guiados por las directivas de acción Zagobel: estudio de los problemas, la observación sistemática, el análisis de la situación, plan de atención y registro. La población fue de cinco pacientes con Alzheimer. Un cuestionario fue desarrollado para la recogida de datos. Resultados: los datos fueron analizados descriptivamente, que desembocó en la redacción del protocolo a las personas con enfermedad de Alzheimer, el plan de intervenciones de enfermería y la identificación de los diagnósticos de enfermería: 1) Proceso modificado Del pensamiento, 2) Riesgo de alteración de Procesos de Familia, 3) riesgo de usar la persona que proporciona la atención, 4) Déficit en el autocuidado. Conclusión: los beneficios se extienden no sólo al paciente y su familia, sino también a la enfermería, ya que con esta práctica, se dedican cada vez más la atención basada en el rigor científico, permite sistematizar la asistencia de enfermería y para describir las acciones de medio de un plan para atender a los pacientes de Alzheimer, con el apoyo en el diagnóstico de enfermería identificados.
