Background and purpose:The number of National Health Insurance (NHI) participants switching their primary healthcare (PHC) providers is increasing and dominated by self-paid participants. This switching could result in unequal distribution of NHI participants and amount of capitation among PHC providers. This study aims to explore the reasons underlying self-paid NHI participants to switch PHC providers. Methods: This is a qualitative study using in-depth interviews with a total of 14 informants recruited purposively, consisted of ten NHI selfpaid participants who had switched and four participants who intend to switch PHC providers. Informants were recruited by approaching NHI participants at BPJS counters and inviting participants through social media. Semi-structured interview guide incorporated the aspects of Consumer Switching Behavior Model. The interviews explored four aspects of participants' intention to switch providers including inconvenience, service encounter failures, core health service failures, and attraction by competitor. The verbatim transcripts were analyzed thematically. Results: Self-paid NHI participants in this study cited four main reasons for switching PHC providers. The first reason is inconvenience in accessing PHC provider's services due to long distance, long waiting times, unsuitable opening hours, and poor facilities. Secondly, patients cited the poor interaction between healthcare providers and patients specifically, lack of attentiveness, poor eye contact, poor delivery of health information, lack of consideration of patients' opinions, and lack of responsiveness to patients' complaints. The third reason is core healthcare service failures, including hasty examinations, inaccurate diagnoses, and errors in prescribing medication. Finally, informants reported that they wished to switch PHC providers due to the attractiveness of other providers, in terms of facilities availability, professionalism of healthcare providers, additional health programs, and ease of access to referral healthcare facilities. Conclusion: Self-paid NHI participants' intention to switch PHC providers was mainly attributed to the perceived poor healthcare quality of the provider and superiority of the others. Continuous quality improvement should be undertaken by PHC providers to increase participants' loyalty.
Masalah gizi pada remaja harus mendapat perhatian khusus karena akan mempengaruhi pertumbuhan dan perkembangan tubuhnya serta dapat berdampak pada masalah gizi saat dewasa. Pengetahuan tentang gizi seimbang pada remaja merupakan kunci penting dalam memilih makanan yang bergizi dan aman sehingga dapat menunjang kesehatannya. Tujuan penelitian ini adalah untuk mengetahui pengaruh pengetahuan gizi seimbang terhadap perilaku memilih jajanan pada remaja. Penelitian ini menggunakan metode deskriptif kuantitatif dengan desain cross sectional. Penelitian ini akan dilaksanakan di SMK Kesehatan Bali Khresna Medika pada bulan September – Desember 2021. Sampel penelitian adalah remaja usia 15-18 tahun sebanyak 114 orang yang ditentukan berdasarkan purposive sampling. Pengumpulan data dilakukan dengan menggunakan angket pengetahuan gizi seimbang dan perilaku pemilihan makanan jajanan pada remaja. Data dianalisis secara statistik menggunakan uji chi-square dengan SPSS. Hasil penelitian menunjukkan bahwa sebagian besar subjek penelitian memiliki pengetahuan gizi seimbang dalam kategori baik (58,8%), pengetahuan pemilihan jajanan dalam kategori kurang (50,9%), sikap dalam pemilihan jajanan dalam kategori kurang (60,5%), tindakan/praktik dalam pemilihan jajanan dalam kategori baik (64%), dan perilaku dalam pemilihan jajanan secara keseluruhan dalam kategori baik (53,5%). Berdasarkan uji statistik diketahui bahwa tidak ada hubungan antara pengetahuan gizi seimbang dengan perilaku memilih jajanan pada remaja p > 0,05.
The still manual management of medical records at Puskesmas I Melaya resulted in medical records not being found, caused by the medical record storage racks being in three different places and not being kept safe. This causes the medical record search time to exceed the standard procedure. With the above problems, a web-based mobile view-based electronic medical record was designed to facilitate the search for medical records and their management in a computerized manner. The development of a web-based mobile view electronic medical record system uses the System Development Life Cycle method. Sample determination was determined by probability sampling technique with simple random sampling with 19 samples. Testing this information system uses black box testing. Tested its usability using the USE Questionnaire with 30 questions. Usability testing with the USE Questionnaire proved 81.40% results with an "excellent" classification, which was shown to speed up the patient's medical record search time from 12 minutes to 0.47 minutes. Dengan ini menyebabkan waktu pencarian rekam medis melebihi standar prosedur. Dengan masalah diatas dibuat rancang bangun rekam medis elektronik berbasis web mobile view untuk mempermudah pencarian rekam medis dan pengelolaannya secara terkomputerisasi. Pengembangan sistem rekam medis elektronik berbasis web mobile view ini menggunakan metode System Development Life Cycle. Determinasi sampel di tentukan dengan teknik probability sampling dengan simple random sampling dengan 19 sampel. Pengujian sistem informasi ini menggunakan black box testing. Diuji usability-nya menggunakan USE Questionnaire dengan 30 pertanyaan. Pengujian usability dengan USE Questionnaire membuktikan hasil 81,40% dengan klasifikasi “sangat baik”, yang terbukti mempercepat waktu pencarian rekam medis pasien dari 12 menit menjadi 0,47 menit. Berdasarkan data tahun 2020, rata-rata kunjungan pengguna asuransi Badan Penyelenggara Jaminan Sosial (BPJS) Kesehatan di Rumah Sakit Surya Husadha Denpasar berkisar 6000 kunjungan per bulannya sehingga sering terjadi penumpukan antrian di loket pendaftaran pasien. Hal ini secara tidak langsung menambah beban kerja petugas loket yang harus menginput data pasien satu per satu secara manual. Tidak jarang terjadi kelelahan sehingga menurunkan kinerja serta menambah tingkat kesalahan petugas. Tujuan penelitian ini untuk mengidentifikasi pengaruh sistem antrian berbasis QR code terhadap beban kerja petugas loket pendaftaran di rumah sakit. Penelitian ini merupakan penelitian kuantitatif dengan pendekatan quasi eksperimen one group prettes-posttes. Pengambilan sampel dilakukan pada 20 petugas loket pendaftaran BPJS dengan menggunakan teknik sampling jenuh. Pengembangan sistem QR code menggunakan System Development Life Cycle (SDLC). Pengumpulan data dilakukan dengan menyebarkan kuesioner sebelum dan setelah petugas diberikan intervensi QR Code. Analisis yang digunakan yaitu uji Paired T-tes untuk melihat pengaruh QR Code terhadap beban kerja petugas sebelum dan setelah diberikan QR Code. Hasil penelitian menunjukkan hasil pre test dan post test beban kerja diperoleh nilai signifikansi (p-value) sebesar 0,002 yang bermakna bahwa terdapat perbedaan yang signifikan antara rata-rata beban kerja pre-test dan post-test setelah penggunaan sistem atrian berbasis QR Code
