Ligas Acadêmicas (LAs) são projetos universitários de estudantes com um interesse em comum, que devem ser pautados no tripé universitário de ensino, pesquisa e extensão e que surgem da percepção da necessidade de complementação da formação acadêmica e integração da universidade com a comunidade. O pilar da extensão, consolidado no século 19, passou por diversas modificações conceituais e expressa um dos objetivos primordiais de uma LA: construir uma relação entre o universo acadêmico e a comunidade na qual se insere. Mais do que isso, aproximar-se, conhecer e transformar, dentro de seu escopo, a realidade social muitas vezes negligenciada pelos currículos. Realizar ações extensionistas, no entanto, é um constante desafio às LAs, pela burocracia envolvida e o pouco tempo disponível para atividades paralelas à matriz curricular. Este trabalho se propõe a relatar e discutir a experiência da Liga Acadêmica de Medicina de Família e Comunidade da UFPR – Campus de Toledo (LAMF), em sua tentativa de realizar extensão em um contexto e estrutura que parecem desvalorizar essas iniciativas. A LAMF promoveu extensão por meio de fóruns em simpósio acadêmico, publicações em linguagem simples nas redes sociais, palestras, encontros e formação de grupos de trabalho que debateram questões negligenciadas e da realização de atividades práticas. É possível vencer os obstáculos que impedem a realização de extensão popular e aproximar o meio acadêmico da comunidade, de modo que o ensino na universidade possibilite, além da formação técnica, a construção de profissionais críticos e capazes de modificar a realidade social muitas vezes ignorada pela academia.
