In Brazil, abortion is only allowed in cases of rape, serious risk to a woman's life or fetal anecephaly. Legal abortion services cover less than 4% of the Brazilian territory and only 1,800 procedures are performed, in average, per year. During the COVID-19 pandemic, almost half of the already few Brazilian abortion clinics shut down and women had to travel even longer distances, reaching abortion services at later gestational ages. In this paper, we describe three bottom-up advocacy strategies that emerged from difficulties deepened during the COVID-19 pandemic at a single abortion service in Brazil, amidst anti-gender policies from the federal government. Telemedicine abortion, outpatient surgical abortion and the provision of abortion after 20 weeks' gestation are important strategies that may reduce inequalities that impact the most vulnerable populations, such as black and indigenous women, children, adolescents and women experiencing domestic violence.legal abortion, outpatient surgical abortion, second and third trimester abortion, South America, telemedicine | BACKGROUNDIn Brazil, access to legal and safe abortion has been difficult even before the COVID-19 pandemic. Brazil has one of the most restrictive laws on induced abortion in the world. According to the Brazilian Penal Code, abortion is only allowed in cases of rape or serious risk to a woman's life. In 2012, fetal anencephaly was included as a third legal ground to abortion. 1 Although the Brazilian Penal Code is dated from 1940, it was only in 1999 that the Ministry of Health published its first regulations in an attempt to increase the number of legal abortion services throughout the country. 2 Despite such effort, there is still a scarcity of clinics providing access to lawful abortion. In 2019, there were 251 services with at least one abortion performed, covering less than 4% of the Brazilian territory. 3 In these few services, a regimen of suspicion of the sexual violence survivors' narratives is common, and health professionals tend to question the victim's subjectivity in order to decide whether she legitimally holds the right to an abortion. A criação de ofertas de cuidados de saúde substitutivos à necessidade de internação, com redução da utilização de leitos para a prestação desses cuidados, tem evidenciado o novopapel que o hospital passa a desempenhar no sistema de saúde e implica em mudanças em seu perfil assistencial, de forma a facilitar e produzir maior articulação com a rede de serviços de saúde nos diversos níveis de atenção. O objetivo do presente artigo é discutir a situação atual e as tendências da atenção hospitalar do Sistema Único de Saúde (SUS) na Bahia, com base na análise da quantidade dos leitos ofertados ao SUS, porte e natureza dos estabelecimentos, frequência de internações, média de permanência e taxa de ocupação apuradas nos sistemas de informação oficiais do SUS. Seu objetivo específico é compreender o quanto as característicasatuais e modificações da oferta de serviços hospitalares no período estão compatíveis com as tendências da atenção hospitalar apresentada na literatura de referência. Trata-se de uma análise exploratória a partir de bases de dados oficiais do SUS. Os resultados foram comparados com a literatura disponível, buscando identificar-se sua adequação com tendências de desospitalização em outros países e com diretrizes do Ministério da Saúde para a atenção hospitalar no Brasil. Na Bahia, a retração de leitos SUS no setor privado é coerente com o restante do país. Até 2009, embora tenha havido crescimento de leitos em hospitais públicos de maior porte e resolutividade (34,2%), persistiu uma grande concentração de leitos naqueles de pequeno porte(33,9%), superando em quase duas vezes o observado no restante do Brasil. A frequência de internações, médias de permanência e taxas de ocupação mostram ociosidade nos pequenos hospitais. Conclui-se que há uma tendência de ampliação de unidades públicas de maior porte, compatível com experiências nacionais e mundiais de maior investimento em unidades com maior adensamento tecnológico, sem evidências de reorientação do perfil dos hospitais de pequeno porte.
O objetivo deste trabalho é apresentar a trajetória de constituição de um grupo feminista no campo da saúde coletiva e colocar em análise sua produção a partir da metodologia de relato de experiência em uma perspectiva histórico-narrativa. O coletivo “Adelaides: feminismos e saúde” é um grupo de mulheres profissionais, pesquisadoras e ativistas que se identificam com a Saúde Coletiva e se organizam afetiva e politicamente para intervir nos espaços que ocupam, especialmente no que se refere às pautas de interface entre saúde e feminismos. O grupo surge em 2018, ano de comemoração dos 30 anos do Sistema Único de Saúde (SUS). A divulgação de uma mesa de debate sobre a Saúde Pública composta exclusivamente por notáveis homens brancos trouxe à tona a crítica feminista à invisibilidade da mulher na Saúde por parte do Coletivo. Aproveitando-se do momento do 12º Congresso da Associação Brasileira de Saúde Coletiva (ABRASCO), o grupo organizou duas intervenções iniciais no campo científico da Saúde: uma mesa de debate composta de mulheres notáveis da Saúde Coletiva, representando diferentes gerações e recuperando a trajetória do campo sob a perspectiva dessas mulheres; e lançou a pesquisa "Mulheres da Saúde Coletiva: um retrato de quem constrói o campo" que, através de questionário on-line, já alcançou mais de mil mulheres-participantes. Essas ações contribuíram para o início de uma agenda científica, política e institucional para o grupo, um constituir-se que se deu no próprio fazer coletivo, resultado do modo de atuação sócio-político-afetiva adotado pelas Adelaides para seus modos de existir e produzir ciência.
