MCS (mechanical circulatory support) has shown great efficacy in treatment of respiratory and cardiovascular diseases when conventional therapy is insufficient. The use of ECMO (extracorporeal membrane oxygenation) for the treatment of cardiogenic shock secondary to sepsis is still controversial. Studies have shown that ECMO should be considered as a valuable therapeutic option in cases of refractory cardiovascular dysfunction in severe septic patients, although more studies are needed to consolidate its efficacy. We report the case of two patients, without previous history of cardiopathy, subjected to temporary MCS, with use of ECMO for the treatment of refractory cardiogenic shock secondary to sepsis.
Esse estudo tem como objetivo avaliar a melhora cognitiva e de parâmetros séricos em dependentes químicos submetidos a uma suplementação de ômegas 3, 6 e 9 em internação de 90 dias em centros de reabilitação para drogadição. A população do estudo é de pacientes entre 18 e 40 anos com diagnóstico presuntivo de abuso ou dependência de cocaína/crack e que deram entrada voluntariamente em comunidades terapêuticas destinadas ao tratamento de dependência química com previsão mínima de 90 dias de tratamento. A partir da avaliação de exames séricos de triglicerídios (TGs) e da proteína-C reativa (PCR) e da evolução em testes neuropsicológicos Trilhas A e B (TMT) e dígitos (Digit Span- DS), dividimos e comparamos dois grupos dos pacientes recém-internados: um grupo de intervenção (n = 7), ao qual um leite rico em ômegas foi ingerido pelos 90 dias, e um grupo controle (n = 8), que ingeriu o mesmo leite sem essa suplementação, durante o mesmo tempo. A comparação dos testes psicológicos evidenciou, em média, melhora maior do grupo da intervenção em relação ao controle nos testes de Trilhas A e B, com redução significativa do tempo de realização, indicando melhora na velocidade de atenção concentrada e alternada e na velocidade de processamento cognitivo. No teste de dígitos, não houve melhora significativa em nenhum dos grupos. Quanto aos exames séricos, a PCR elevou-se discretamente no grupo de intervenção, com redução significativa no grupo controle. Houve pequeno aumento também dos TGs no grupo da intervenção, enquanto o grupo placebo apresentou pequena redução desse lipídio. Os resultados sugerem que a suplementação dos ômegas 3, 6 e 9 pode ser uma alternativa segura e viável para o tratamento de usuários de cocaína/crack após interrupção do uso da droga para melhora cognitiva e redução de indicadores inflamatórios, como a proteína-C reativa. A suplementação pode estar associada ao aumento de triglicerídeos nesses pacientes
O carcinoma hepatocelular (CHC) é a malignidade hepática primária mais comum e constitui um problema de saúde pública a nível mundial, sendo uma causa notável de morbidade e mortalidade. Entre os fatores de risco que predispõem o desenvolvimento dessa doença, a infecção prévia pelos vírus hepatite B e hepatite C desempenha importante papel, sendo este último mais propenso a evoluir a uma forma primária neoplásica (CHC). A cirrose e o uso abusivo de álcool também são prevalentes entre os que desenvolvem essa neoplasia. Atualmente, o diagnóstico do CHC é geralmente tardio, realizado pela biópsia e por exames de imagem, de acordo com os Critérios de Barcelona, de forma que os métodos de imagem e os marcadores séricos não têm sensibilidade suficiente para a detecção do CHC em estágios iniciais. Nesse trabalho, buscou-se caracterizar o perfil de lipídeos em plasma de pacientes com CHC por espectrometria de massas, correlacionando-os com dados clínicos dos pacientes para o estabelecimento de potenciais biomarcadores moleculares. Como resultado, demonstramos que pacientes com CHC apresentam perfil lipídico em plasma diferente do expresso em indivíduos não patológicos, sendo reflexo das alterações metabólicas tumorais típicas. A identificação de íons lipídicos de maior relevância revelou alteração principalmente da classe dos glicerofosfolipídios, sendo a fosfatidilcolina (PC) o lipídio mais abundante no grupo controle, e a fosfatidiletanolamina (PE) caracteristicamente equilibrada com a PC nos grupo com CHC. Verificou-se, assim, importante mudança da razão entre PC/PE entre os grupos patológico e não patológico. Esse estudo apresenta resultados que possibilitam a descoberta de possíveis biomarcadores séricos para o CHC, sugerindo a possibilidade de exames menos invasivos e de valor diagnóstico mais sensível e específico para essa neoplasia
