(1) Background: The outbreak of the COVID-19 pandemic represented a real challenge for all of humanity. Characterized by a complex spectrum of signs and symptoms, by various severity degrees, the disease spread rapidly around the world. After more than two and half years since the beginning of COVID-19 pandemic, in the context of a paradoxical, enigmatic, and relentless COVID-19, the objective of the current study was to evaluate the characteristics and evolution of patients with SARS-CoV-2 infection, hospitalized in “St. Parascheva” Clinical Hospital of Infectious Diseases (Iasi, Romania). (2) Methods: This is a retrospective study that used the medical database recorded between July and November 2021 in order to highlight the characteristics of SARS-CoV-2 infection in patients from the northeastern region of Romania. (3) Results: We enrolled in the study a total of 1732 SARS-CoV-2 infected patients, mean age 67 ± 3.4 years, the female gender predominating (987 cases; 56.98%) as well as patients from the urban environment (982 patients; 56.69%). Moderate form of the disease predominated (814 cases; 47%), pulmonary imaging changes were found in 1042 (60.16%) cases, and 1242 (71.71%) patients had at least one underlying disease. After a median length of hospitalization of 9.5 days, 1359 (78.46%) patients were discharged cured, 48 (2.77%) were transferred to other services by decompensating the associated pathologies, 302 (17.43%) patients needed extensive support in the intensive care unit and there were 325 (18.76%) deaths. (4) Conclusions: The epidemiological characteristics of SARS-CoV-2 infection recorded in our study were mostly the same as characteristics of COVID-19 from all over the world.
(1) Background: Recurrent pregnancy loss (RPL) and recurrent implantation failure (RIF) have in common a deficient maternal adaptation to the semi-allogeneic fetus, in which killer immunoglobulin-like receptor (KIR) family expressed by natural killer (NK) cells play an important role. The aim of this study was to evaluate the influence of maternal KIR haplotype on the reproductive outcomes after single embryo transfer in IVF cycles in patients with RPL and RIF. (2) Methods: Patients with RIF and RPL who presented at Origyn Fertility Center from Iasi, Romania, were prospectively enrolled between January 2020 and December 2022. Clinical and paraclinical data was examined. Descriptive statistics and a conditional logistic regression model were used to analyze our data. (3) Results: Patients with a KIR AA haplotype had significantly more chances of miscarriage if they underwent an IVF procedure (aOR: 4.15, 95% CI: 1.39–6.50, p = 0.032) compared with those who spontaneously achieved a pregnancy. Moreover, it appeared that the same haplotype increased the chances of obtaining a pregnancy for patients who underwent an IVF procedure (aOR: 2.57, 95% CI: 0.85–6.75, p = 0.023). (4) Conclusions: Determination of KIR haplotype could be beneficial for patients with RPL or RIF in order to offer an individualized management.
(1) Introduction: While the primary impairment in COVID-19 is pulmonary, the ubiquitous distribution of angiotensin-converting enzyme 2 (ACE2) demonstrates the possible presence of systemic disease with involvement of the heart, kidneys, liver and other organs. (2) Methods: We retrospectively studied the observation sheets of patients diagnosed with SARS-CoV-2 infection hospitalized in the “Sf. Parascheva” Clinical Hospital of Infectious Diseases from Iasi for a period of 3 months. The aim of the study was to identify the frequency of liver injury due to SARS-CoV-2 infection among patients and its impact on the course of the disease. (3) Results: Out of the total number of hospitalized cases (1552), 207 (13.34%) were the subjects of our analysis. The severe form of SARS-CoV-2 infection predominated (108 cases; 52.17%) and in terms of liver damage, in all cases increased transaminase levels predominated and were determined to be secondary to the viral infection. We divided the lot into two groups, A (23 cases; 23.19%) and B (159 cases; 76.81%), depending on the time of onset of liver dysfunction, either at the time of hospitalization or during hospitalization. The evolution of liver dysfunction was predominant in most cases, with an average time of onset at 12.4 days of hospitalization. Death occurred in 50 cases. (4) Conclusions: This study revealed that high AST and ALT at hospital admission was associated with a high mortality risk in COVID-19 patients. Therefore, abnormal liver test results can be a significant prognostic indicator of outcomes in COVID-19 patients.
Conclusions:We think that the internal compression therapy procedure is a new, fast, minimally invasive, and efficient method for treatment of patients with chronic venous insufficiency due to incompetence of the terminal valve of the saphenofemoral junction and can be performed under outpatient conditions. This is a preliminary study. Further studies with a larger population of patients and longer follow-up are warranted.Objective: Flying commercially is far safer than undergoing surgery. Nevertheless, pilots and vascular surgeons face similar types of technical and environmental challenges. Differences in outcomes may be attributed to the use of human-factors science in the aviation industry. The study aimed to identify methods of improving patients' safety in vascular surgery based on the expertise of senior airline pilots.Methods: A qualitative study was undertaken with 17 airline training captains from 4 major UK and U.S. airlines. Semistructured interviews asked, What safety advice would you give a surgeon about to operate on you? Participants had a total of 188,800 flight-hours in 21 different aircraft. Thematic analysis of data was undertaken using the framework method and reported according to the Consolidated Criteria for Reporting Qualitative Research guidelines.Results: Five themes emerged to inform threat and error management in vascular surgery: 1. Error is inevitable. Consequently, error anticipation allows harm to be avoided, trapped, and mitigated. 2. Human performance is decayed by stress, fatigue, and cognitive overload. Optimization of the operator's condition is essential for the patient's safety.3. Situational awareness in lost in complex environments. Cognitive tools and heuristics can facilitate rational decision-making at times of high workload. 4. The "startle response" ("fight, flight, or freeze") is prevalent in crisis situations. Surgical crisis management may require dual expert involvement (to mirror pilot flying vs pilot monitoring task distribution). 5. "Setting the tone" for a culture of confident humility is essential in building trust and collaboration in high-performing teams.Conclusions: High-reliability organizations optimize operator and systems performance. Human-factors tools used in aviation may be judiciously applied in vascular surgery to enhance safety and efficiency.Objective: Surgeon-specific mortality data (SSMD) are used to enhance transparency, to drive quality improvement, and to facilitate patients' choice. The use of SSMD is controversial, but patients' views on such data are largely unknown. The aim of this study was therefore to establish the views of patients and to identify their priorities for outcome reporting.Methods: A prospective questionnaire study of 165 patients receiving care in a single academic vascular unit was performed.
