INTRODUÇÃO: A dor lombar, considerada importante problema de saúde pública, pode ser tratada com cinesioterapia aliada à "Escola de Postura"; entretanto, ainda não está definido o papel desta associação como alternativa de tratamento para lombalgias crônicas. OBJETIVO: Avaliar a efetividade de um programa cinesioterapêutico de grupo aliado à Escola de Postura, sobre a dor e a capacidade funcional, em indivíduos com lombalgia crônica. MATERIAIS E MÉTODOS: Trata-se de um estudo clínico transversal, realizado com 24 indivíduos que apresentavam dor lombar, de ambos os gêneros, com média de idade de 55,3 ± 6,3 anos, pertencentes à área de abrangência da Unidade de Saúde Bairro Novo (Curitiba, PR). Foram realizados 12 atendimentos de um programa de cinesioterapia e educação postural, com frequência de uma vez por semana, durante três meses. Foram avaliadas dor (Escala Visual Analógica - EVA) e capacidade funcional (questionário de Rolland Morris) antes e após o período de intervenção. RESULTADOS: Na EVA observou-se diminuição significativa da dor (p < 0,05) do primeiro dia de intervenção (atendimento de 1h30min) para a EVA inicial de pré e pós-intervenção (programa de três meses). Também houve melhora na capacidade funcional no período de três meses de intervenção (p < 0,05). CONCLUSÃO: O programa de cinesioterapia em grupo mostrou-se favorável na melhora da dor e da capacidade funcional de pacientes com lombalgia.
INTRODUÇÃO: Após a descoberta da existência de campos elétricos em lesões de tecidos, começou-se a empregar a aplicação externa de correntes elétricas na cicatrização de feridas. Baseados nesses conceitos, estudos mostram que a estimulação elétrica por alta frequência (HF - high frequency) é concebida como uma opção útil no processo cicatricial. Apesar dessa afirmativa, não há consenso entre os pesquisadores para sua utilização no tratamento de afecções da pele, sendo utilizada empiricamente no tratamento de feridas crônicas. OBJETIVO: Investigar o uso do gerador de alta frequência sobre a dor, cicatrização e área de superfície de úlceras de pressão (UP). MATERIAIS E MÉTODOS: Estudo piloto de ensaio clínico controlado realizado na Unidade de Tratamento Intensivo do Hospital de Clínicas do Paraná em oito indivíduos (49,6 ± 15,9 anos de idade), com diagnóstico clínico de UP grau II e III, classificados pela National Pressure Ulcer Advisory Panel (NPUAP). Indivíduos foram divididos em dois grupos: controle (n = 2) e alta frequência (n = 6), com aplicação diária durante dez dias. Avaliou-se dor (por meio da Escala Visual Analógica - EVA), processo de cicatrização da úlcera (por meio do Pressure Ulcer Scale for Healing - PUSH Tool e do Pressure Score Status Tool - PSST) e a área de superfície da úlcera (planimetria e ImageJ). RESULTADOS: Houve efeito significativo intragrupo (pré e pós-intervenção) na utilização do gerador de alta frequência no processo de cicatrização (PUSH Tool de 8,6 ± 5,8 para 5,1 ± 5,5, p = 0,04 e PSST de 31,3 ± 5,6 para 18 ± 6,5, p = 0,02) e área de superfície da úlcera (planimetria de 690,3 ± 622,6 para 470 ± 642,4, p = 0,02 e ImageJ de 786,5 ± 690,9 para 537,0 ± 719,3, p = 0,02). Também houve diferença significativa entre os grupos controle e grupo alta frequência em todas as variáveis do estudo (p = 0,04). CONCLUSÕES: A utilização do gerador de alta frequência causou melhora na cicatrização de pacientes com UP. Contudo, são necessários mais estudos com maior número de indivíduos para corroborar tais resultados. Nesse contexto, esta pesquisa agrega valor à procura de alternativas de tratamento para UP.
Objectives: To compare the immediate analgesic effects of 2 kHz or 4 kHz interferential current (IFC) with different amplitude-modulated frequencies (AMFs) (2 Hz or 100 Hz) on chronic low back pain (CLBP). Design: Three-arm double-blinded randomized controlled trial. Setting: Primary care. Participants: 175 patients (19 to 60 years of age, 105 female) with CLBP. Interventions: One session of IFC: Interferential group (GI): GI2 kHz/100 Hz, GI2 kHz/2 Hz, GI4 kHz/100 Hz, GI4 kHz/2 Hz, or placebo. Main Outcomes Measures: Pain intensity by numerical rating scale of pain (NRS), McGill Pain Questionnaire (MPQ), and algometry. Results: There was a significant difference in NRS scores (P < 0.05) in the GI2 kHz/2 Hz, GI4 kHz/2 Hz, and GI4 kHz/ 100 Hz groups compared with those of the placebo group (PG), and a significant difference in MPQ scores in the GI4 kHz/2 Hz and GI4 kHz/100 Hz groups compared with those of the PG. In algometry, only the GI4 kHz/100 Hz group showed a significant difference (by 2 points in the lumbar region) compared with the PG. Of the carrier frequencies, an IFC of 4 kHz showed more effective results, although no significant difference was noted between the AMFs. Conclusion: An IFC with a carrier frequency of 4 kHz and an AMF of 100 Hz provide immediate analgesic effects in individuals with CLBP. Clinical Trial Registration number: RBR-59YGRB. &
OBJETIVO: Verificar se o fortalecimento abdominal promove influência na musculatura do assoalho pélvico feminino. MATERIAIS E MÉTODOS: Caracterizou-se como um ensaio clínico, realizado com 21 nulíparas, com idade média de 21,7 anos, divididas por conveniência em dois grupos: A (n = 10) - com orientação de contração perineal simultânea à contração abdominal e B (n = 11) - sem orientação de contração perineal. As participantes foram submetidas à avaliação ginecológica e abdominal pré e pós-intervenção por meio de: avaliação funcional do assoalho pélvico (AFA), cones vaginais, perineometria, teste de flexão anterior de tronco, descida de membros inferiores, força de oblíquos e endurance abdominal. O protocolo de fortalecimento abdominal continha três exercícios distintos, associados ou não à contração perineal, com três séries de dez repetições e repouso de 60 segundos, três vezes por semana, em um período de seis semanas. Para a análise estatística foi aplicado o teste t de Student e ANOVA. RESULTADOS: A comparação entre os valores iniciais e finais demonstrou diferenças significativas entre os dois grupos na avaliação do períneo, somente no teste com cones (p = 0,00) e na perineometria (resistência/pressão) (p = 0,03), indicando piora das variáveis no grupo B. Já na avaliação abdominal, o grupo A apresentou resultado significativo em três testes (descida de membros inferiores: p = 0,03, endurance: p = 0,03 e endurance modificado: p = 0,00) comparando-se a somente um do grupo B (endurance: p = 0,01). CONCLUSÕES: Sugere-se que a associação de exercícios de fortalecimento abdominal com o perineal pode potencializar o ganho de força e a função desses grupos musculares, e a falta dessa associação pode prejudicar a funcionalidade do assoalho pélvico.
Introduction: studying the influence of surfing on the prevalence of injuries may contribute to prevention. Objective: to analyze the influence of time practicing sports and the occurrence of previous surgery on the profile and prevalence of injuries caused by surfing. Methods: Sixty-six Brazilian surfers (26.16 ± 0.73 years old) participated in this study. Anthropometric data, physical activity level, surfing practice time and the prevalence of injuries (type of injury, anatomical region affected, and mechanism of injury) were evaluated. To assess which of the studied variables exerted significant influence on the mean number of injuries, a Poisson log-linear model was adjusted through R software (p < 0.05). Results: most surfers were classified as eutrophic (73%), very active (60.6%), had an average practice time of 10.1 ± 1 years, and were not members of a surfing federation (74%). It was also observed that 90.9% of participants reported injuries caused by surfing and 44.9% affected the lower limbs. The majority of these injuries affected the integumentary system (46.6%). The main mechanism of injury was impact with the board or seabed (40.4%). Furthermore, it was found that surfing federation members presented an average of 58.4% more injuries than non-members (p = 0.007). Surfers who had undergone previous surgeries showed an average number of injuries that was 56.9% higher than other surfers (p = 0.012). In addition, it was found that for each extra year of surfing, the average number of injuries increased by 2.5% (p = 0.0118). Conclusion: the average number of injuries increased with increment in time practicing the sport, previous surgery and membership in a surfing federation.
BACKGROUND: Therapeutic high-frequency ultrasound, microcurrent, and a combination of the two have been used as potential interventions in the soft tissue healing process, but little is known about their effect on the immune system. OBJECTIVE: To evaluate the effects of therapeutic high frequency ultrasound, microcurrent, and the combined therapy of the two on the size of the wound area, peritoneal macrophage function, CD4+ and CD8+, T lymphocyte populations, and plasma concentration of interleukins (ILs). METHOD: Sixty-five Wistar rats were randomized into five groups, as follows: uninjured control (C, group 1), lesion and no treatment (L, group 2), lesion treated with ultrasound (LU, group 3), lesion treated with microcurrent (LM, group 4), and lesion treated with combined therapy (LUM, group 5). For groups 3, 4 and 5, treatment was initiated 24 hours after surgery under anesthesia and each group was allocated into three different subgroups (n=5) to allow for the use of the different therapy resources at on days 3, 7 and 14 Photoplanimetry was performed daily. After euthanasia, blood was collected for immune analysis. RESULTS: Ultrasound increased the phagocytic capacity and the production of nitric oxide by macrophages and induced the reduction of CD4+ cells, the CD4+/CD8+ ratio, and the plasma concentration of IL-1β. Microcurrent and combined therapy decreased the production of superoxide anion, nitric oxide, CD4+-positive cells, the CD4+/CD8+ ratio, and IL-1β concentration. CONCLUSIONS:Therapeutic high-frequency ultrasound, microcurrent, and combined therapy changed the activity of the innate and adaptive immune system during healing process but did not accelerate the closure of the wound.

This study aimed to evaluate the effect of microcurrent electrical stimulation on pain and area of venous ulcers. In a pilot study for a single-blind controlled clinical trial, carried out at an outpatient clinic during four weeks, 14 subjects with venous ulcers (mean age 62±9 years) were divided in two groups: microcurrent (n=8) and control group (n=6). Pain (by Visual Analogue Scale) and the ulcer area were measured by planimetry. There was a significant difference between the two groups with respect to pain (microcurrent group from 8.5 (6.5-9.75) to 3.5 (1-4.75) and control group from 7.5 (5.75-10) to 8.5 (5.5-10), p<0.01).Non-significant changes were found with respect to ulcer area (planimetry by graph paper, p=0.41 and by Image J ® , p=0.41). In conclusion, the application of microcurrent improves the pain of patients with venous ulcers (ClinicalTrials.gov: NCT01372020).
