Чернівці ОСОБЛИВОСТІ ТОПОГРАФІЇ СУДИННИХ КОМПОНЕНТІВ ПЕЧІНКОВО-ДВАНАДЦЯТИПАЛОКИШКОВОЇ ЗВʼЯЗКИ ПІЗНІХ ПЛОДІВ ЛЮДИНИРезюме. Незважаючи на значний досвід, накопичений хірургами-науковцями, частота ускладнень пі-сля холецистектомії залишається відносно високою і становить, за різними даними, до 1% випадків, що засвідчує про необхідність поглибленого дослідження особливостей топографії компонентів печі-нково-дванадцятипалокишкової звʼязки. У статті наведені результати дослідження 34 препаратів пло-дів людини 4-6 місяців внутрішньоутробного розвитку. Встановлено два типи топографії артеріальних судин та три варіанти топографії ворітної печінкової вени. Ключові слова: печінково-дванадцятипалокишкова звʼязка, ворітна печінкова вена, плід, людина.Сучасна медична наука накопичила значний ма-сив інформації щодо будови печінково-дванадця-типалокишкової звʼязки. У науковій літературі часто трапляються повідомлення щодо особливо-стей топографії її структурних компонентів. Це засвідчує про високу увагу науковців до даної проблеми, адже, не зважаючи на стрімкий розви-ток сучасних хірургічних технологій, досі трапля-ються поодинокі випадки інтраопераційних ускладнень при виконанні операцій з приводу ви-далення жовчного міхура.Перша лапароскопічна холецистектомія ви-конана понад 25 років тому і нині є методом ви-бору під час хірургічного лікування калькульоз-ного холециститу. Проте, незважаючи на значний досвід накопичений хірургами-науковцями, час-тота ускладнень залишається відносно високою і становить, за різними даними, до 1% випадків [1][2][3]. Часто причинами ускладнень та помилок хіру-ргів є атипова топографія, рідкісні варіанти анато-мії та аномалії розвитку компонентів печінково-дванадцятипалокишкової звʼязки. Серед уродже-них вад розвитку позапечінкових жовчних проток (частка яких серед усіх вад розвитку становить 6-8 %) найчастіше трапляються вади розвитку спі-льної жовчної протоки [4, 5].Досліджуючи літературні джерела ми ви-явили деякі суперечності щодо трактування поло-жень про топографію судин печінково-дванадця-типалокишкової звʼязки та характеру кровопоста-чання позапечінкових жовчних проток [6][7][8].З огляду на вищенаведене вважаємо за необ-хідне провести поглиблене та комплексне ви-вчення варіантів будови та перинатального розви-тку деяких компонентів печінково-дванадцятипа-локишкової звʼязки. Мета дослідження: зʼясувати особливості топографії кровоносних судин у складі печінково-дванадцятипалокишкової звʼязки. Визначити ва-ріанти галуження загальної печінкової артерії та особливості її синтопії з ворітною печінковою ве-ною.Матеріал та методи. Нами досліджено 34 препарати плодів 7-10 місяців, методами тон-кого препарування під контролем мікроскопа та методом інʼєкції порожнистих структур. The components of the hepatoduodenal ligament play an important role in the process of digestion. Calculi in the common bile duct are found in 10-15% of individuals suffering from chronic calculous cholecystitis, and obstruction of bile ducts is found in 59,1-67,4% of cases. The objective of the study was to investigate peculiarities of component morphogenesis of the hepatoduodenal ligament during the perinatal period of human ontogenesis, individual and age anatomical variability, spatial-temporal transformations, and anatomical-histological peculiarities of its structures. To achieve the stated purposes the following complex of methods was used: macroscopic – for visual detection of the state of the hepatoduodenal ligament components, vascular injection – to study peculiarities and variants of angioarchitectonics of the arterial components of the hepatoduodenal ligament, statistical – to determine peculiarities of morphological transformations of the components and adjacent structures of the hepatoduodenal ligament at different periods of prenatal and postnatal periods of human ontogenesis. According to the results of the study several accessory triangles of the hepatoduodenal ligament were found and described. The common bile duct, the common hepatic duct, cystic duct, left and right hepatic ducts, hepatic portal vein, common hepatic artery, right gastric artery, hepatic artery proper, right, left and accessory branches of the hepatic artery proper are permanent components of the ligament forming a number of combinations-crossings between themselves. Meanwhile, we have determined certain regularities of the formed pattern including minimum 5 permanent triangles to be distinguished: Calot’s triangle or "superior lateral triangle", "superior medial triangle", "inferior medial triangle", "inferior lateral triangle", "central space of the hepatoduodenal ligament".
During any surgery, in addition to pathology-related factors, it is necessary to evaluate the individual features of the anatomy of a particular area or complex of organs. The relevance of such data is confirmed by the results of numerous studies that show that half of patients are characterized by certain anatomical variants, including the location of the arteries and biliary tract. The purpose of the work is to establish the peculiarities of the morphometric parameters of the components of the hepatic-duodenal ligament. The study was based on 50 preparations of fetus corpses (4 to 10 months) and 10 newborns without external signs of anatomical abnormalities or congenital malformations. Adequate anatomical methods were used during the study: macropreparation, injection of blood vessels, making topographic anatomical sections, morphometry. Statistical analysis of the obtained data was performed using the licensed program RStudio. During the perinatal period, the length of the hepatic-duodenal ligament was found to increase from 5.224±0.572 mm in fetuses for 4 months to 32.12±1.77 mm in newborns. The chart of change of the average values of its length by months of prenatal development indicates uneven increase of organometric parameters. From 4 to 5 months observed a significant increase in the length of the hepatic-duodenal ligament, while from 5 to 7 months of development observed a period of relative slowdown of its length, and from 5 to 6 months of development even a relative decrease in length. The period from 7 to 10 months determines the period of accelerated growth of ligament. The indices of the width of the hepatic-duodenal ligament in the perinatal period increased from 3.292±0.227 mm in fetus of 4 months to 21.25±0.938 mm in newborns. The width increases are not uniform. The periods of accelerated development (4-5 months and 9 months – newborns) and the period of slow development (5-8 months) were observed. It was proved that there are periods of accelerated and slow development, in which during periods of intensive development, organometric indicators always differed significantly, were smaller than the previous ones and outweighed the following ones respectively (4-5 months and 7-9 months, р˂0.05). Regarding the periods of slow development, the organometric indicators in these periods did not differ significantly (р˃0.05). Therefore, analyzing the dynamics of changes in the morphometric parameters of the components of the hepatic-duodenal ligament, revealed periods of their accelerated and slow growth.
