Aim. To evaluate the effectiveness of regional prolonged irrigation with ropivacaine ozonated solution at large and giant postoperative ventral hernia prosthetic hernioplasty. Methods. 77 patients who underwent a planned surgery for large and giant postoperative ventral hernia from 2010 to 2012 were examined. All patients were distributed to two groups. The main group included 26 patients in whom a polyvinyl chloride catheter was installed in periprosthetic tissue for the wound irrigation using anesthetic (ropivacaine) ozonated solution for regional prolonged anesthesia and wound complications prevention as well as for preoperative preparation and intra-abdominal pressure monitoring for abdominal compression syndrome prevention using the technique developed by authors. The comparison group consisted of 51 patients in whom prevention of wound complications was carried out conventionally, and who received narcotic analgesics. Ultrasonography of postoperative wound area was used for local inflammation intensity assessment. Pain was assessed using numeric visual analogue scale in both groups. Results. 4 (15.4%) patients of the main group developed subcutaneous tissue seroma at the 5-6th day. In comparison group wound complications were registered in 15 (29.4%) patients, including 11 cases of seromas, 2 cases of suppurative inflammation, 1 case of ischemic necrosis of the wound edges, and also an isolated case of limphorrhea which occurred on the first day. The observations showed that the average pain level among patients of the main group was 3.8±0.4 points, compared to the control group - 5.3±0.4 points. Conclusion. Periprosthetic irrigation with ropivacaine ozonated solution provides adequate anesthesia, reduces the number of wound complications and promotes early rehabilitation.
Objective: to assess the quality of life of patients and the state of the hepatopancreatobiliary system before and after cholecystectomy in the long-term period. Materials and methods: at the first stage, 107 people were examined after emergency, planned cholecystectomy (with symptomatic and asymptomatic course of GI). At the second stage, 90 people at different times after cholecystectomy for a more detailed examination of the external secretory function of the pancreas was performed. Results: the majority of patients, regardless of the type of surgery, had symptoms of dyspepsia associated with the development of functional disorders of the gastrointestinal tract (gastroesophageal reflux, duodenogastric reflux, sphincter Oddi dysfunction). A decrease in the external secretory function of the pancreas was shown regardless of the period after cholecystectomy. Conclusion: after cholecystectomy, functional disorders of the digestive system predominate in the long-term period due to loss of the physiological function of the gallbladder.

Несомненным шагом вперед явились использование современных лучевых методов исследования и двухэтапная тактика лече-ния, когда вначале производится декомпрессия жел-чевыводящей системы с помощью малоинвазивных методик, а затем выполняется (если это возможно) радикальная операция [1][2][3][6][7][8]. Тем не менее диа-гностические и тактические ошибки, допускаемые на всех этапах лечения больных, продолжают оста-ваться основной причиной летального исхода [4].Цель исследования -улучшение результатов лечения пациентов с механической желтухой путем использования рациональной диагностики и такти-ки лечения. Материал и методыПроанализированы результаты лечения 820 боль-ных в возрасте от 17 до 89 лет с синдромом механиче-ской желтухи, которые были госпитализированы по экстренным показаниям либо переведены из других лечебных учреждений в хирургические отделения №1 и 2 городской клинической больницы №7 Каза-ни с 2004 по 2014 г. Цель исследования. улучшение результатов лечения больных с механической желтухой путем использования рациональной диагностики и тактики лечения. Результаты исследования. Диагностические и тактические ошибки, допущенные на этапах оказания медицинской помощи имели место у 143 (17,4%) пациентов на догоспитальном этапе, на госпитальном у 105 (12,8%), из них в 53 (6,5%) случаях ошибки имели сочетанный характер (встречались на двух этапах). Ретроспективный анализ тяжелых послеоперационных осложнений и летальных исходов у больных с механической желтухой показал, что в 23,8% случаев они были обусловлены диагностическими и тактическими ошибками, допущенными на различных этапах обследования и лечения. Заключение. Для снижения числа диагностических и тактических ошибок предложен алгоритм ведения пациентов при синдроме механической желтухи, направленный на повышение информативности обследования, снижение частоты диагностических и тактических ошибок. Это позволило уменьшить количество послеоперационных осложнений до 16,5%, а летальность -до 3,0%. Retrospective analysis of severe postoperative complications and lethal outcomes in patients with obstructive jaundice showed that in 23.8% of cases they were explained by diagnostic and tactical mistakes at various stages of examination and treatment. Сonclusion. We developed an algorithm for obstructive jaundice management to reduce the number of diagnostic and tactical errors, a reduction in the frequency of diagnostic and tactical errors. it reduced the number of postoperative complications up to 16.5% and mortality rate to 3.0%.
