The aim of this study was to investigate the impact of vaginal and abdominal hysterectomy on women's sexual behavior, sexual dysfunction, body image and satisfaction with surgery. A prospective study was conducted on 90 women to evaluate the outcomes of hysterectomy. Data were collected prior to surgery, three months and two years after surgery, using self-report questionnaires. The results showed significant differences in women's sexual behavior and sexual dysfunction before and after hysterectomy, independent of the surgical procedure performed. Women in both groups reported improvements in sexual desire, sexual activity and sexual intercourse three months and two years after surgery. Sexual dysfunction such as dyspareunia, vaginismus, lack of orgasm and loss of sexual interest diminished significantly after surgery. Regression analyses revealed that postmenopausal status, severity of gynecological complaints and frequency of sexual intercourse were the most important factors for improved sexual outcomes. Women in the abdominal group were dissatisfied with their body image because of the abdominal scar, experienced more pain and had a longer period of recovery from surgery compared to women in the vaginal group. According to the results, sexual behavior alone is not an important factor in choosing vaginal or abdominal hysterectomy. However, sexual behavior was important in both groups when evaluating outcomes after hysterectomy.
We report on an 80-year-old female patient with osteoporotic collapse of the L1 vertebra who developed absolute stenosis of the spinal canal. During preoperative vertebra augmentation with PMMA (polymethylmethacrylate), pulmonary embolism developed due to perivertebral leakage of acrylic cement. According to the literature, leakage of PMMA during (percutaneous) vertebroplasty is a more frequent complication than assumed so far. In view of the favorable prognosis of osteoporotic vertebral compression fractures with noninvasive management, we emphasize the need for clear and individual indications and adequate technical settings.
Undislocated odontoid fractures may lead on the basis of conventional x-rays only to a wrong conclusion with regard to biomechanical aspect of stability. In this aspect the classification based on Anderson and D'Alonzo takes a high risk to misunderstand the fracture stability and can results in a secondary fracture dislocation. Therefore it is important to make the decision about operative versus nonoperative treatment on the base of the trauma mechanism. In this case report we elucidate this problem and the higher risk of anterior approach for correction. Furthermore a better classification of dens fractures will be recommended.
Zusammenfassung Hintergrund Physiotherapeuten behandeln meist fremde Personen, häufig mit Körperkontakt. Dies kann einen erheblichen Stressfaktor darstellen. Der Kohärenzsinn wird als Bewältigungsressource gesehen, um psychisch und physisch gesund zu bleiben. Ziel Die Ausprägung von psychischer Belastung, emotionaler (Sympathie, Antipathie), räumlicher und kultureller Nähe und Distanz in der Therapie wird aufgezeigt und in Relation zum Kohärenzsinn gebracht. Die Vorhersage der psychischen Belastung dient zur Entwicklung von gesundheitserhaltenden Strategien. Methode 10 Vignetten zu typischen Situationen in der Einzeltherapie wurden erstellt, um Nähe und Distanz zu untersuchen. Die Einschätzung von psychischer Belastung, emotionaler, räumlicher, kultureller Nähe und Distanz erfolgte auf einer Skala von 0 – 10. Zur Messung des Kohärenzsinns wurde der Fragebogen SOC-29 vorgegeben. Ergebnisse Insgesamt wurden 51 Physiotherapeuten und 102 Physiotherapeutinnen mit durchschnittlich 8,69 Jahren Berufserfahrung untersucht. Über alle Vignetten wurden 7 Skalen gebildet: psychische Belastung, Sympathie, Antipathie, räumliche und kulturelle Nähe und Distanz. Die am höchsten eingestuften psychischen Belastungen waren unfreundliche Patienten, unangenehmer Körpergeruch, unerwünschtes Eindringen in die Privatsphäre und unerreichbarer Therapieerfolg. Eine höhere psychische Belastung steht im Zusammenhang mit niedrigerem Kohärenzsinn, Partnerschaft mit Kindern, stärkerer Antipathie, kultureller Nähe und kürzerer Berufstätigkeit. Schlussfolgerungen Aus der Nähe- und Distanzproblematik bei Physiotherapeuten resultierende psychische Belastung und geringer Kohärenzsinns als Bewältigungsressource hängen stark zusammen. Um die psychische Belastung gering zu halten, werden institutionelle und individuelle Strategien diskutiert.
Die vorliegende Studie untersuchte die Wirkung der Implementierung von AGnES (Arztentlastende, Gemeindenahe, E-Health gest?tzte, Systemische Intervention)-Fachkr?ften und hier im Besonderen der ?Mobilen Praxisassistentin (MoPra)? in l?ndlichen Regionen von Sachsen-Anhalt. Es wurde angenommen, dass sich die Lebensqualit?t und der psychische und physische Gesundheitszustand von neu diagnostizierten T2DM-Patienten, wenn sie 6 Monate von einer MoPra betreut werden im Vergleich zu einer Gruppe ohne MoPra-Betreuung verbessert und mit den Patienten gemeinsam gesundheitsbezogene Versorgungsdefizite abgebaut werden k?nnen. Es wurden der SF-36 zur ?berpr?fung der psychischen und physischen Gesundheit, der WHOQUOL-Bref zur Messung der Lebensqualit?t kurz nach der Diagnose und 6 Monate danach vorgegeben. Die Ergebnisse zeigten, dass durch die kontinuierliche Betreuung, Beratung und Anleitung der T2DM-Patienten durch MoPras eine wesentliche Verbesserung und Stabilisierung des selbst berichteten Gesundheitsstatus und der Lebensqualit?t herbeigef?hrt und kritische Parameter, wie der HbA1c und der BMI verbessert werden konnten. Gleichzeitig setzte bei der Mehrheit der Patienten die von den MoPras betreut wurden, eine Verhaltens?nderung ein, die auf eine verbesserte Selbstregulation im Umgang mit dem Diabetes hinweist.
